The discovery of a Middle Pleistocene hominid clavicle is reported here. This discovery is particularly important because clavicles are hitherto unrepresented in the fossil record of Asia. The Narmada clavicle comes from the Boulder Conglomerate horizon at Hathnora near Hoshangabad in the Central Narmada Valley. This is the same deposit that previously yielded the Homo erectus/archaic Homo sapiens partial cranium, which has recently been dated to between 0.2 and 0.7 ma (million years ago). We evaluated 242 consecutive fractures of the clavicle in adults which had been treated conservatively. Of these, 66 (27%) were originally in the middle third of the clavicle and had been completely displaced. We reviewed 52 of these patients at a mean of 38 months after injury. Eight of the 52 fractures (15%) had developed nonunion, and 16 patients (31%) reported unsatisfactory results. CASE: A 67-year-old patient sustained a medial clavicle fracture (MCF) treated with open reduction and fixation with cerclage sutures. A year later, he presented with symptomatic nonunion and skin prominence, which was revised with an intramedullary tendon allograft fixation including the sternoclavicular joint.
