A novel large animal model of smoke inhalation-induced acute respiratory distress syndrome.

BACKGROUND: Acute respiratory distress syndrome (ARDS) is multifactorial and can result from sepsis, trauma, or pneumonia, amongst other primary pathologies. It is one of the major causes of death in critically ill patients with a reported mortality rate up to 45%. The present study focuses on the development of a large animal model of smoke inhalation-induced ARDS in an effort to provide the scientific community with a reliable, reproducible large animal model of isolated toxic inhalation injury-induced ARDS. METHODS: Animals (n = 21) were exposed to smoke under general anesthesia for 1 to 2

Application of flexible bronchoscopy in inhalation lung injury.

BACKGROUND: As acute inhalational injury is an uncommon presentation to most institutions, a standard approach to its assessment and management, especially using flexible bronchoscopy, has not received significant attention. METHODS: The objective of this study is to evaluate the value of using flexible bronchoscopy as part of the evaluation and management of patients with inhalational lung injury. Twenty-three cases of inhalational lung injury were treated in our three hospitals after a fire in a residential building. The twenty cases that underwent bronchoscopy as part of their management a

Problems associated with the diagnosis of vitality in burned bodies.

The most important signs of vitality in burned bodies are soot deposits in the respiratory tract, the esophagus and the stomach as well as elevated CO-Hb values in the blood. But these findings show only that a person was exposed to fire fumes while alive; they are no indicator for vital heat exposure. As the external findings in burned bodies are often not very conclusive due to postmortem consumption by the fire, the internal findings are all the more important. Macroscopic signs that hot fumes were inhaled may be edematous swelling and vesicular detachment of the mucosa in the pharynx, the

Predicting High Variable Direct Cost in Burn Care: A Comparison of Logistic and Random Forest.

INTRODUCTION: Burn care is uniquely resource-intensive, and early identification of patients at risk for high hospital-incurred costs may enable proactive care coordination and more efficient resource allocation. This study uses the actual hospital cost data, rather than indirect billing metrics, to develop early prediction models for high-cost burn admissions. METHODS: We performed a retrospective cohort study of 363 adult and pediatric burn patients admitted to a regional burn center between 2022 and 2023. High variable direct cost was defined as variable direct cost exceeding the 75th perc

The anticatabolic and wound healing effects of the testosterone analog oxandrolone after severe burn injury.

PURPOSE: Severe burn injury leads to marked catabolism and decreased lean mass, which can impair healing. Anabolic agents can attenuate net catabolism. Our purpose was to determine whether the testosterone analog, oxandrolone, given during the acute post burn period decreased the degree of nitrogen loss and loss of body weight while also increasing the healing rate of a skin donor site. MATERIALS AND METHODS: Patients with burns between 40% and 70% of body surface were studied. A randomized double-blinded placebo-controlled study design was used. Patients were given oxandrolone 20 mg/day (n =

Inhalation injury, pulmonary perturbations, and fluid resuscitation.

Inhalation injury (INHI) associated with thermal injury has been shown to increase the rate of mortality. Several investigators have shown that patients with inhalation and burn injuries will require increased fluid volumes during acute resuscitation when compared with patients with burn injury alone. Other groups have examined the use of lung compliance and airway resistance as predictors of outcome in patients with INHI. We hypothesized that increased fluid requirements would more closely correlate with perturbations in pulmonary performance than with mere presence or absence of INHI or the

Chemical pulmonary burn injury and chemical intoxication.

Two large groups of patients with chemical pulmonary burns who sustained their injuries in the same environment were presented. Data from the blaze smoke analysis of two imitative experiments which were performed under the same conditions as were present when these patients sustained their burns showed a high concentration of carbon monoxide of 35700-571400 ppm and multivariable components of chemicals. The environmental conditions, the clinical course, laboratory findings and blaze analysis clearly demonstrated the close relationship between chemical pulmonary burn injury and chemical intoxic

Successful organ transplantation after treatment of fatal cyanide poisoning with hydroxocobalamin.

BACKGROUND: Cyanide-poisoned patients are potential organ donors provided that organs are not damaged by the poison or by antidotal treatment. CASE STUDY: A patient with third-degree burns and smoke inhalation-associated cyanide poisoning confirmed by measurements of whole blood cyanide was found in cardiac arrest and administered epinephrine and hydroxocobalamin (5 g + 5 g). Cardiac activity resumed, but the patient was declared brain dead on the third day of hospitalization when coma deteriorated to a shock state with refractory hypoxemia. Kidneys, heart, and liver were removed and transpla

Awake Tracheostomy in the Sitting Position for a Predicted Difficult Airway in Ankylosing Spondylitis With Inhalation Injury: A Case Report.

BACKGROUND: Airway management in patients with inhalation injury is particularly challenging because of rapidly progressive airway edema. This difficulty is further compounded in patients with ankylosing spondylitis, in whom cervical spine rigidity and deformity severely restrict neck mobility and may prevent standard positioning for airway interventions. In such situations, conventional airway management techniques may fail, necessitating alternative strategies. CASE PRESENTATION: We report the case of a 48-year-old man with inhalation injury following a flame burn who also had long-standing

Identifying Risk Factors for Inpatient Falls in Burn Patients: A Retrospective Cohort Analysis.

Inpatient falls are associated with prolonged hospital stays, increased complications, and substantial economic burden. Burn patients face unique risks due to impaired mobility. Yet, risk factors and outcomes in burn units remain underexplored. This retrospective cohort study evaluated the impact of inpatient falls on adult burn outcomes. Predictors of fall risk to guide prevention strategies were identified. Adult patients admitted for burn injuries from January 1, 2015, to July 31, 2024, were included. Fallers were identified through an internal auditing system and matched 1:2 to non-fall co

Fungal infections in burns: Diagnosis and management.

Burn wound infection (BWI) is a major public health problem and the most devastating form of trauma worldwide. Fungi cause BWI as part of monomicrobial or polymicrobial infection, fungaemia, rare aggressive soft tissue infection and as opportunistic infections. The risk factors for acquiring fungal infection in burns include age of burns, total burn size, body surface area (BSA) (30-60%), full thickness burns, inhalational injury, prolonged hospital stay, late surgical excision, open dressing, artificial dermis, central venous catheters, antibiotics, steroid treatment, long-term artificial ven…

[The tresh dust lung syndrome in children].

Farmer's lung, caused by the inhalation of microspores--particularly of the genus, Thermoactinomyces--has been recognised for the past 30-40 years as a condition affecting adult farm workers, especially up to the mid-sixties when undried crops were still threshed indoors. Although it used to be extremely rare in children, between 1985 and 1989 occurred five of the seven cases reported hitherto. The possibility is suggested that with the increasingly large scale use of grain drying plant and stubble burn-off on Danish farms, we shall see an increase in the incidence of the disease, even in infa

Household inhalants exposure and nasopharyngeal carcinoma risk: a large-scale case-control study in Guangdong, China.

BACKGROUND: Epidemiological studies show that cigarette smoking increase the risk of nasopharyngeal carcinoma (NPC), however, whether other common, potentially adverse household inhalants increase NPC risk remains uncertain. METHODS: We conducted a large case-control study to explore the effects of household inhalants, such as incense, mosquito coil, cooking fumes, and wood combustion, on NPC risk. We recruited 1,845 cases and 2,275 controls from Guangdong province, a high-risk area for NPC in China, to obtain the demographic data and relevant exposure information through face-to-face intervi

In vitro study of injury on human bronchial epithelial cells caused by gunpowder smog.

Smog inhalation is associated with acute respiratory symptoms in exposed victims. However, despite the evidence from cell injury caused by smog, a stable and practical apparatus used to treat cells with smog is necessary. The aim of this study is to develop a cell research platform of smoke inhalation injury. In the smog-generation device, a wireless electromagnetic heater was used to ignite gunpowder and generate smog. The quality of black powder was checked by the black powder burn rate, and experimental smog was indirectly checked by the amount of cell damage. The temperature and humidity w

Effect of autologous fat transfer in acute burn wound management: A randomized controlled study.

OBJECTIVE: The use of fat grafting is being widely used for different indications one of which is wound healing. In this study we compare the use of autologous fat grafting (AFG) as a novel indication in acute burn wounds healing and burn scarring to the conventional methods of burn wound management both clinically and histologically. Several small observational studies demonstrated the effect of the AFG in healing of chronic wounds, different vascular ulcers or effect on scars yet no randomized controlled trial is available to compare its role with conventional methods. METHODS: The study wa

[Follow-up of patients with burn injuries following long-term ventilation].

Follow-ups were performed five years post burn on 13 severely burned patients after long-term ventilation in order to identify damage to the trachea and lung. The study comprised clinical examination, tracheal X-rays, pulmonary function tests with whole-body plethysmography for thoracic compliance, and spiroergometry. Tracheal stenosis was detected in five patients (38%), two patients had reduced total lung capacity and functional residual capacity was increased. Six patients had high values of pulmonary resistance. The comparison between actual and predicted physical work capacity in the spir
