Long-term Risk of Stroke in Patients With Inflammatory Bowel Disease: A Population-Based, Sibling-Controlled Cohort Study, 1969-2019.

BACKGROUND AND OBJECTIVES: Patients with inflammatory bowel disease (IBD) are at an increased risk of thromboembolic events, but evidence on the long-term risk of stroke remains scarce. We aimed to explore whether patients with a biopsy-confirmed IBD had an increased long-term risk of stroke. METHODS: This cohort included all patients with biopsy-confirmed IBD in Sweden between 1969 and 2019 and up to 5 matched reference individuals per patient who were randomly selected from the general population and IBD-free full siblings. The primary outcome was incident overall stroke; secondary outcomes

Primary Epiploic Appendagitis Mimicking Acute Appendicitis: A Case Report and Narrative Review of the Literature.

Primary epiploic appendagitis is uncommon and is estimated to induce 1.1-1.3% of all abdominal pain. We report a 42-year-old male who appeared in the morning in the emergency department with abdominal pain localized in the right lower abdomen and associated with anorexia and nausea. Clinical examination, laboratory tests, and abdominal ultrasound revealed deep tenderness at Mc Burney point and a mild elevation of CRP (0.7 mg/dL). In the evening, the symptoms were exacerbated, and a diagnostic laparoscopy was performed. Intra-operatively, the appendix was normal and a twisted, necrotic epiploic

MDCT in acute ischaemic left colitis: a pictorial essay.

The pathogenesis of acute ischaemic colitis depends on two different forms of vascular colonic insult: occlusive injury and non-occlusive injury. Clinically, ischaemic colitis may be classified as two major forms: mild (non-gangrenous) and acute fulminant (gangrenous). The classic presentation is abdominal pain, diarrhoea and/or rectal bleeding, but it is not specific and highly variable and so the diagnosis usually depends on clinical suspicion and is supported by serologic and colonoscopic findings. Imaging methods have their role in diagnosing IC. While plain radiography and ultrasound can

A case of idiopathic deep vein thrombosis improved with glucocorticoid therapy.

A healthy, middle-aged Japanese man with no family history of thrombotic disorders presented with acute abdominal pain due to ischemic colitis. Two months later, he developed left leg pain and swelling. A venogram of the lower limbs, computed tomography, and a scintigram of pulmonary blood flow revealed deep vein thrombosis of the left lower limb extending to the inferior vena cava and emboli of both pulmonary arteries with bilateral pleural effusions. The responsible coagulation disorder was not detected in this case. Since these thrombi were refractory to the thrombolytic therapy with urokin

Assessing trends, morbidity, and mortality in ruptured abdominal aortic aneurysm repair with 9 years of data from the National Surgical Quality Improvement Program.

BACKGROUND: The American College of Surgeons' National Quality Improvement Program (NSQIP) database can be used to assess trends and outcomes of ruptured abdominal aortic aneurysm (rAAA) repair. The purpose of this study is to examine the morbidity and mortality for ruptured endovascular (rEVAR) and ruptured open (rOPEN) aneurysm repair compared with elective endovascular (EVAR) and elective open (OPEN) aneurysm repair. METHODS: Ruptured and nonruptured abdominal aortic aneurysms were identified from the NSQIP database between 2008 and 2016. Data regarding demographics and comorbidities, 30-d

Esophageal necrosis and perforation associated with the anticardiolipin antibody syndrome.

The anticardiolipin antibody syndrome has been previously associated with seven cases of gastrointestinal ischemia involving the duodenum, jejunoileum, or colon. In prior cases patients presented with gastrointestinal bleeding, abdominal pain, or an acute abdomen without gastrointestinal perforation. A patient with prior pulmonary emboli, right leg thrombophlebitis, and right popliteal artery thrombosis associated with anticardiolipin antibodies developed fatal esophageal ischemia. Postmortem examination revealed esophageal necrosis and perforation due to esophageal vascular thrombosis, as wel

Gastrointestinal Basidiobolomycosis Mimicking Crohn's Disease in a Young Adult.

Gastrointestinal basidiobolomycosis (GIB) is a rare, emerging fungal infection caused by Basidiobolus ranarum. It primarily affects immunocompetent individuals and often presents as a mass lesion mimicking colorectal malignancy or inflammatory bowel disease. Due to its non-specific presentation and rarity, GIB is frequently misdiagnosed, leading to delayed or inappropriate management. In this article, we present a case of GIB in a young male in his 20s who presented with a four-month history of intermittent and recurrent severe abdominal pain associated with significant weight loss. Based on t

Fecal colitis obliterans and cytomegalovirus enteritis after pancreaticoduodenectomy for resectable pancreatic cancer.

A 74-year-old male patient with a resectable pancreatic cancer underwent radical surgery after receiving neoadjuvant chemotherapy. The patient went into shock after surgery owing to sepsis caused by fecal impaction. The sepsis healed rapidly after the patient was encouraged to defecate through stool evacuation. He subsequently presented with gastrointestinal bleeding from an aneurysm, requiring interventional radiology hemostasis and blood transfusion. Bleeding continued after arterial embolization. On the 35th day after surgery, cytomegalovirus enteritis was detected during a lower gastrointe

Effects of perioperative granulocyte colony-stimulating factor on horses with ascending colonic ischemia.

Granulocyte colony-stimulating factor (G-CSF) is a glycoprotein that regulates the proliferation and maturation of hematopoietic progenitor cells and modulates the function of mature neutrophils. The responses to administration of G-CSF alone, and in combination with antimicrobials, were studied in an equine model of ascending colon ischemia. Complete segmental colonic ischemia (3.75 hours) with pelvic flexure enterotomy was created in four treatment groups. Group 1 horses received recombinant canine G-CSF (10 micrograms/kg, every 24 hours, intramuscularly), gentamicin sulfate (2.2 mg/kg, ever

Total bowel ischemia after carbon dioxide angiography in a patient with inferior mesenteric artery occlusion.

A 70-year-old man with a history of hypertension and chronic renal failure experienced claudication in his left leg. He was treated by endovascular stenting in his left leg. We used CO2 angiography to prevent contrast-induced nephropathy. After the procedure, livedo vasculitis and lactic acidosis developed. We diagnosed the patient with CO2-induced ischemic colitis, after which we changed his position and administered conservative therapy. Contrast-enhanced computed tomography showed total bowel ischemia and total occlusion at the ostium of the inferior mesenteric artery. This rare complicatio

Extensive sclerosing mesenteritis of the rectosigmoid colon associated with erosive colitis.

Sclerosing mesenteritis is a rare, idiopatic, usually benign, inflammatory process of the mesenteric adipose tissue. The most common site of involvement is the small bowel mesentery. We present a case of sclerosing mesenteritis of the rectosigmoid colon as a cause of severe abdominal pain, abdominal obstruction, and ischemic colic mucosal lesions. Contrast enema, colonoscopy, angiography, and CT were the imaging modalities used. A 20 cm diameter, fibrotic mass causing extensive compression of rectosigmoid colon was found at laparotomy. Histological examination showed extended fibrosis, inflamm

Colonic complications of acute pancreatitis.

Colonic complications of acute pancreatitis include "pseudo-obstruction," necrosis, hemorrhage, fistula, and ischemic colitis. With the ten cases reported in this article, there are now 75 cases reported in the literature to our knowledge. The fulminating lesions (necrosis and hemorrhage) are usually associated with pancreatic abscess and/or pseudocyst and may occur because of a direct pressure effect with secondary vascular compromise. The lesions are predominant in the transverse colon and at the splenic flexure. Because the risk factor for a colonic complication from pancreatitis is highest

Cocaine-associated abscesses with lethal sepsis after splenic infarction in an 17-year-old woman.

Well known complications related to cocaine use are myocardial insufficiency, myocardial infarction, myocarditis, aortic dissection, neurologic damages, ischemic colitis, thrombotic phenomenons, renal infarction and acute liver failure. Cases of splenic infarctions related to cocaine use are extremely rare. A 17-year-old drug addict was found by her boy-friend liveless in her bed. She was well known using cocaine since years. Autopsy revealed multiple splenic infarctions with secondary mixed bacterial infection and abscesses. Petechial bleedings were found and microabscesses in the myocardium,

[Gastrointestinal complications related to NSAIDs].

Chronic use of non-salicylate NSAIDs causes in most individuals an asymptomatic enteropathy involving the small bowel, particularly its distal part. This enteropathy is characterised by an increase in intestinal permeability and a mild mucosal inflammation. Hypoalbuminemia and iron deficiency may occur. In addition, non-salicylate NSAIDs may cause focal lesions of the small intestine. Ulcerations and ulcers, that can be accidentally discovered during an ileoscopy, may cause acute or chronic bleeding. Deep ulcers may provoke sudden peritonitis. Small bowel diaphragms are rare fibrotic lesions,

Studies on lymphocyte subpopulations in human colonic biopsy specimens by colonoscopy.

Lymphocytes, isolated by the enzymatic technique, from colonic mucosal specimens obtained from eight patients with ulcerative colitis (UC), two with ischemic colitis, two with antibiotic-associated colitis, and 15 controls by colonoscopic biopsy were used to examine T and B cells. In control biopsy specimens, the mononuclear cell yield averaged 3.6 +/- 0.3 (SEM) X 10(6)/g with a viability of 93 +/- 1%, while T and B cells expressed as a percentage of total lymphocyte counts were 65 +/- 1% and 15 +/- 1%, respectively. T and B cells in the ascending, transverse, and sigmoid colon and rectum did

Peritonitis on sigmoidal perforation in a cocaine user: A rare case report.

INTRODUCTION AND IMPORTANCE: Cocaine, the second most abused drug in Morocco after cannabis, has been associated with multiple cardiac, pulmonary, neurological, and digestive complications. Colonic perforation following cocaine abuse is relatively lesser-known and requires attention as abuse rates are increasing, and existing evidence is scarce. Only a few cases have been reported in medical literature. CASE PRESENTATION: We report the case of a 42-year-old male cocaine addict who presented with acute peritonitis. A laparotomy revealed a 3 cm perforation in the sigmoid, The absence of radiolo

[Drug induced diarrhea].

Diarrhea is a frequent adverse event involving the most frequently antibiotics, laxatives and NSAI. Drug induced diarrhea may be acute or chronic. It may be due to expected, dose dependant properties of the drug, to immuno-allergic or bio-genomic mechanisms. Several pathophysiological mechanisms have been described resulting in osmotic, secretory or inflammatory diarrhea, shortened transit time, or malabsorption. Histopathological lesions sometimes associated with drug induced diarrhea are usually non specific and include ulcerations, inflammatory or ischemic lesions, fibrous diaphragms, micro
