Retrograde (transseptal) activation of right bundle branch during sinus rhythm.

A 40-year-old man undergoing electrophysiologic study for recurrent palpitations was found to have a right bundle branch block during sinus rhythm and right bundle potentials occurring after ventricular activation. The sequence of activation of the proximal and distal right bundle potentials was suggestive of a retrograde invasion of the right bundle during sinus rhythm. This transseptal activation of the right bundle via the left bundle exhibited decremental conduction characteristics during atrial as well as ventricular stimulation. Observations made in this case may provide some important i

[Importance of PQ time in the evaluation of bifascicular block].

In 103 patients with bifascicular block was tested by means of the His bundle electrography, whether with the help of the PQ-time an indirect estimation of the conduction capacity of the still conducting fascicle (HV-interval) is possible. In 70% of the cases with bifascicular blocking and AV-block of first degree the HV-interval was prolonged. However, also 45% of the patients with bifascicular block showed a prolonged HV-interval despite normal PQ-time. Of 21 patients with syncopes due to an intermitting trifascicular block 10 had a normal PQ-time in phases of the transduction. Thus a normal

Tachycardia and bradycardia-dependent bundle branch block alternans: clinical observations.

Eleven patients with tachycardia-dependent, bradycardia-dependent, or "pseudobradycardia-dependent" bundle branch block (BBB) alternans were studied. This classification is based on the following criteria: 1) When alternans is initiated by a sudden acceleration in ventricular rate, or it appears with aberration of the second beat after a pause, the alternans is tachycardia-dependent and results from a 2:1 bidirectional block in the affected bundle branch. 2) When alternans begins with the aberrant complex terminating a pause it is bradycardia-dependent; such an alternans results from alternati

Second-degree atrioventricular block in the His-Purkinje system following acute myocardial infarction. Clinical observations on its evolution.

The evolution of second-degree atrioventricular block in the His-Purkinje system was studied in four patients with acute myocardial infarction, utilizing continuous electrocardiographic monitoring and serial His bundle recordings. Three patients had anterior wall infarction; all developed acute right bundle-branch block and two also showed a left anterior hemiblock one to four hours prior to onset of second-degree atrioventricular block. The fourth patient had an old anteroseptal infarction and chronic right bundle-branch block and left anterior hemiblock; he developed second-degree atrioventr

Sex-Based Differences in Left Bundle Branch Capture During Left Bundle Branch Area Pacing.

OBJECTIVE: To identify sex-based differences in meeting left bundle branch (LBB) capture criteria on postimplant ECGs. BACKGROUND: Despite differences in QRS duration (QRSd) between males and females, no sex-based criteria have been established for defining LBB capture during LBB area pacing (LBBAP). METHODS: Patients aged >18 y.o. with a pacing indication and dual-chamber LBBAP pacemaker implantation from 2020 to 2023 were studied. Baseline and postoperative day 1 (POD1) ECGs were assessed for electrical dyssynchrony (QRS-area, sum-absolute integral of QRS-T (SAI QRS-T)), with LBB capture d

[Physiological pacing and cardiac resynchronization therapy: state of the art].

Permanent cardiac pacing has progressively evolved toward increasingly physiological strategies aimed at preserving or restoring ventricular electromechanical synchrony. Cardiac resynchronization therapy using biventricular pacing remains the established standard of care for patients with heart failure, reduced left ventricular ejection fraction and electrical dyssynchrony, particularly in the presence of left bundle branch block. However, anatomical constraints, suboptimal clinical and echocardiographic response, and procedural complexity have promoted the development of conduction system pac

Left bundle branch pacing versus left ventricular septal pacing as a primary procedural endpoint during left bundle branch area pacing: Evaluation of two different implant strategies.

INTRODUCTION: Implant procedure features and clinical implications of left bundle branch pacing (LBBP) and left ventricular septal pacing (LVSP) have not been yet fully described. We sought to compare two different left bundle branch area pacing (LBBAP) implant strategies: the first one accepting LVSP as a procedural endpoint and the second one aiming at achieving LBBP in every patient in spite of evidence of previous LVSP criteria. METHODS: LVSP was accepted as a procedural endpoint in 162 consecutive patients (LVSP strategy group). In a second phase, LBBP was attempted in every patient in s

Characteristics of intracardiac electrogram of the interventricular septum in the left bundle branch pacing.

BACKGROUND: Left bundle branch pacing (LBBP) has become a hot topic in the field of physiological pacing. However, only a few studies have described the characteristics of the intrinsic intracardiac electrogram (EGM) while placing the left bundle branch (LBB) lead. CASE PRESENTATION: Herein, we reported a case with atrial premature contractions to the ventricle during the LBBP procedure. Paced and intrinsic (supraventricular) EGMs were recorded and analyzed. CONCLUSIONS: The myocardium of the interventricular septum could be divided into four regions based on electrophysiology: the right sep

Assessment of the physical performance in children with asymptomatic pre-excitation.

AIMS: Pre-excitation syndrome can lead to recurrent supraventricular tachycardias (SVTs) and carries a risk of sudden cardiac death (SCD). However, an underestimated consequence of antegrade conduction through an accessory pathway is fusion of intrinsic and accessory conduction that causes asynchronous activation and myocardial contraction that could be a cause for cardiac dysfunction and dilation. It is not known to what extent pre-excitation affects myocardial and physical performance in those patients. The aim of the study was to assess to what degree ventricular pre-excitation affects phys

Can we improve the accuracy of electrocardiographic algorithms for accessory pathway location in children?

BACKGROUND: Predicting an accessory pathway location is extremely important in pediatric patients. AIMS: We designed a study to compare previously published algorithms by Arruda, Boersma, and Chiang. METHODS: This multicenter study included patients who had undergone successful ablation of one accessory pathway. Analysis of resting 12-lead electrocardiograms was carried out. An aggregated prediction score was constructed on the basis of algorithm agreement, and a structured workflow approach was proposed. RESULTS: The total population was 120 patients (mean age, 12.7 [± 3.6] years). The alg

Left bundle branch pacing shortened the QRS duration of a right bundle branch block.

Left bundle branch pacing (LBBP) has recently emerged as a novel physiological pacing technique with a paced morphology of a pseudoright bundle branch block (RBBB). We herein present a 63-year-old man with a high-degree atrioventricular block and complete RBBB, whose intrinsic QRS duration and terminal R' wave duration in V1 were significantly shortened after LBBP and further shortened with the increase in output.

[Ablation of a para-Hisian accessory pathway with high-power short-duration].

We report the case of a 30-year-old man who presented to our emergency department with rapid heart beat and a narrow complex tachycardia at a rate of 215 beats per minute. With the working diagnosis of Wolf-Parkinson-White syndrome with paroxysmal orthodromic tachycardias, we performed an electrophysiological study. In this examination, there was a para-Hisian accessory pathway with very fast, prognostically relevant conduction properties. In order to protect the His bundle that is located deeper in the tissue and to avoid a deeper lesion, we decided to use high-power short-duration ablation.
