Antibiotic Resistances and Molecular Characteristics of Clostridioides difficile in ICUs in a Teaching Hospital From Central South China.

Clostridioides (C.) difficile is a major healthcare-associated pathogen inducing infectious diarrhea. Approximately 25-33% of patients with antibiotic-associated diarrhea (AAD) and 90% of patients with pseudomembranous enteritis are caused by C. difficile infection (CDI). Stool samples were collected from hospitalized adults with presumptive AAD in four nonneonatal intensive care units (ICUs). Diagnosis of CDI was based on both clinical symptoms and laboratory results. The stool specimens were transferred onto CDIF (C. difficile agar), and C. difficile was finally confirmed by the latex agglut…

Blastocystis and Clostridioides difficile: Evidence for a Synergistic Role in Colonization Among IBD Patients with Emphasis on Ulcerative Colitis.

BACKGROUND: Regarding the controversial role of Blastocystis in inflammatory bowel diseases (IBD) patients, it seems that this protozoan may lead to an overgrowth of some non-beneficial bacteria. The current study aimed to investigate the co-existence of Blastocystis and Clostridioides difficile in IBD patients. METHODS: Stool samples of 102 IBD patients were collected and cultivated for C. difficile and Blastocystis. DNA extraction was performed on positive samples and C. difficile and Blastocystis were toxinotyped and subtyped, respectively. Fisher's exact test and logistic regression were …

Level of Knowledge of Medical Staff on the Basis of the Survey in Terms of Risk Management, Associated with Clostridioides difficile Infections.

Infections caused by the toxigenic strains of Clostridioides difficile in the hospital environment pose a serious public health problem. The progressive increase in hospital infections in Poland indicates that risk management is a tool that is not used in an effective way and significantly differs from the goals set by the Leading Authorities, the Ministry of Health and its subordinate units. Systematic education of medical personnel constitutes the basic element of rational risk management aimed at reducing the number of infections as it allows for the transfer of knowledge, development of ap…

Antibiotic resistance of clinical isolates of Clostridioides difficile in China and its association with geographical regions and patient age.

It is known that antibiotic usage is associated with the development of Clostridioides difficile infection (CDI), especially clindamycin, third-generation cephalosporins, and fuoroquinolones. Antibiotic resistance rates to many antibiotics varies a lot by study. We performed a study focused on antibiotic resistance in clinical isolates of C. difficile from more widespread geographic regions across China. Of 319 C. difficile isolates tested against 11 antibiotics, 313 (98.1%) were resistant to at least one antibiotic. The highest rate of resistance was to ciprofloxacin, clindamycin, and erythro…

The Phases of Clostridioides difficile Infection (CDI) in the United States: The History and Epidemiology of CDI From 1935 to Present.

This review combines historical reports and surveillance data to contextualize Clostridioides difficile infection (CDI) in the United States as a phased epidemic, currently in the deceleration phase. C. difficile was first isolated in 1935. During a prolonged emergence phase, the organism attracted limited clinical attention. In 1978, C. difficile was established as the cause of antibiotic-associated pseudomembranous colitis and from 1978 to a peak in 2010, CDI was in an acceleration phase characterized by fluoroquinolone-resistant, toxin-overproducing strains, large hospital outbreaks, and in…

Clostridioides difficile couples responses to host nutritional immunity and a commensal-derived antibiotic to enable growth in the gut.

The Gram-positive opportunistic pathogen Clostridioides difficile colonizes the gut by adapting to an array of threats from the host and the microbiome. A potential threat from the microbiome is the antimicrobial peptide bacitracin, which is produced by the commensal Bacillus licheniformis and prevents dephosphorylation and recycling of undecaprenyl phosphate, a lipid carrier essential in cell wall biogenesis. Gram-positive bacteria typically use multiprotein membrane complexes called Bce modules to sense and respond to bacitracin. We identified a Bce module in C. difficile that incorporates t…

An Integrated Workflow for the Purification and Fluorescence Imaging of Clostridioides difficile Spores Using Lanthanide Beta-Diketone Complexes.

We present a robust protocol for both the purification and imaging of C. difficile spores. First, we describe a simplified, one-step isopycnic centrifugation method using Percoll that enables efficient separation of spores from vegetative cells. Next, we introduce a fluorescence-based imaging method using lanthanide beta-diketone complexes (primarily Eu[TTA]3) that allows for the differential visualization of dormant and germinated spores, as well as sporulating cells. These complexes exploit the unique photophysical properties of lanthanides, including sharp emission spectra and resistance to

Effect of sub-minimum inhibitory concentrations of metronidazole, vancomycin, and fidaxomicin on growth rate and virulence of Clostridioides difficile.

Clostridioides difficile infection (CDI) is one of the most common health care-associated infections, primarily driven by the use of broad-spectrum antibiotics, which disrupts gut microbiota and facilitates bacterial overgrowth. This study aimed to investigate the effect of sub-minimum inhibitory concentrations (sub-MICs: 0.25× and 0.5×) of metronidazole (MTZ), vancomycin (VAN), and fidaxomicin (FDX) on growth rate, motility, toxin production, and sporulation of toxigenic C. difficile ribotype (RT001) and a non-toxigenic C. difficile strain (ATCC 700057, RT038). RT-qPCR was employed to assess …
