Rating the seriousness of maternal and child health outcomes linked with pregnancy weight gain.

BACKGROUND: Current pregnancy weight gain guidelines were developed based on implicit assumptions of a small group of experts about the relative seriousness of adverse health outcomes. Therefore, they will not necessarily reflect the values of women. OBJECTIVE: To estimate the seriousness of 11 maternal and child health outcomes that have been consistently associated with pregnancy weight gain by engaging patients and health professionals. METHODS: We collected data using an online panel approach with a modified Delphi structure. We selected a purposeful sample of maternal and child health p…

The role of the traditional leader in implementing maternal, newborn and child health policy in Malawi.

Traditional leaders play a prominent role at the community level in Malawi, yet limited research has been undertaken on their role in relation to policy implementation. This article seeks to analyse the role of traditional leaders in implementing national maternal, newborn and child health (MNCH) policy and programmes at the community level. We consider whether the role of the chief embodies a top-down (utilitarian) or bottom-up (empowerment) approach to MNCH policy implementation. Primary data were collected in 2014/15, through 85 in-depth interviews and 20 focus group discussions in two dist…

The Combination of Sibling Victimization and Parental Child Maltreatment on Mental Health Problems and Delinquency.

This study examined how the combination of sibling victimization and parental child maltreatment is related to mental health problems and delinquency in childhood and adolescence. Co-occurrence, additive associations, and interactive associations of sibling victimization and parental child maltreatment were investigated using a sample of 2,053 children aged 5-17 years from the National Survey of Children's Exposure to Violence. The results provide primarily evidence for additive associations and only suggest some co-occurrence and interactive associations of sibling victimization and child mal

The association between child maltreatment and emotional, cognitive, and physical health functioning in Vietnam.

BACKGROUND: There is a paucity of research on correlates of child maltreatment in limited-resource countries with a relatively high tolerance of harsh discipline. This Vietnamese study aimed to investigate associations between different types of child maltreatment and child emotional, cognitive, and physical health functioning as well as moderation effects of gender and ethnicity. METHODS: This cross-sectional study was conducted with 1851 randomly selected students aged 12-17 years. Both self-report and more objective measures (weight, height, study ranking, and a memory test) were used. RE

Measures of Maternal Socioeconomic Status in Yemen and Association with Maternal and Child Health Outcomes.

BACKGROUND: Reliable measurement of socioeconomic status (SES) in health research requires extensive resources and can be challenging in low-income countries. We aimed to develop a set of maternal SES indices and investigate their associations with maternal and child health outcomes in rural Yemen. METHODS: We applied factor analysis based on principal component analysis extraction to construct the SES indices by capturing household attributes for 7295 women of reproductive age. Data were collected from a sub-national household survey conducted in six rural districts in four Yemeni provinces …

The Rising Burden of Preeclampsia in the United States Impacts Both Maternal and Child Health.

OBJECTIVE: Preeclampsia is one of the top six causes of maternal mortality in the United States (US) and is associated with considerable perinatal morbidity and mortality. Evidence suggests the US incidence of preeclampsia has increased dramatically over the past two decades. This study aims to compile, summarize, and critique the literature on the health and economic burden of preeclampsia and early-onset preeclampsia. STUDY DESIGN: We reviewed the literature for estimates of burden of preeclampsia and early-onset preeclampsia to both mother and child, summarized the evidence on economic and

Evaluation of child maltreatment in the emergency department setting: an overview for behavioral health providers.

Emergency providers are confronted with medical, social, and legal dilemmas with each case of possible child maltreatment. Keeping a high clinical suspicion is key to diagnosing latent abuse. Child abuse, especially sexual abuse, is best handled by a multidisciplinary team including emergency providers, nurses, social workers, and law enforcement trained in caring for victims and handling forensic evidence. The role of the emergency provider in such cases is to identify abuse, facilitate a thorough investigation, treat medical needs, protect the patient, provide an unbiased medical consultatio

Micronutrient fortification of food and its impact on woman and child health: a systematic review.

BACKGROUND: Vitamins and minerals are essential for growth and metabolism. The World Health Organization estimates that more than 2 billion people are deficient in key vitamins and minerals. Groups most vulnerable to these micronutrient deficiencies are pregnant and lactating women and young children, given their increased demands. Food fortification is one of the strategies that has been used safely and effectively to prevent vitamin and mineral deficiencies. METHODS: A comprehensive search was done to identify all available evidence for the impact of fortification interventions. Studies wer

Child and adolescent abuse in relation to obesity in adulthood: the Black Women's Health Study.

OBJECTIVE: To investigate the association of physical and sexual abuse in childhood and adolescence with risk of adult obesity among black women in the United States. METHODS: Participants were women enrolled in the Black Women's Health Study, an ongoing prospective cohort study begun in 1995. In 2005, 33298 participants completed a self-administered questionnaire on early life experiences of abuse. Log-binomial regression models were used to derive risk ratios (RRs) and 95% confidence intervals (CIs) for the relation of child/teenager abuse with obesity (BMI ≥ 30) and central adiposity (wais

Risk factors for premature termination of treatment at a child and family mental health clinic.

OBJECTIVE: Premature termination of treatment amongst children and families attending mental health services is a significant problem for both outcomes research and clinical practice in South Africa and elsewhere. This study investigated factors that are associated with premature termination of treatment at a public service child and family clinic in Cape Town. METHOD: A retrospective archival analysis of clinic files from 2002-2009 was conducted. Administrative, child and family factors, and type of treatment were explored as risk factors for premature termination. RESULTS: A single-parent

The difference transport makes to child mortality and preventive healthcare efforts: Riders for Health.

The 20th and 21st century witnessed the development of many sophisticated vaccinations and other key preventive health interventions, and yet child mortality in sub-Saharan Africa remains unacceptably high. One of the barriers identified to reducing child mortality in rural regions of Africa is distance and lack of transportation. In order to address this, a growing charitable organisation, Riders for Health (http://www.riders.org) has developed a reliable and cost-effective system for managing vehicles used in all types of healthcare delivery. The system intends to ensure that the delivery of

Breastfeeding and atopic eczema in Japanese infants: The Osaka Maternal and Child Health Study.

Epidemiological studies associated with breastfeeding have provided conflicting results about whether it is preventive or a risk factor for atopic eczema in children. The current prospective study investigated the relationship between breastfeeding and the risk of atopic eczema in Japan. A birth cohort of 763 infants was followed. The first survey during pregnancy and the second survey between 2 and 9 months postpartum collected information on potential confounding factors and atopic eczema status. Data on breastfeeding and symptoms of atopic eczema were obtained from questionnaires in the thi

Continuum of care for maternal, newborn, and child health: from slogan to service delivery.

The continuum of care has become a rallying call to reduce the yearly toll of half a million maternal deaths, 4 million neonatal deaths, and 6 million child deaths. The continuum for maternal, newborn, and child health usually refers to continuity of individual care. Continuity of care is necessary throughout the lifecycle (adolescence, pregnancy, childbirth, the postnatal period, and childhood) and also between places of caregiving (including households and communities, outpatient and outreach services, and clinical-care settings). We define a population-level or public-health framework based…

Child mental health problems in Arab children: application of the strengths and difficulties questionnaire.

AIM: This study aimed at establishing the mental health profile among 322 Arab children living in the Gaza strip. METHOD: Children were selected in four age bands, i.e. 3, 6, 11 and 16 years of age. The relevant forms of the Strengths and Difficulties Questionnaire (SDQ) was completed by parents, teachers and 16-year-olds. RESULTS: Factor analyses of the parent-related questionnaires identified similar general factors as in the UK-based studies of validating the SDQ. Certain items did not load as highly on the general factors, i.e. distractability, feeling scared, feeling unhappy, stealing,

Genital infections and reproductive health: infertility and morbidity of mother and child in developing countries.

Genital infections in impoverished countries take a heavy toll both in the etiology of childlessness and in pregnancy-related morbidity, both for the carrier (mother) and the passenger (fetus/newborn). Tubal damage is by far the most common single etiology in cases of childlessness. Among the three factors principally contributing to childlessness, infertility, pregnancy wastage and child loss, tubal infertility is the predominant one. Prevalence figures of N. gonorrhoeae among women in reproductive ages range between 5 and 15% in several countries. Similar figures prevail among pregnant/puerp

[Significance of the registration of testis localization in child health services for the avoidance of unnecessary orchiopexies].

The health records of 1816 11-year-old Dutch schoolboys were examined to assess the importance of these records for boys with an apparently non-scrotal testicle. Full records from birth onward existed for 58% of the boys; at least 4 examinations were recorded in 76% and no prior examinations in less than 4% of the boys. Orchidopexy had been performed in 2.9% of the boys; the records of 33 of the 53 boys with a history of orchidopexy showed intrascrotal position of both testicles at least once. The conclusion is that the Dutch youth health system possesses relevant records which should be consu
