Expert Consensus on Clinical Decision Making in the Disease Trajectory of Oropharyngeal Dysphagia in Adults: An International Delphi Study.

To date, no consensus exists on the complex clinical decision-making processes involved in oropharyngeal dysphagia, or swallowing disorders. This study aimed to develop an international consensus on a clinical decision tree for the disease trajectory of oropharyngeal dysphagia in adults, taking into account physiological impairments of swallowing, risk factors for the development of complications from oropharyngeal dysphagia, and prognostic factors for treatment outcomes. Using the Delphi technique, consensus was achieved among dysphagia experts across 31 countries, resulting in a total of 10 …

Comprehension of written texts for the assessment of clinical competence and decision making in people with mild to moderate Alzheimer disease.

BACKGROUND: Clinical competence is the term used to describe an individual's capacity to express a choice regarding their participation in clinical procedures or experimental studies. Understanding the information provided is a prerequisite but consent forms are often lengthy and complicated. Alzheimer's disease patients may be vulnerable in written comprehension, due to cognitive deficits, but unfortunately to date, a specific evaluation of this ability is not included in periodical assessments. METHODS: One hundred thirty Italian patients with Alzheimer's disease were compared with 130 cont

Moyamoya Syndrome in a Child with Neurofibromatosis Type 1: Magnetic Resonance Imaging as a Tool for Clinical Decision Making.

Moyamoya syndrome is a rare cerebrovasculopathy of unknown etiology which is associated with multiple risk factors. Moyamoya was first discovered in Japan and is reported to have an increased prevalence in the Japanese population. The term "Moyamoya" translates into "puff of smoke" and is named after the finding of the collateral cerebral vasculature that develops secondary to occlusion of an internal carotid artery at the entrance into the circle of Willis. This collateral vasculature characterizes the disease. Moyamoya should be included in the differential diagnosis in the pediatric populat

A Clinical Focus on Shared Decision Making in Clinical Practice When Providing Dysarthria and Dysphagia Services to Individuals With Amyotrophic Lateral Sclerosis.

PURPOSE: Traditional health care decision-making models center on clinicians making decisions for patients/caregivers based on the best available research evidence. However, this diminishes patient/caregiver involvement in their care and hinders the ability to align care plans with patient values and preferences. Shared decision making is a potentially beneficial process to implement with individuals with amyotrophic lateral sclerosis (ALS) to provide more holistic, patient-centered dysarthria and dysphagia treatment. Shared decision making promotes active involvement by patients/caregivers by

How Continuous Glucose Monitoring Reports Inform Clinical Decision-Making for Older Adults With Type 2 Diabetes: Mapping Clinical Workflow to Situation Awareness.

BACKGROUND: Older adults with type 2 diabetes experience high risk of hypoglycemia, yet clinicians often lack actionable glucose data to guide individualized treatment decisions. This study examined how standardized continuous glucose monitoring (CGM) reports inform clinicians' decision-making using Endsley's situation awareness (SA) framework. METHODS: We conducted semi-structured interviews. Thirty clinicians reviewed three simulated older adult type 2 diabetes cases before and after reviewing CGM reports. Data saturation was achieved, consistent with qualitative research standards. Using a

The factorial survey as an approach to investigate clinical decision-making: examining influences on a clinician's decision to initiate life-sustaining clinical technology for a child with spinal muscular atrophy type 1.

BACKGROUND: Spinal Muscular Atrophy (SMA) type 1 is a debilitating condition with a poor prognosis, though therapeutic advances are promising. Long-term ventilation is a common management strategy as respiratory function deteriorates. Without consensus on best practice respiratory management, the decision to initiate invasive LTV (I-LTV) for this group of young children involves many ethical considerations. Understanding the main influencing factors on a clinician's likelihood to initiative I-LTV for a child with chronic critical illness is important to maintain transparency and trust with the

Objective Functional Phenotyping in Spine Surgery: A Translational Framework for Quantifying Disability, Guiding Clinical Decision-Making, and Monitoring Recovery.

Imaging, physical examination, and patient-reported outcome measures (PROMs) remain central to spine care but incompletely characterize how spinal disease affects physical performance and how function changes following treatment. Objective assessment of gait, balance, functional transitions, lifting, and neuromuscular control provides complementary information regarding movement impairment, compensation, and recovery. This narrative review synthesizes current evidence supporting objective functional assessment in spine care and proposes Disability and Functional Outcome Measures (DFOMs) as a t

Development and Evaluation of a Two-Stage Machine Learning Multivariable Model to Support Clinical Decision-Making in Thrombophilia Management.

Thrombophilia is a disease characterized by an increased risk of venous thromboembolism. Diagnosing and treating thrombophilic patients requires the nuanced interpretation of a great number of medical data. In this single-center, retrospective study, we evaluated the use of a two-stage machine learning (ML) model to predict the thrombophilia risk category and subsequent anticoagulation recommendation. The model was trained and tested using clinical and laboratory data obtained from 496 patients who underwent a complete thrombophilia work-up at the University Hospital Bonn between 2019 and 2024

Urban-rural disparities in clinical decision-making for older adults with cancer in Japan: Results from a nationwide survey.

INTRODUCTION: Japan has an ageing population and the highest proportion of the population aged 75 years and older worldwide. Geriatric assessment (GA) tools are recommended for optimising cancer treatment in older adults. This study assessed the use of GA tools for older adults with cancer in Japan by the level of urbanisation. MATERIALS AND METHODS: A nationwide web-based questionnaire survey of Japanese Society of Medical Oncology members was conducted in October 2024. Respondents were asked how they defined "older adults with cancer," including considerations such as chronological age, fun

Associations Between Health Information Sources, Sociodemographic and Clinical Characteristics, and Decision-Making Among Women with Early-Stage Breast Cancer.

BACKGROUND: Most breast cancer cases in the United States are diagnosed early, and women often must decide whether to undergo mastectomy or breast-conserving surgery to treat their cancer. Ideally, women engage in shared decision-making with their provider, which involves becoming informed about surgical options. This study examined where women obtain health information to inform surgical decision-making, sociodemographic and clinical factors associated with their use of specific sources, and potential factors (e.g., cancer stage) associated with surgical decisions. METHODS: Women diagnosed w

Minimally Invasive Bone Regeneration in Implant Dentistry: From Biological Principles to Indication-Driven Clinical Decision-Making-A Narrative Review.

Background/Objectives: Contemporary implant dentistry is increasingly oriented toward minimally invasive regenerative strategies designed to reduce surgical morbidity while preserving or improving clinical outcomes. Conventional bone augmentation procedures remain effective and biologically well established, but they may be associated with greater patient burden, increased risk of complications, and higher technical demands in selected clinical scenarios. This narrative review critically examines minimally invasive bone regeneration approaches in implant dentistry, with particular focus on the

A meta-review of patient engagement, shared decision-making, and factors influencing equity-deserving populations' participation in clinical trials.

BACKGROUND: Many equity-deserving populations, including those facing structural health inequities, lack support to participate in clinical trials while facing barriers to participation. Two approaches—patient engagement (PE) and shared decision-making (SDM)—can help trialists better understand and address such barriers. PE can improve the relevance of trials to silenced communities while SDM can align participation decisions among socially disadvantaged groups with their values, needs, and preferences, which may help overcome health inequities. Further, Indigenous community engagement is vita
