Glory or cage? A qualitative exploration of moral injury among retired college soldiers in Mainland China.

OBJECTIVE: Moral injury correlates significantly with posttraumatic stress disorder, depression, and suicide, affecting soldiers' combat effectiveness and well-being. Despite growing awareness, it is an emerging research area, mainly studied in Western military cohorts. In China, research on its mechanisms and influencing factors in the unique social-cultural context is scarce. Retired college soldiers are crucial, being high-quality military reserves and national defense educators. Their moral injury during service impacts postretirement adaptation and college conscription. Thus, exploring th

Identifying Inter-Individual Differences in Cognitive Decline Using the Brain Connectome in Osteoporosis.

BACKGROUND: Osseous structures have been recognized as an endocrine organ that bidirectionally interacts with the brain. Osteoporosis (OP) is a systemic endocrine disorder linked to neurodegenerative disorders. This bone-brain axis interdependence highlights the necessity of cognitive monitoring in OP management to detect early neurodegeneration markers, particularly given individual variability in brain reserve that may predispose patients to accelerated cognitive decline. PURPOSE: To investigate the individual differences in functional connectome and its association with cognitive ability i

Predictors of independent ambulation at discharge following hip fracture surgery: A focus on the relative contribution of skeletal muscle mass and cognitive function.

BACKGROUND: Skeletal muscle mass is often associated with long-term outcomes, but its specific influence on short-term recovery, such as walking ability at discharge, remains unclear. This study aimed to determine whether preoperative skeletal muscle mass or other clinical factors, including cognitive function, serve as stronger predictors of early functional recovery after hip fracture surgery. METHODS: We retrospectively analyzed 350 patients with proximal femoral fractures who were ambulatory before surgery. Skeletal muscle mass was assessed using the psoas muscle index measured on preoper

Empathy on the edge: A neurocognitive model of empathy in aging and dementia.

Empathy, the ability to understand others' perspectives (cognitive empathy) and share their emotions (emotional empathy), is critical for social functioning across the lifespan. Although empathy changes with age and is often disrupted in neurodegenerative disease, prior research has largely examined isolated components or focused narrowly on healthy aging or specific disorders. This systematic review addresses this gap by synthesizing behavioral and neuroimaging evidence on empathy across typical and pathological aging, from healthy older adulthood and mild cognitive impairment to Alzheimer's

Stressor anticipation and subsequent affective well-being: A link potentially explained by perseverative cognitions.

Anticipatory stress can prospectively and negatively influence diverse outcomes, including cognitive performance and emotional well-being. It has been suggested that perseverative cognitions (e.g., worry, rumination) during the anticipation period constitute a key mechanism driving these effects. The present study investigated the temporal dynamics among stressor anticipation, perseverative cognitions, and affective well-being. To accurately test the suggested mechanism, we focused on how these dynamics unfold within individuals over time. To that end, we analyzed data from an ecological momen

Working with dreams and nightmares: A review of the research evidence.

In this article, we describe methods for working with dreams and nightmares in individual psychotherapy, provide clinical examples, and review research evidence of immediate and distal outcomes of each method. An original meta-analysis of eight studies using the cognitive-experiential dream model with 514 clients showed moderate effect sizes for session depth and insight gains. In the nightmare treatment literature, a previous meta-analysis of 13 studies with 511 clients showed moderate to large effects in reducing nightmare frequency and small to moderate effects in decreasing sleep disturban

Butyrylcholinesterase genotype and gender influence Alzheimer's disease phenotype.

Retrospective data are presented to support a spectrum of early Alzheimer's disease (AD) along a continuum defined by gender and genotype. T … on of the functional integrity of the neural network may be an important component of strengthening cognitive reserve and significantly delaying the onset and progression of dementia, particularly in females. Prospective confirmation of these hypotheses is required. Implications for future research and therapeutic opportunities are discussed.

The association between sleep and cognitive function in people with spinal cord injury (SCI).

PURPOSE/OBJECTIVE: While there is evidence in other clinical groups to suggest that sleep problems can negatively impact cognitive performance, this relationship has not yet been examined in people with spinal cord injury (SCI). Thus, we sought to examine the association between sleep and cognitive function in people with SCI. RESEARCH METHOD/DESIGN: Over the course of 7 days, 167 individuals with SCI completed daily subjective ratings of sleep (sleep quality, number of hours slept per night, and bedtime variability) and wore a wrist-worn device that continuously monitored autonomic nervous s

CADASIL: a practical review for the neurologist.

CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy) is a hereditary small vessel disease caused by pathogenic NOTCH3 variants, predominantly affecting cysteine residues in epidermal growth factor-like repeats. Clinically, patients present with migraine with aura, subcortical ischaemic strokes, mood disturbances, apathy and progressive cognitive decline, with disability accumulating over decades. Neuroimaging typically shows symmetrical white matter hyperintensities, lacunar infarcts and cerebral microbleeds. Its diagnosis relies on genetic testi

Age-Triggered Cognitive Testing of Late-career Physicians: Time to Reassess Current Practices.

Physicians and health care organizations have been urged to respond to public concerns regarding age-associated cognitive impairment among physicians while avoiding ageist stigma and preserving due process. Because cognitive aging is heterogeneous, many clinicians retain or deepen clinically relevant capacities while others develop cognitive, medical, or sensory impairments that raise justifiable safety concerns. A key ethical and evidentiary paradox confounds discussions regarding current assessment policies: low scores on a focused cognitive assessment are found in many impaired physicians b

Cognitive deficits in Spanish-speaking Mexican children with developmental dyslexia.

OBJECTIVE: Developmental dyslexia (DD) has been related to deficits in multiple cognitive skills. Phonological processing deficits are the most frequently reported in opaque orthographies, but there are few studies of transparent orthographies, such as Spanish. The aim of this study was to comprehensively explore possible deficiencies in cognitive functions in Spanish-speaking Mexican children with DD, to determine whether these deficits can explain problems with decoding fluency and accuracy, and analyze whether they provide support for some of the explanatory hypotheses of DD. METHOD: An ex

Offloading reduces prospective memory learning.

Across two experiments, we investigated the effects of cognitive offloading on prospective memory (PM). Participants performed two PM tasks, either relying on internal memory for both (Dual PM condition) or using an external reminder for one (Offload condition). Although reminders improved performance on the offloaded PM, such reminders did not improve performance on the concurrent, nonoffloaded PM. When all reminders were removed in a subsequent phase, performance on the previously offloaded PM was significantly impaired, falling below the baseline levels of performance observed for participa

Lumateperone for treatment of psychotic symptoms in Lewy body disease: A case report.

Individuals experiencing Lewy body disease (LBD) are particularly vulnerable to the adverse effects of neuroleptics. This sensitivity has been employed by some authorities as a diagnostic component for this disorder. At present, we do not have any Food and Drug Administration-approved antipsychotic for the management of psychotic symptoms in this condition. We present the first case of an LBD patient, showing favorable response in psychotic symptoms with lumateperone, a novel atypical neuroleptic. Our report revealed improvements in cognition, psychosis, and sleep following the initiation of l

Trajectory-Oriented Brain Vulnerability Framework for Cognitive Decline in Type 2 Diabetes.

Type 2 diabetes mellitus (T2DM) is associated with heterogeneous cognitive and functional trajectories rather than a single diabetes-specific encephalopathy. This structured narrative review proposes a conceptual, hypothesis-generating exposure-injury-structure-function framework linking metabolic, vascular, and frailty-related exposures with molecular and neurovascular injury, structural change, cognitive decline, and loss of independence. Its contribution is the temporal assignment of measurements, lagged testing between adjacent levels, and comparison with clinical-risk and unordered biomar

Bilateral common carotid artery stenosis in normotensive rats impairs endothelium-dependent dilation of parenchymal arterioles.

Chronic cerebral hypoperfusion is a risk factor for cognitive impairment. Reduced blood flow through the common carotid arteries induced by bilateral carotid artery stenosis (BCAS) is a physiologically relevant model of chronic cerebral hypoperfusion. We hypothesized that BCAS in 20-wk-old Wistar-Kyoto (WKY) rats would impair cognitive function and lead to reduced endothelium-dependent dilation and outward remodeling in the parenchymal arterioles (PAs). After 8 wk of BCAS, both short-term memory and spatial discrimination abilities were impaired. In vivo assessment of cerebrovascular reserve c
