Cochlear implants in aplasia and hypoplasia of the cochleovestibular nerve.

OBJECTIVE: To report on the outcome of four patients with aplasia or hypoplasia of the cochleovestibular nerve who have received a cochlear … aplasia/hypoplasia of the cochleovestibular nerve should be counseled with caution with respect to cochlear implantation, but particular circumstances may justify the intervention. At present these circumstances seem to be a type IIa aplasia or hypoplasia in which the end organ (cochlea or common cavity) still connects to a neural structure on MRI.

Temporal bone pathology of a patient with cochlear implant.

A 66-year-old man with profound total deafness received a 22-channel cochlear implant. After surgery, his hearing improved and the postoperative course was uneventful except transient vertigo. The patient died of lung cancer after 2.5 years. Twelve hours after death, the temporal bone was harvested for histological and electron microscopic examination. The mastoid air cells were preserved, although a few mucosal folds were present. The Dacron mesh tie had largely been absorbed, but part remained embedded in fibrous tissue. No inflammation was noted in the tympanic cavity or the round window th

Cochlear implant soft surgery: fact or fantasy?

A basic surgical principle is to be as gentle as possible to accomplish the goals of the operation. The concept of "soft surgery" for cochlear implants consisted of a small, localized cochleostomy and gentle electrode insertion, the hope being that by limiting damage to the inner ear, superior hearing results might be obtained. The technique includes deferring the cochleostomy until immediately before electrode insertion, use of a large burr to flatten the promontory, followed by a smaller burr to expose the endosteum, preservation of the endosteum of the scala tympani, smoothing of the bony e

[Auditory and communication learning in patients with cochlear implants].

Nowadays the cochlear implant (CI) offers a new method of hearing, especially for deaf patients who do not benefit from conventional hearing aids. However, it may be difficult to get used to this kind of hearing advice. CI users have special problems in interpreting strange acoustic sensations and relating them to auditory features of language and noise they knew before the onset of deafness. An interdisciplinary approach is very important for rehabilitation. Beside otological examination and the surgical implantation, audiological assessment is needed for the correct adjustment of the speech

Stapedius reflex threshold in cochlear implant patients.

The contralateral stapedius reflex elicited by sinusoidal electrostimulation via cochlear implant was investigated in 12 patients. Contraction of the middle ear muscle was monitored by time-resolved synchronized digital sampling of the impedance change in the contralateral ear. The reflex was detected in all patients in whom the uncomfortable loudness level (UCLL) could be reached and who had intact middle ear function. Generally, the reflex threshold is located in the upper part of the dynamic range between the most comfortable loudness (MCL) and the UCLL. The demonstrated method may provide

Histologic evaluation of temporal bones with cochlear implants.

Ectopic bone in the scala tympani is characteristic of cochlear otosclerosis and meningitis, which are the two most frequent etiologies of deafness in cochlear implant patients. The ectopic bone in a multiple-electrode implanted bone is probably caused by disease (syphilis) rather than by the electrode or electrical stimulation. The sensory elements being stimulated by intracochlear electrodes probably are the ganglion cells.

Evaluation of the Prognostic Capacity of the Imaging Features in Cochlear Implant Patients with Cochlear Nerve Deficiency.

BACKGROUND: Cochlear nerve deficiency (CND) is a prevalent inner ear malformation, and several studies have identified a correlation between imaging features and postoperative outcomes in cochlear implant patients with CND. Nevertheless, the extent to which various imaging features influence postoperative cochlear implantation (CI) outcomes remains unclear. This study sought to identify the imaging parameter with the most significant impact on postoperative outcomes in CND patients. METHODS: A total of 107 CND children were included in this study. Imaging features, including internal auditory…

Correlation between cochlear nerve size and cochlear implant rehabilitation in sensorineural hearing loss.

BACKGROUND: Cochlear implantation (CI) is a standard intervention for congenital severe to profound sensorineural hearing loss. However, postoperative auditory and speech outcomes vary considerably across patients, and the developmental status of the cochlear nerve is considered a major determinant of rehabilitation efficacy. OBJECTIVE: This study aimed to assess the correlation between cochlear nerve cross-sectional area (CNCSA) and outcomes after cochlear implantation (CI). METHODS: We retrospectively analyzed 136 bilateral CI recipients, comparing auditory and speech rehabilitation result…

Interaural correlation of acoustic hearing preservation following sequential cochlear implantation.

INTRODUCTION: Compared to unilateral cochlear implantation, bilateral cochlear implantation demonstrates multiple advantages in speech understanding in noise and sound localization, especially when acoustic hearing is preserved in both ears. However, outcomes and factors influencing acoustic hearing preservation following cochlear implant (CI) remains poorly understood. Patients receiving sequential cochlear implantation are a valuable population to investigate as studying bilateral ears allows for a high degree of control of individual factors that may be affecting trends in hearing. METHODS…

Cochlear Implantation and Facial Nerve Stimulation: Clinical and Anatomic Correlations.

OBJECTIVES: One rare complication of cochlear implantation (CI) is facial nerve stimulation (FNS). The aim of the study was to analyze the location and insertion depths for electrode contacts causing FNS. The anatomical variance of the human facial nerve canal (FNC) was explored to elucidate the mechanisms underlying FNS. METHODS: Data were collected from adults that had received a CI from a single tertiary care center. Medical records from 307 patients were retrospectively reviewed. The Uppsala human temporal bone library was leveraged to analyze anatomical structures relevant to FNS. Micro-…

Hampshire Sheep as a Large-Animal Model for Cochlear Implantation.

BACKGROUND: Sheep have been proposed as a large-animal model for studying cochlear implantation. However, prior sheep studies report that the facial nerve (FN) obscures the round window membrane (RWM), requiring FN sacrifice or a retrofacial opening to access the middle-ear cavity posterior to the FN for cochlear implantation. We investigated surgical access to the RWM in Hampshire sheep compared to Suffolk-Dorset sheep and the feasibility of Hampshire sheep for cochlear implantation via a facial recess approach. METHODS: Sixteen temporal bones from cadaveric sheep heads (ten Hampshire and si…

Does Childhood Cochlear Implantation Spill Over to Carers' Employment Status?

OBJECTIVES: Carers of children with disabilities, especially primary carers, tend to have poorer labor market outcomes than carers of typically developing children. However, the extant literature has been largely silent on whether interventions for children's disabilities spill over to carers' employment outcomes, if at all. We aimed to fill this gap. DESIGN: We analyzed data from the Longitudinal Outcomes of Children with Hearing Impairment study, which is a unique panel dataset of Australian children who are deaf or hard-of-hearing (DHH). The Longitudinal Outcomes of Children with Hearing I
