COMPARISON BETWEEN TWO TYPES OF ABDUCTION ORTHOTICS IN TREATING CONGENITAL CLUBFOOT.

OBJECTIVE: The objective of this study was to analyze and compare the effectiveness of two types of abduction orthotics used for the feet, the Denis-Browne type (traditional) and the Dobbs type (dynamic), with regard to maintenance of deformity correction and prevention of recurrence . METHOD: In this comparative retrospective case study, information was collected from the medical records of children with idiopathic congenital clubfoot (CCF). We evaluated a total of 43 feet in 28 patients, which were divided into two groups. Group 1 was comprised of 16 patients with a total of 24 CCFs treated…

Clubfoot relapse: does presentation differ based on age at initial relapse?

PURPOSE: Treatment of idiopathic clubfoot with the Ponseti method is now standard, but predicting relapse can be difficult. Most experts recommend bracing to the age of four years, but this can be challenging for families, and may not be necessary in all patients. The purpose of this study is to compare patterns of bracing and age of relapse to help determine if predictable patterns exist. METHODS: The 70 patients with idiopathic clubfoot treated initially with the Ponseti technique who had relapse of their clubfoot were identified. Relapse was defined as a return to casting or surgery due to…

Apoptotic genes expression in placenta of clubfoot-like fetus pregnant rats.

OBJECTIVE: To investigate the apoptotic gene expression of placenta in an all-trans-retinoic acid (ATRA) induced fetus congenital clubfoot pregnant rat model. METHODS: Sprague-Dawley (SD) rats were divided randomly into ATRA-exposed group and control group. On day 10 of pregnancy, a dose of 120 mg/kg ATRA dissolved in mineral oil was given intragastrically to the rats in the ATRA-exposed group and equivalent volume of mineral oil was given intragastrically to the control rats. Fetuses were delivered on day 20 of pregnancy, the placenta was collected for the pathological and biochemical analys…

Genome-wide association study identifies new disease loci for isolated clubfoot.

BACKGROUND: Clubfoot is a common congenital birth defect with complex inheritance patterns. Currently, the genetic and morphological basis of clubfoot is poorly understood. To identify genetic risk factors associated with clubfoot, we performed a genome-wide association study of common genetic variants. METHODS: The DNA of 396 isolated clubfoot patients and 1000 controls of European descent was genotyped for >600 000 single nucleotide polymorphisms (SNP) using the Affymetrix 6.0 array. Replication was performed with an independent cohort of 370 isolated clubfoot cases and 363 controls of Euro…

Ponseti Brasil: a national program to eradicate neglected clubfoot - preliminary results.

BACKGROUND: After hearing about the reproducible and excellent results of the Ponseti method for clubfoot treatment, a group of Brazilian orthopaedic surgeons organized and participated in a standardized national program to teach the Ponseti technique in 21 different cities across Brazil. METHODS: A total of 21 Ponseti symposiums were organized in a standard fashion from January, 2007 to December, 2008. They consisted of a two-day program with lectures, hands-on cast application, and discussion of local clinical cases presented by orthopaedic surgeons. Thirteen Brazilian orthopaedic surgeons,…

Comparison between Ponseti's and Kite's clubfoot treatment methods: a meta-analysis.

The objective of our study was to compare both Kite's and Ponseti's methods to evidence which one is the most efficient technique in the treatment of congenital idiopathic clubfoot, based on a meta-analysis of current scientific literature. We performed a search of the past 20 years of literature (1986 to 2006) on MEDLINE, LILACS, and EMBASE databases for clinical trials that compared both Kite's and Ponseti's methods. The search in the literature provided 4 selected papers for the meta-analysis. There was a significant difference between the groups, in which the Ponseti's group was more effec…

Quantitative gait characteristics of children who had successful unilateral clubfoot operation.

OBJECTIVES: In this study, we aimed to evaluate the functional results of patients operated for unilateral clubfoot who had good clinical outcome with gait analysis. We also investigated the deviation from the normal, and determined the compensation mechanisms by comparing this data with the unaffected feet and with the feet of healthy children. METHODS: Sixteen children [10 boys, 6 girls; mean age, 6.8 years (range 4-9 years)] with surgically treated unilateral clubfoot and 24 age-matched healthy children were included in the study. Foot length, calf circumference, ankle range of motion, and…

Three-dimensional hindfoot motion in adolescents with surgically treated unilateral clubfoot.

Advances in imaging and computerized analyses have enabled three-dimensional bone motion in the treated clubfoot to be measured precisely. Three-dimensional translations and rotations of the talus, calcaneus, navicular, and cuboid of surgically treated clubfeet were less in magnitude and sometimes different in direction (or without motion in a specific plane) compared with the contralateral normal feet. Surgical techniques used for clubfoot treatment do not restore normal hindfoot bone motion when examined with high-resolution magnetic resonance imaging, computer reconstruction, and image anal…

Calcaneus gait following treatment for clubfoot: preliminary results of surgical correction.

Calcaneus gait is a known complication of surgical treatment of clubfoot, and is characterized by weak triceps surae strength combined with limited ankle plantarflexion at terminal stance, preventing adequate power generation. We evaluated the results of attempted reconstruction of this functional disturbance in 13 symptomatic patients (17 feet) using kinematic and kinetic analysis at a minimum of 1 year postoperatively. Three types of procedures were performed: group 1 (n=7) received calcaneal osteotomy alone; group 2 (n=6) received calcaneal osteotomy or hindfoot fusion combined with tendon …

Pulsed color-flow Doppler analysis of arterial deficiency in idiopathic clubfoot.

This prospective study used pulsed color-flow Doppler sonography to determine differences in the presence and direction of flow through the dorsalis pedis, posterior tibial, and peroneal arteries in a group of children with clubfoot and a comparison group of controls. There was a statistically significant difference in the prevalence of deficient (absent or retrograde flow) dorsalis pedis arteries in children with clubfoot (45%) compared with controls (8%). This indicates that there is an association between some clubfeet and deficiency of the dorsalis pedis artery. There was a trend toward di…

Reduced tibial torsion in congenital clubfoot: CT measurements in 24 patients.

We determined leg torsion with CT scans through the femoral condyles and the ankle joints in 24 children (17 boys) with congenital clubfoot. In 16 cases, there was a bilateral clubfoot, in 5 cases it was left-sided and in 3 cases, it was on the right side. These children were compared to 17 healthy boys and 7 girls. The ages of all children ranged between 3 and 12 years. The average external torsion of the leg in the patients with clubfoot was 20 (15) versus 31 (7) degrees in the healthy children (p = 0.002). This study shows that there are great variations in leg torsion in children with cong…

Posteroplantar release for congenital clubfoot in children younger than 1 year.

One hundred thirty-three resistant congenital clubfeet in 93 patients between 3 and 10 months of age were operated on using a standardized posteroplantar release. Clinical and radiographic assessments were done with a mean followup of 7 years 4 months (range, 3-12 years). Using the McKay score, 79.7% of the surgically treated clubfeet were classified as having a good or excellent result. Three patients had relapse of their clubfoot that required additional surgery. Seventeen feet in 15 patients had residual forefoot adduction at the time of followup. The radiographs showed that the early poste…
