Electrophysiologic assessment of shoulder girdle weakness in patients with cervical spondylosis: prognostic value of supraclavicular stimulation.

OBJECTIVES: We tested the use of supraclavicular stimulation as a prognostic measure of deltoid function after cervical spondylotic radiculopathy. METHODS: Electrophysiological study consisted of supramaximally stimulating the brachial plexus and recording compound muscle action potentials (CMAPs) from the deltoid in 32 patients with unilateral weakness of 3 months or shorter duration as compared with 69 healthy subjects. RESULTS: Despite considerable individual variability and age-related reduction in amplitude and area of deltoid CMAPs, the side-to-side ratios yielded useful, reproducible

[True neurogenic outlet syndrome: report of 2 cases].

True neurogenic thoracic outlet syndrome is caused by compression of the lower trunk of the brachial plexus usually by a cervical rib, fibrous band or an elongated transverse process of C7. We describe two cases of female patients (23 and 19 years old) with pain in the right superior limb and progressive muscular weakness and atrophy of the intrinsic muscles of hand. Electrodiagnostic studies showed reduced amplitude of compound muscle action potential of median nerve and decreased amplitude of ulnar sensory nerve action potential. Motor and sensory nerve conduction velocities were normal in b

Fatal injuries of the brain stem and/or upper cervical spinal cord in traffic accidents: nine autopsy cases.

Nine forensic autopsy cases were studied. All had injuries of the brain stem and/or upper cervical spinal cord due to traffic accidents. Among the nine subjects, eight were pedestrians and one was a left, front seat occupant of a vehicle. Examination of these 9 cases revealed that three had ponto-medullary avulsion, two had medullary avulsion and the other four had laceration of the upper cervical spinal cord. Atlanto-occipital dislocation was observed in five cases, and a ring fracture around the foramen magnum in the other two cases. Intraventricular haemorrhage, probably due to tears of the

Heuristic map of myotomal innervation in humans using direct intraoperative nerve root stimulation.

OBJECTIVE: Considerable overlap exists in nerve root innervation of various muscles. Knowledge of myotomal innervation is essential for the interpretation of neurological examination findings and neurosurgical decision-making. Previous studies relied on cadaveric dissections, animal studies, and cases with anomalous anatomy. This study investigates the myotomal innervation patterns of cervical and lumbar nerve roots through in vivo stimulation during surgeries for spinal decompression. METHODS: Patients undergoing cervical and lumbar surgeries in which nerve roots were exposed in the normal c

Ultrasound-guided root/trunk (interscalene) block for hand and forearm anesthesia.

BACKGROUND: Historically, the anterolateral interscalene block--deposition of local anesthetic adjacent to the brachial plexus roots/trunks--has been used for surgical procedures involving the shoulder. The resulting block frequently failed to provide surgical anesthesia of the hand and forearm, even though the brachial plexus at this level included all of the axons of the upper-extremity terminal nerves. However, it remains unknown whether deposition of local anesthetic adjacent to the seventh cervical root or inferior trunk results in anesthesia of the hand and forearm. METHODS: Using ultra

Nerve-sparing and individually tailored surgery for cervical cancer.

Cancer of the cervix is the second most common cancer in women worldwide, with about 500,000 new cases and 273,000 deaths reported annually. Ideal surgical management of cervical cancer should reduce early and late morbidity without compromising oncological disease control. Type of surgical radicality in early cervical cancer should be a consequence of exact preoperative and intraoperative assessments of risk factors. During the past 15 years, substantial progress has been made in understanding the neuroanatomy of the autonomic pelvic plexus. This progress has resulted in individually tailored

[Thoracic outlet syndrome].

Thoracic outlet syndrome (TOS) is due to compression/irritation of brachial plexus elements ("neurogenic TOS") and/or subclavian vessels ("vascular TOS") in their passage from the cervical area toward the axilla. The usual site of entrapment is the interscalenic triangle. TOS is a highly controversial subject in regard to its incidence, diagnostic criteria and optimal treatment. Constitutional factors--osseous or more often fibromuscular--and external factors such as trauma predispose to the development of TOS. Various clinical pictures include pain in the cervical region and arm, paresthesias

The pathomechanics of chronic, recurrent cervical nerve root neurapraxia. The chronic burner syndrome.

This study defined chronic recurrent cervical nerve root neurapraxia, the chronic burner syndrome, characterized the clinical findings, and described the responsible pathomechanics. We studied a subset of 55 athletes (mean age, 22 years) for evaluation of recurrent burners. Eleven subjects were professional athletes. The mechanism of injury was extension combined with ipsilateral-lateral deviation in 46 patients (83%). Spurling's sign was positive in 39 patients (70%). Twenty-nine patients (53%) had developmentally narrowed cervical canals, and 48 patients (87%) had evidence of disk disease by

[The management of patients with thoracic outlet syndrome (TOS) and an assistant diagnosis to discriminate between TOS and cervical spondylosis].

Between 1984 and 2007, 20 patients, admitted to our hospital for the treatment of thoracic outlet syndrome (TOS), were analyzed. Of these 20 patients, 2 subsequently entered rehabilitation to strengthen the muscle power surrounding the shoulder, 5 were followed by a brachial plexus anesthesiological block for the purpose of both diagnosis and treatment of TOS, and the other 13 patients underwent a total of 18 surgeries. Surgical methods consisted of a supraclavicular first rib resection with anterior and middle scalenectomy (SCFR + AMS) in 16 surgeries, and a transaxillary first rib resection

Mechanisms of autonomic disturbance in the face during and between attacks of cluster headache.

Lacrimation and nasal secretion during attacks of cluster headache appear to be due to massive trigeminal-parasympathetic discharge. In addition, the presence of oculo-sympathetic deficit and loss of thermoregulatory sweating and flushing on the symptomatic side of the forehead indicate that the cervical sympathetic pathway to the face is injured in a subgroup of cluster headache patients. In this review, it is argued that a peripheral rather than a central lesion produces signs of cervical sympathetic deficit, probably resulting from compression of the sympathetic plexus around the internal c

Perioperative Care of a Six-Year-Old Child With Atlanto-Occipital Dislocation: A Focus on Techniques of Airway Management.

Atlanto-occipital dislocation (AOD), once regarded as a rare and almost universally fatal injury identified primarily at autopsy, is now recognized with increased frequency in surviving trauma patients. Improved diagnostic imaging, coupled with advances across the entire continuum of trauma care, has resulted in the potential survivability of this devastating injury. Despite the high mortality associated with this devastating injury, survival with successful outcome is feasible. Airway management in patients with AOD must balance minimizing cervical spine movement to prevent exacerbation of ne

Hirayama disease: neutral and flexion magnetic resonance imaging and utility of inter-segmental angle of flexion.

PURPOSE: Hirayama disease (HD) is a rare disease that was commonly mis-diagnosed in the past. The importance of neutral and flexion magnetic resonance imaging (MRI) in its accurate diagnosis has been emphasized along with utility of the inter-segmental angle of flexion. Aim of the study was to observe MRI findings of HD in neutral and flexion position and measure the inter-segmental angle of flexion. MATERIAL AND METHODS: Cervical MR images of 17 patients of suspected HD were evaluated retrospectively for loss of attachment (LOA) of posterior dura, lower cervical cord atrophy, T2 hyperintensi

Post-traumatic thoracic outlet syndrome (TOS).

TOS is a compressive non-tumorous syndrome of the brachial plexus. It is possible, however, to consider as TOS the irritative and lesional plexus syndrome following trauma as long as compression (or traction) on the nerves is triggered by long-lasting pathological changes of the area after trauma. Overload work of judges and lawyers after traffic accidents does not help to remind the real victim's problem, that is stretching of the neck soft tissues during head acceleration-extension. This movement is due to a forward acceleration. Both the car and the victim's trunk are violently pushed forwa

C7 radiculopathy: importance of scapular winging in clinical diagnosis.

Lesions of the seventh cervical (C7) root are common and cause a readily recognised neurological syndrome. Recognition of this pattern is essential in differentiating C7 root lesions from lesions of the brachial plexus or peripheral nerves. Serratus anterior weakness is not generally included in this syndrome. We report six verified cases of C7 radiculopathy in which weakness of the serratus anterior was present in addition to the usual findings. This was manifest as winging of the scapula, when pushing forward against a wall, either with the hands at shoulder level or, in some cases, only whe

Treatment of iatrogenic brachial plexopathy persisting for 3 years due to cervical foraminotomy using Korean medicine: A case report (CARE-compliant).

BACKGROUND: Extrinsic or iatrogenic brachial plexopathy often resolves spontaneously within six months. Although steroid therapy and rehabilitation interventions are employed in the management of brachial plexopathy, their efficacy remains limited with insufficient supporting evidence. Moreover, therapeutic options to improve symptoms after the natural recovery period are limited. CASE PRESENTATION: A 49-year-old Asian woman was admitted with a three-year history of right upper extremity weakness and pain. The patient was diagnosed with sequelae of iatrogenic right brachial plexus injury due
