The cervicocranium: its radiographic assessment.

Acute injuries of the cervicocranium (from the occiput to the second cervical intervertebral disk) may be radiographically obscure due to minimal displacement of fracture fragments, minor alterations of normal anatomic relationships (occipitoatlantal subluxation), or superimposition of normal skeletal structures. With the nasooropharynx adequately distended with air, the normal cervicocranial prevertebral soft-tissue contour is congruent with the anterior cortical margin of the cervicocranium; namely, concave above, convex anterior to, and concave below the anterior tubercle of C1. Alterations

Longterm follow-up of posterior cervical fusion for atlanto-axial subluxation in rheumatoid arthritis.

Surgical fusion of the upper cervical spine was used to treat atlanto-axial subluxation in 7 patients with rheumatoid arthritis. Postsurgical follow-up was 3 to 16 yr. Despite progressive and often severe radiological changes in the cervical spine, no patient has had a clinical recurrence of neck symptoms requiring further surgery. These results suggest that surgical fusion is successful in the longterm, despite the often dramatic radiological changes which occur in the cervical spine with the passage of time.

The biomechanical study of different fixation techniques for combination fractures of atlas and axis: a finite element analysis.

Combination atlas-axis fractures are less studied but relatively common with a higher incidence of neurological deficits than isolated C1 or C2 fractures. Several authors focused on the treatment strategies, but there is no study to compare the stability of different fixation methods; neither not yet clear which technique represents the best choice and whether stabilization devices can be efficient and beneficial for complex atlantoaxial fractures. The aim of this study was to compare the biomechanical properties of three fixation techniques: atlantoaxial pedicle screws fixation (PSF), occipit

Rare anatomic variation on a 10,000-year-old South American skeleton: the case of Iuzu, Toca dos Coqueiros, Piauí State, Brazil.

PURPOSE: A skeleton named Iuzu has been unearthed from an exceptional middle Holocene burial in Toca dos Coqueiros site, in Serra da Capivara National Park (UNESCO World Heritage Site, Piauí State, Brazil). During a bioarchaeological analysis of its remains, we discovered that Iuzu was suffering from rare vertebral malformations. A double foramen transversaria, the agenesis of a foramen on the atlas and the hypoplasia of the transverse process of the axis have been highlighted. We aimed to deduce the clinical consequences of the malformation on the patient's health. METHODS: We proceeded to m

Fractures of the dens in adult patients.

The series involves 42 consecutive acute fractures of the dens, of which 41 were treated nonoperatively. There were 26 Type II and 16 Type III fractures, as classified by Anderson and D'Alonzo. All Type III fractures and undisplaced or anteriorly displaced Type II fractures united. Posteriorly displaced Type II fractures had an overall nonunion rate of 50%, which increased to 75% if displacement was more than 5 mm. Eight patients died within 40 days after sustaining their fracture; seven had Type II fractures with an average of 10-mm posterior displacement. Seven patients with a Type II fractu

[Fractures of the cervical spine].

A gentle traction is often sufficient to repose unstable lesions of the cervical spine, when a retention by means of conservative methods is not possible. Therefore a ventral, intracorporal fusion with autologous, corticospongious chip material combined with a plate fixation is an adequate solution to obtain a stable, movable blocking of the lower six cervical vertebrae. In case of a completely ruptured dorsal ligamentous apparatus, a ventral bone-chip blocking is generally insufficient. Ventral spondylodesis should be combined with a dorsal traction belt in these cases. Fresh, instable fractu

Traumatic basilar impression: case report.

A very rare case of traumatic basilar impression is reported. The patient, a 57-year-old man, was hit on the head vertically in the parietal region. X ray of the cervical spine and computed tomography (CT) scans showed intracranial indentation of the atlas and the odontoid process with a depressed fracture around the foramen magnum. There are no previous reports about this type of fracture.

Unusual mechanism of lethal cervical spinal cord injury in a case of atlanto-axial diastasis.

A case of traumatic cervical spinal cord injury due to congenital craniocervical abnormalities is described. The autopsy revealed evidence of hyperextension of the neck due to frontal impact, congenital occipitalization of the atlas accompanied by basilar invagination of the odontoid process and atlanto-axial diastasis. The subsequent narrowing of the spinal canal led to a predisposition for this unusual mechanism of spinal cord injury.

[Anatomical basis for a new method of atlas osteosynthesis in unstable C1 vertebra fractures].

BACKGROUND: Surgical treatment for unstable atlas fractures is still controversial. Harm's atlantoaxial fusion or more extensive occipitospondylodesis reduces quality of life and functional status. Biomechanical rationales for anterior and posterior C1 osteosynthesis are based on small sample, and the role of ligamentous complex in C1-C2 segment remains understudied. OBJECTIVE: To evaluate the effectiveness of a new method of surgical treatment for unstable fractures of the first cervical vertebra. MATERIAL AND METHODS: Biomechanical analysis enrolled 10 anatomical specimens. Various injurie

[Evidence-based clinical guidelines of acute atlas fractures in adults (2025 edition)].

In 2015, the Spinal Trauma Working Group of the Chinese Association of Orthopaedic Surgeons (CAOS) developed the Evidence-Based Clinical Guidelines of Acute Atlantoaxial Fractures in Adults based on literature published prior to 2014. Over the past decade, extensive research of acute atlantoaxial fractures in adults has been conducted globally, with the number of published studies in Chinese and English exceeding the cumulative total of all previous literature. To address evolving clinical needs and align with evidence-based principles, CAOS initiated a comprehensive revision of the guidelines

Role of Atlas Assimilation in the Context of Craniocervical Junction Anomalies.

Atlas assimilation (AA) may be associated with atlantoaxial dislocation, Chiari malformation (CM), and basilar invagination. The importance of AA in the context of craniocervical junction (CVJ) anomalies is unclear. Considering this context, this study's objective is to discuss the role of AA in the management of CVJ anomalies, especially in CM. A comprehensive literature review was performed. In addition, some illustrative cases were discussed on the basis of our review. Finally, we propose a theoretic algorithm to evaluate patients with AA and CM. AA is a proatlas segmentation anomaly that m

Spontaneous Fractures in the Setting of Extensive Craniocervical Pneumatization: Case Report and Literature Review.

BACKGROUND: Cranial pneumatization in humans is normally confined to the paranasal sinuses and the petrous and mastoid parts of the temporal bones. CASE DESCRIPTION: We present a case of left-sided fractures of the occipital condyle and lateral mass of the atlas in the setting of extensive craniocervical pneumatization but in the absence of trauma, with a resulting unilateral hypoglossal nerve palsy. CONCLUSIONS: We discussed the possible etiology of this rare disease, its management, and prognosis.

Motion-preserving, 2-stage transoral and posterior treatment of an unstable Jefferson fracture in a professional football player.

The authors report the case of a patient who suffered a Jefferson fracture during a professional football game. The C-1 (atlas) fracture was widely displaced anteriorly, but the transverse ligament was intact. In an effort to enable a return to play and avoid intersegmental (C1-2) fusion, the patient underwent a transoral approach for open reduction and internal fixation of the fracture. The associated posterior ring fracture displacement widened after this procedure, and a subsequent posterior arthrodesis and fixation of the fracture site was performed 6 months later when the fracture failed

Coincidental deficiency of the posterior arch of the atlas and thalassaemia minor: possible pitfalls in a trauma victim.

Congenital abnormalities of the cervical spine are rare findings in trauma victims. Deficiency of the posterior arch of the atlas and coincidental thalassaemia minor are even more unusual. This case report is about a young female trauma victim with both abnormalities, a combination that has previously not been described in literature. The classification, as proposed by Currarino et al in 1994, and the importance of being aware of these abnormalities are discussed.
