INTRODUCTION: Osteopetrosis is a rare genetic disorder characterized by increased bone density. This condition is clinically manifested with a brittle intramedullary structure and reduced bone toughness, increasing the risk of fracture. A limited case has been reported on the management of physeal fracture in patients with osteopetrosis. The objective of this study is to report a rare osteopetrosis patient who sustains Salter-Harris II at the distal tibia with its long-term follow-up along with its literature review.
PRESENTATION OF CASE: We report a case of a four-year-old boy with an establ

The treatment and results of a retrospective study are presented on 23 patients who underwent surgical treatment for a fracture of the base of the thumb metacarpal. The 12 patients with a Bennett fracture were not found to be suffering from limitations in their daily activities, work, sport or hobbies. In the majority of cases an anatomical or almost anatomical position was achieved and maintained using closed reduction and extra-articular Kirschner wire fixation. Two of the seven patients with a Rolando fracture were found to have complaints which gave rise to limitations in the above-mention

A series of 34 juxta-epiphyseal fractures of the base of the proximal phalanx of the fingers of children and adolescents are presented. The pattern of injury appeared identical in all these fractures, with a lateral angulation force separating a small triangular metaphyseal fragment from the base of the phalanx on the side of angulation and the fracture line then continuing through the metaphysis, 1-2 mm distal to the growth plate. Fractures were classified into two types according to the degree of displacement. Type 1 fractures (n=18) were mildly displaced and were all successfully treated wi

Ipsilateral concomitant hip dislocation and femoral shaft fracture in childhood is extremely rare. A further case and a literature review, with 35 additional observations are presented. The dislocation was initially missed in more than 50% of cases, but when immediately diagnosed, closed reduction manoeuvres were effective in all observations but one, and results were rated normal. As delayed reduction predisposes to avascular necrosis, every child with a femoral shaft fracture should be given a routine X-rays of pelvis, including lateral hip views, as a safeguard against missing an associated

BACKGROUND: Masissoneuve fractures of the proximal half of the fibula are infrequently reported with only a few reports existing recommending closed reduction of the syndesmosis and percutaneous syndesmotic fixation. The purpose of this study was to compare the reduction obtained with open and closed reduction techniques of the fibular fracture with screw fixation of the syndesmosis.
MATERIALS AND METHODS: Twelve patients with acute displaced fibula fractures in the proximal one-half with syndesmotic disruption were enrolled in the study. One orthopedic traumatology team at the time of the st

BACKGROUND: Closed reduction and splinting followed by outpatient management is standard of care for temporizing most ankle fractures. However, ankle fracture-dislocation potentially warrants a different approach based on the propensity for loss of reduction. The purpose of this study was to determine the rate of complications associated with closed reduction and splinting of unstable ankle fracture-dislocations. Further, we sought to determine the efficacy of immediate external fixation as an alternative to splinting in cases too swollen for acute operation.
METHODS: This retrospective chart

Non-unions are quite rare in closed fractures in children. Most distal radius fractures require orthopedic reduction and conservative treatment with very good radiological and clinical/functional results. In case of unsatisfactory reduction, surgical treatment is necessary to correct significant displacement. Surgical treatment consists of closed reduction and percutaneous fixation using K-wires. If closed reduction is not possible, open reduction and fixation is mandatory. Generally, fixation is obtained using K-wires, in most cases, even if open reduction is necessary, rarely locking plates,

Proximal humerus fractures are a common upper extremity injury that the pediatric orthopaedic surgeon will certainly encounter. Most of these fractures are managed with closed reduction with expectant remodeling. However, inevitably, some fractures are not well suited to minimal or nonsurgical management. Whether the fracture alignment would cause functional impingement or rotational impairment, if the fracture is irreducible, or if the patient is an adolescent with minimal remodeling potential, there are various treatment options. Closed reduction and creative immobilization techniques can ad

OBJECTIVES: To determine (1) which factors are associated with the choice to perform an open reduction and (2) by adjusting for these factors, if the choice of reduction method is associated with reoperation.
DESIGN: Retrospective cohort study with radiograph and chart review.
SETTING: Twelve Level 1 North American trauma centers.
PATIENTS: Two hundred thirty-four adults 18-65 years of age with an isolated, displaced, OTA/AO type 31-B2 or type 31-B3 femoral neck fracture treated with internal fixation with minimum of 6-month follow-up or reoperation. Exclusion criteria were pathologic fract

CASE: A 14-year-old adolescent girl sustained a Lisfranc fracture-dislocation with an interposed extensor hallucis brevis (EHB) tendon. Following multiple failed attempts at closed reduction in both the emergency department and the operating room, the patient was treated in a staged manner with temporizing closed reduction and percutaneous pinning in improved alignment, followed by definitive open reduction and internal fixation once soft tissues allowed.
CONCLUSIONS: Anatomic reduction and stable fixation of Lisfranc injuries is vital to regain stability and reduce the risk of midfoot arthri

BACKGROUND: Reduction of pediatric forearm fractures with the patient under sedation in the emergency department is a common practice throughout the United States. We hypothesized that the use of a mini-c-arm fluoroscopy device as an alternative to routine radiographs for evaluation of fracture reduction would (1) allow a more anatomic fracture reduction, (2) decrease the number of repeat reductions or subsequent procedures, (3) reduce overall radiation exposure to the patient, and (4) decrease the orthopaedic consultation time in the emergency department.
METHODS: A retrospective cohort anal

BACKGROUND: Two out of three wrist fractures seen in the emergency are displaced but most can be treated conservatively after closed reduction. Patient-reported pain during closed reduction of distal radius fractures varies widely and the best method to decrease the perceived pain has not been well established. The purpose of this study was to assess the pain during closed reduction of distal radius fractures after using the hematoma block as method of anaesthesia.

Supracondylar humerus fractures are among the most common paediatric injuries. Although neurologic complications are often seen in these fractures, iatrogenic nerve injuries following closed reduction and percutaneous pinning remain relatively uncommon. Radial nerve palsy is particularly rare in this context, and most cases represent neuropraxic injuries that recover spontaneously. This report describes a rare case of iatrogenic high radial nerve palsy following closed reduction and K-wire fixation of a comminuted supracondylar humerus fracture in a child, with complete functional recovery und

INTRODUCTION: T-condylar fractures of the humerus are very rare in children. The treatment options vary from simple reduction and percutaneous stabilisation to open reduction and fixation with plates, screws and Kirchner wires. We evaluated 19 patients who underwent different types of treatment aiming to compare two different approaches of reduction, postoperative results and complications.
METHODS: The study encompassed total of 19 patients with T-condylar fracture, aged from 3 to 16 years, who underwent either closed or opened reduction between February 2005 and September 2012. Closed reduc

INTRODUCTION: Posterior hip dislocations combined with acetabular posterior wall fractures are usually managed with open reduction and internal fixation. However, select cases may benefit from closed reduction followed by minimally invasive arthroscopic evaluation.
CASE REPORT: We report a case of a 27-year-old male who sustained left side complex posterior hip dislocation with ipsilateral compound Grade 2 fracture of the shaft of the tibia with facial and chest injury following a high-speed motorcycle accident. The injury was successfully managed with closed manual reduction of the hip joint

Many techniques have been described for closed reduction of posterior hip dislocations, but most require forceful and sustained traction, assistants to stabilise the pelvis and awkward positioning of the surgeon and/or the patient. We describe a new technique of closed reduction that does not need traction-countertraction or special positioning and can be safely and effectively performed by one surgeon.Fifteen patients, mean age 36.4 years, were subjected to reduction. There were five type I, four type II and six type III dislocations; ipsilateral injuries included one knee dislocation with pa

INTRODUCTION: Bisphosphonate-associated atypical femur fractures have been reported to have high rates of delayed union and nonunion, as well as high rates of contralateral fracture. There have been limited studies looking at specific factors associated with delayed union or nonunion of atypical fractures. Many studies are limited by small numbers of patients. To date, no studies have examined the effect of reduction technique (closed versus open versus percutaneous) and reduction quality on healing.
METHODS: This was a multicentered retrospective review of patients greater than 50 years of a
