Labia minora elongation as understood by Baganda male and female adolescents in Uganda.

Labia minora elongation is a common traditional female genital modification practice among the members of the Baganda ethnic group in Uganda. In 2002, a study carried out by the Padua Working Group on Female Genital Mutilation analysed how Baganda girls residing in Wakiso District graphically represented their experiences of labia minora elongation. In the present study, using the same methodology and in the same geographic setting 10 years later, we asked young men and women to prepare graphical representations of this rite. The purpose was to learn about how the practice is perceived and rep

Lower urinary tract symptoms following female genital mutilation.

OBJECTIVE: To estimate the prevalence of lower urinary tract symptoms (LUTS) following female genital mutilation (FGM). METHODS: A hospital-based, cross-sectional study was conducted from January 1, 2009, through January 1, 2012, at the urogynecology clinic of Sohag University Hospital. The participants were 251 women who had undergone different forms of FGM. RESULTS: A total of 251 women agreed to participate. All had the clitoris amputated, 7.1% had some form of mutilation to the labia majora, 33.8% had adhesions of the labia minora, 19.2% had clitoral retention cysts, and 38.8% reported a

Cosmetic clitoridectomy in a 33-year-old woman.

The Female Genital Mutilation Act (2003) in England allows for mental health exceptions for cosmetic surgery resulting from perceived abnormality. Similar legislation exists in other countries. There are no reported cases of clitoridectomy for cosmetic reasons or any discussion in the literature of mental health exceptions to the Act. This is a single case report on a 33-year-old married, heterosexual woman who had already had a cosmetic labiaplasty and was seeking a clitoridectomy for aesthetic reasons. At assessment, there were no psychiatric contra-indications or unrealistic expectations an

Prevalence and determinants of the practice of genital mutilation of girls in Khartoum, Sudan.

BACKGROUND: Female genital mutilation (FGM) is widely practised in Sudan, despite many decades of attempts to prevent it. AIMS: To estimate the prevalence of FGM, identify the types performed in Khartoum and investigate whether FGM is associated with various social factors. METHODS: Girls aged 4-9 years (n=255) presenting to a paediatric emergency ward were recruited. A detailed history was obtained and full examination, including inspection of the genitalia, was performed to verify the type and extent of FGM. RESULTS: Twenty per cent of the study group had undergone FGM, 50% of guardians i

Out of Eastern Africa: defibulation and sexual function in woman with female genital mutilation.

INTRODUCTION: Female genital mutilation (FGM) is defined by the World Health Organization (WHO) as all procedures that involve partial or total removal of the female external genitalia and/or injury to the female genital organs for cultural or any other non-therapeutic reasons. AIM: Aim of this study was to determine sexual function before and after defibulation using a CO(2) laser in migrant women who had undergone FGM in the past. MAIN OUTCOME MEASURES: Female Sexual Function Index (FSFI) before and 6 months after defibulation. METHODS: Patients were asked to fill the FSFI before surgery

"A heart of the man is lighter than that of the woman…" exploring men's motivation and capability to access HIV services in Lusaka, Zambia: findings from the Yaba Guy Che study.

BACKGROUND: In southern Africa, men are less likely than women to access HIV services, including HIV testing, antiretroviral therapy (ART), and HIV prevention services. As a result, men living with HIV are less likely to be virally suppressed and more likely to transmit HIV than their female peers. Using the socio-ecological model as a framework, we explored factors that influence men’s motivation and capability to access available HIV services, including how social norms and social networks influence men’s engagement with services, in Lusaka, Zambia. METHODS: We conducted seven focus group di

No increased HIV risk in general population near sex work sites: A nationally representative cross-sectional study in Zimbabwe.

OBJECTIVES: Sex work sites have been hypothesised to be at the root of the observed heterogeneity in HIV prevalence in sub-Saharan Africa. We determined if proximity to sex work sites is associated with HIV prevalence among the general population in Zimbabwe, a country with one of the highest HIV prevalence in the world. METHODS: In this cross-sectional study we use a unique combination of nationally representative geolocated individual-level data from 16,121 adults (age 15-49 years) from 400 sample locations and the locations of 55 sex work sites throughout Zimbabwe; covering an estimated 95

Modelling impact and cost-effectiveness of oral pre-exposure prophylaxis in 13 low-resource countries.

INTRODUCTION: Oral pre-exposure prophylaxis (PrEP) provision is a priority intervention for high HIV prevalence settings and populations at substantial risk of HIV acquisition. This mathematical modelling analysis estimated the impact, cost and cost-effectiveness of scaling up oral PrEP in 13 countries. METHODS: We projected the impact and cost-effectiveness of oral PrEP between 2018 and 2030 using a combination of the Incidence Patterns Model and the Goals model. We created four PrEP rollout scenarios involving three priority populations-female sex workers (FSWs), serodiscordant couples (SDC

Spatial analysis of factors associated with HIV infection among young people in Uganda, 2011.

BACKGROUND: The HIV epidemic in East Africa is of public health importance with an increasing number of young people getting infected. This study sought to identify spatial clusters and examine the geographical variation of HIV infection at a regional level while accounting for risk factors associated with HIV/AIDS among young people in Uganda. METHODS: A secondary data analysis was conducted on a survey cross-sectional design whose data were obtained from the 2011 Uganda Demographic and Health Survey (DHS) and AIDS Indicator Survey (AIS) for 7 518 young people aged 15-24 years. The analysis

Anatomical Reconstruction following Female Genital Mutilation/Cutting.

BACKGROUND: International migration from high-prevalence regions has increasingly confronted nonendemic countries with female genital mutilation/cutting (FGM/C), and Western-based health care providers have seen a greater demand for surgical reconstruction of female anatomic units. The authors introduce novel surgical techniques developed by the first author for clitoral and vulvovestibular reconstruction and examine operative outcomes. METHODS: The authors performed a retrospective cohort study of operative outcomes of the omega domed flap, neurotizing and molding of the clitoral stump (NMCS

'I want what every other woman has': reasons for wanting clitoral reconstructive surgery after female genital cutting - a qualitative study from Sweden.

Female genital cutting (FGC) involves the removal of women's external genitalia for non-therapeutic reasons. An estimated 38,000 women living in Sweden have undergone some form of the procedure. These women often belong to marginalised minorities of immigrant women from countries where FGC is widespread. Clitoral reconstructive surgery following FGC has recently been introduced in Sweden. This study investigates women's perceptions of FGC and clitoral reconstructive surgery with a particular focus on: (1) reasons for requesting reconstructive surgery, and (2) FGC-affected women's expectations

Skinning clitorectomy and skin replacement in women with vulvar intra-epithelial neoplasia.

BACKGROUND AND AIM: Partial or total clitoris amputation for vulvar intra-epithelial neoplasia (VIN) affects quality of life and sexual function and is likely to constitute over-treatment as superficial excision of only the involved, thinly cornified, stratified squamous clitoral epithelium would suffice. For this reason, we applied skinning clitorectomy and replacement of clitoral skin as an organ-sparing surgical therapy for clitoral VIN. METHODS: Seven consecutive patients presenting with VIN were treated from July 2003 to February 2008. The skin of the glans clitoridis was resected from t

Community-based survey on female genital excision in Faranah District, Guinea.

This paper reports on a community-based study in 1999 of the beliefs and practices of people in Faranah District, Guinea regarding female genital excision (FGE). Semi-structured individual interviews and focus group discussions were carried out with women of reproductive age, older women, married men, community and religious leaders, traditional practitioners and health workers. The study found that FGE was being carried out on girls aged 6-14, mostly using a traditional knife and involving total excision of the clitoris and partial removal of the external genitals, in conjunction with instruc

Parent Race and Communication During Elective Pediatric Surgery Consultations.

IMPORTANCE: Child and adolescent members of marginalized racial groups have poorer surgical outcomes compared with White children and adolescents. Clinical communication may contribute to these disparities. OBJECTIVE: To explore racial differences in parent-clinician communication during initial consultations for elective surgical procedures in children and adolescents. DESIGN, SETTING, AND PARTICIPANTS: This cohort study investigated communication during consultations between parent-clinician dyads: surgical clinicians and parents of children and adolescents ages 2 to 17 years referred for

Male circumcision for HIV prevention: current research and programmatic issues.

Randomized controlled trials in sub-Saharan Africa have shown that adult male circumcision reduces the risk of HIV acquisition in men by about 60%. In this article, we review recent data on the association of male circumcision and HIV/sexually transmitted infection in men and women. This includes a summary of data showing some evidence of an effect of male circumcision against genital ulcer disease, HSV-2 infection, human papillomavirus and Trichomonas vaginalis, but not Chlamydia trachomatis or Neisseria gonorrhoea in men. Longitudinal studies among HIV discordant couples suggest that male ci

The impact of HIV on cervical cancer elimination in KwaZulu-Natal: a comparative modeling analysis.

BACKGROUND: Achieving cervical cancer (CC) elimination requires addressing disparities in CC burden for women living with HIV (WLHIV) and how disparities evolve in the context of antiretroviral therapy (ART) scale-up. To inform CC elimination for high HIV prevalence regions, we modeled the impact of HIV, HIV interventions, and CC interventions in KwaZulu-Natal, South Africa. METHODS: We used 2 independently developed, dynamic compartmental transmission models of HIV and human papillomavirus (DRIVE and Policy1-Cervix-HIV) calibrated to KwaZulu-Natal. We simulated: a counterfactual without HIV
