PURPOSE: Transdural supply (TDS) is a recognized angioarchitectural feature of brain arteriovenous malformations (bAVMs). We aimed to systematically characterize the angioarchitecture of TDS in bAVMs, focusing on whether transdural feeders terminated within the nidus or connected directly to the draining vein. TDS prevalence and its associated clinical features were also investigated.
METHODS: This retrospective study enrolled 521 patients (524 bAVMs) from 16 centers who underwent systematic six-vessel digital subtraction angiography. bAVMs were classified as nidus- or fistula-dominant. Angio

Cholesterol granulomas are cystic lesions that typically arise in the petrous apex as a result of an inflammatory giant-cell reaction to cholesterol crystal deposits that are formed when normal aeration and drainage of temporal bone air cells become occluded resulting in transudation of blood into the petrous air cells. Surgical strategies include simple cyst decompression, radical excision of the cyst wall, or fenestration and drainage with silastic tubing. The authors present a giant cholesterol granuloma compressing the cerebellopontine angle and brainstem in a 35 year-old male who presente

A simple iontronic memtransistor capable of emulating synaptic plasticity and cognitive functions showcases high performance and low energy consumption, fostering admiration for its efficiency. Here, we report the versatile, high-performance memtransistor behavior of a solid polymer electrolyte-gated few-layer thick two-dimensional molybdenum disulfide (2D MoS2) channel. Transfer characteristics exhibit pinched hysteresis, confirming n-channel enhancement-mode operation, supported by the low-voltage drain characteristics under the influence of the electrical double layer (EDL) formed by iontro

Traditional neuroanatomy long asserted the absence of lymphatic circulation in the central nervous system, contradicting its high metabolic activity and efficient waste removal. This paradox was resolved in the 21st century by Iliff et al., who discovered the glymphatic system (GS)-a glia-dependent network enabling cerebrospinal fluid to flow through periarterial spaces, clear interstitial waste via paravenous pathways, and drain into peripheral circulation through meningeal/cranial lymphatic routes. Beyond waste clearance, the GS mediates nutrient distribution and neuroactive signaling, funda

Infective Endocarditis is characterized by inflammation of the heart's inner lining and valves which can lead to valve masses made of fibrin, platelets, and the infectious organisms. Patients can present with a wide array of symptoms with neurological symptoms quite rare. Here, we present a case of a 41-year-old who was admitted for IE and later developed an intraparenchymal hemorrhage. A Transesophageal echo (TEE) showed > 2 cm tricuspid valve vegetation and small mitral valve vegetation. After the patient developed confusion an urgent head CT showed a large acute left frontal intra-parenchym

PURPOSE OF REVIEW: The aim of this study was to highlight clinical considerations regarding medical versus surgical management of elevated intracranial pressure (ICP), describe limitations of medical management, and summarize evidence regarding timing of neurosurgical interventions.
RECENT FINDINGS: The optimal ICP management strategy remains elusive, and substantial practice variations exist. Common risks of medical treatments include hypotension/shock, cardiac arrhythmias and heart failure, acute renal failure, volume overload, hypoxemia, and prolonged mechanical ventilation. In traumatic br

BACKGROUND: Negative-pressure hydrocephalus is characterized by neurological deterioration with ventriculomegaly but negative intracranial pressure (ICP), clinically indistinguishable from acute hydrocephalus with elevated ICP. Subatmospheric CSF drainage, neck wrapping, endoscopic third ventriculostomy (ETV), and shunt placement are treatment methods. Medical therapy remains elusive. The authors describe a patient with refractory negative-pressure hydrocephalus who failed conventional therapies but responded robustly to theophylline.
OBSERVATIONS: A 30-year-old man with traumatic brain injur

We report a rare case of cefepime-induced encephalopathy (CE) after pancreaticoduodenectomy. An 83-year-old female patient underwent pancreaticoduodenectomy for a distal bile duct tumor. On postoperative day (POD) 6, she developed a pseudoaneurysm secondary to postoperative pancreatic fistula, for which interventional radiology (IVR) treatment was performed. A liver abscess developed after IVR treatment on POD 15. Multidrug-resistant P. aeruginosa was detected in a drain culture, prompting a change of antibiotic therapy to cefepime on POD 30. Antifungal therapy was initiated for fungemia; howe

This report presents an 89-year-old patient with subacute subdural hematoma (SDH) treated with local anesthesia via burr-hole trephination (BHT) and subdural drain placement, followed by liquefaction of the hematoma using the fibrinolytic agent, tissue plasminogen activator (tPA). Initially, the patient presented with acute post-trauma SDH without neurological symptoms, for which conservative treatment was administered. About a week later, the patient developed hemiplegia and progressed to a stuporous state. Radiographic evidence indicated that the subacute SDH had evolved into a chronic stage

We present the case of a 17-year-old male, who complained of a 1-year onset of pulsatile headache, dysphagia, speech changes, and emotional lability. Neuroimaging revealed a large left-sided contrast-enhancing tumor located at the infratentorial space consistent with a large trochlear nerve schwannoma. The tumor was compressing the brainstem, obstructing the outflow of the third and lateral ventricles causing hydrocephalus, and disturbing the cortico-bulbar pathways bilaterally leading to the diagnosis of pseudobulbar palsy. After the patient consented the surgical procedure, he was operated t

AIM: This research aimed to explore how servant leadership nurtures nurses' job embeddedness by uncovering the sequential mediation of psychological contract fulfillment and psychological ownership.
BACKGROUND: The healthcare of Pakistan is undergoing an acute shortage of 1.3 million nurses. The gap is widening due to unprecedented natural uncertainties (floods, earthquakes, COVID-19, dengue, polio, and monkeypox) and the large-scale brain drain of nurses. Therefore, exploring the underlying factors that could facilitate nurses' job embeddedness is imperative.

Posterior reversible encephalopathy syndrome (PRES) refers to white matter vasogenic edema primarily affecting the brain's posterior occipital and parietal lobes, causing acute neurological symptoms like headaches, visual symptoms, seizures, and altered mental status. We present the case of a 32-year-old male with uncontrolled hypertension, altered mental status, and left-sided weakness. He had a rapid neurological decline, and a computed tomography (CT) head showed blurring of gray-white matter interfaces in the right posterior parietal lobe, suggesting infarction or PRES. Magnetic resonance

PURPOSE: Chronic hydrocephalus is rarely seen in developed countries due to the widespread availability of CT scans for diagnosis and early treatment. In developing countries, it is more frequently encountered along with its rare complication of diverticulum formation.
METHODS: We report a case of a previously healthy 6 year old girl who presented with a 10 day history of headache, 3 day history of drowsiness and a single episode of generalized tonic clonic seizures. We review the literature on such cases and include imagining studies at follow up.
RESULTS: A CT scan done outside our hospita

BACKGROUND: Intracranial aneurysms are common vascular malformation occurring in 1-2% of the population and accounting for 80-85% of nontraumatic subarachnoid hemorrhages. About 10% of the ruptured aneurysm causing subarachnoid hemorrhage (SHA) develop intraventricular hemorrhage (IVH). In this scenario, the external ventricular drain (EVD) is a usual treatment for IVH. To reduce the time for the clot absorption, the neuroendoscopy with clot removal and ventricular irrigation is a feasible option, although not routinely used.

OBJECTIVE: Supradiaphragmatic hematoma is a type of hematoma that occurs after transsphenoidal (TS) resection of pituitary adenoma and requires special management.
METHODS: Two patients had symptomatic supradiaphragmatic hematomas after total TS resection of pituitary adenomas in the absence of vascular anomalies. Both patients also had hydrocephalus at the time of diagnosis of the hematoma. The initial endoscopic endonasal inspection showed no subdiaphragmatic bleeding. The hematoma was evacuated via a frontolateral approach after insertion of an external ventricular drain (EVD).

Surgical treatment of dystonia has experienced a tremendous change over the past decade. Whilst selective peripheral denervation is reserved for cervical dystonia refractory to botulinum toxin injections, deep brain stimulation (DBS) of the pallidum has gained a wide scope and presents an elementary column in the treatment of medically refractory patients, nowadays. There is consensus that idiopathic generalized, cervical and segmental dystonia are good indications for DBS, although there is still a paucity of studies providing high-level data according to EBM criteria. Efficacy is maintained
