Effective Communication During and After an Amniotic Fluid Embolism.

Amniotic fluid embolism (AFE) is a life-threatening complication occurring during pregnancy, labor, or birth and is characterized by sudden cardio-respiratory collapse and disseminated intravascular coagulopathy. Amniotic fluid embolism is unpredictable, sudden, and complex. Amniotic fluid embolism is one of the causes of peripartum maternal mortality and morbidity in the United States. Nurses and other health care professionals responding to severe maternal events, such as an AFE, may experience trauma, which can lead to reduced executive functions, and challenges with memory, organization, p

Community financing of health care in Africa: an evaluation of the Bamako initiative.

The Bamako Initiative, a controversial attempt to strengthen Primary Health Care using community financing and community participation and management was launched at a meeting of African Ministers of Health in 1987. This evaluation focuses particularly on the community financing aspects of the Initiative. Previous experiences of community financing highlight particular lessons for the development of the Initiative and issues likely to be encountered in implementation attempts. Four country case studies of the Initiative at its early stages of implementation (in June, July and August 1991) were

Emergency preparedness planning: lessons from COVID-19 and the opioid crisis in rural Ohio.

The COVID-19 pandemic amplified existing public health emergencies, including the overdose crisis. In rural areas, the effect of emergency preparedness planning on local responses to the pandemic is unknown. We conducted 52 semi-structured interviews with two participant groups (28 people who use drugs [PWUD] and 24 local stakeholders) in two rural counties in southern Ohio. Interviews explored service disruptions, perceptions of the pandemic, organizations' pre-existing emergency plans, and communication channels. Transcripts were analyzed thematically to identify data patterns within and bet

Global goods movement and the local burden of childhood asthma in southern California.

OBJECTIVES: As part of a community-based participatory research effort, we estimated the preventable burden of childhood asthma associated with air pollution in the southern California communities of Long Beach and Riverside. METHODS: We calculated attributable fractions for 2 air pollution reduction scenarios to include assessment of the newly recognized health effects associated with residential proximity to major roads and impact from ship emissions. RESULTS: Approximately 1600 (9%) of all childhood asthma cases in Long Beach and 690 (6%) in Riverside were attributed to traffic proximity.

Female Community Health Workers and Health System Navigation in a Conflict Zone: The Case of Afghanistan.

Afghanistan ranked 171st among 188 countries in the Gender Inequality Index of 2011 and has only 16% of its women participating in the labor force. The country has been mired in violence for decades which has resulted in the destruction of the social infrastructure including the health sector. Recently, Afghanistan has deployed community health workers (CHW) who make up majority of the health workforce in the remote areas of this country. This paper aims to bring the plight of the CHWs to the forefront of discussion and shed light on the challenges they face as they attempt to bring basic heal

Reproductive desires of men and women living with HIV: implications for family planning counselling.

The reproductive desires of people living with HIV/AIDS (PLHIV) of low socioeconomic standing attending public health facilities in South Africa were studied. HIV-positive men, pregnant and non-pregnant women were recruited from two clinics at a large public hospital in Tshwane, South Africa. Individual interviews were used to explore the reproductive desires of HIV-positive participants. HIV counsellors' perceptions of their clients' reproductive desires were explored during focus group discussions. Parenthood proved to be an important factor to all participants in continuation of the family

Women's fears and men's anxieties: the impact of family planning on gender relations in northern Ghana.

The Navrongo experiment, a family planning and health project in northern Ghana, has demonstrated that an appropriately designed, community-based family planning program can produce a change in contraceptive practice that had been considered unattainable in such a setting. Simultaneously, however, evidence suggests that newly introduced family planning services and contraceptive availability can activate tension in gender relations. In this society, where payment of bridewealth signifies a woman's requirement to bear children, there are deeply ingrained expectations about women's reproductive

Developing family planning services for female chronic mentally ill outpatients.

The family planning needs of chronic mentally ill women outpatients, many of whom do not use contraceptives and are at high risk for unwanted pregnancies, have been rarely addressed. Taking a thorough sexual history is the first step in assessing patients' need for family planning services. Patient education, including instruction about physiological processes and contraceptive methods and assertiveness training, is the most important service component. Offering family planning services in the mental health center has many advantages, including better communication between mental health care p

Assessing the Provision of Person-Centered Contraceptive Care at Publicly Supported Clinics Providing Contraceptive Services in the United States.

INTRODUCTION: We sought to understand the extent to which the contraceptive care provided at publicly supported family planning clinics in the United States aligns with aspects of person-centered care. MATERIALS AND METHODS: We conducted a descriptive study with a national sample of U.S. family planning clinics between November 2022 and December 2023. We measured person-centeredness by the scope of services offered, including the extent to which providers solicit and prioritize patients' contraceptive intentions, preferences, and goals; the dispensing protocols for various contraceptive metho

A systematic literature review of psychological factors and the development of late whiplash syndrome.

This systematic literature review aims to assess the prognostic value of psychological factors in the development of late whiplash syndrome (LWS). We included prospective cohort studies that provided a baseline measure of at least one psychological variable and used outcome measures relating to LWS (i.e. pain or disability persisting 6 months post injury). A search of electronic databases (Pubmed, Medline, Cinahl, Embase and Psychinfo) up to August 2006 was done using a predetermined search strategy. Methodological quality was assessed independently by two assessors. Data extraction were carri

Public health response to bioterrorism with Bacillus anthracis: coordinating public health laboratory, communication, and law enforcement.

In October 2001, public health departments across the United States were part of an intensive response to a bioterrorism event using anthrax spores delivered by mail. It is useful to examine this experience as an unscripted exercise of bioterrorism response capacity, more realistic than scenarios of planned exercises. The event particularly challenged public health laboratory and communications capacity, but it also tested surveillance and training capacity. The bioterrorism response demonstrated the importance of strong partnerships between the public health laboratory and emergency response

Promoting planned care for patients with indwelling catheters.

Indwelling urinary catheters are common among patients in the community. Use of a catheter entails a significant lifestyle adjustment on the part of the patient, who must learn a new regime of self-care. In order to help patients to cope with the new device, it is essential that nurses play an active role in educating patients and involving them in the decision-making process. By empowering patients through knowledge of their condition and care, nurses can encourage greater compliance with care plans and reduce the risk of potentially distressing complications that can arise from indwelling ca

Managed alcohol: one community's innovative response to risk management during COVID-19.

BACKGROUND: Harm reduction programs often lack community-based support and can be controversial, despite data demonstrating effectiveness. This article describes one small Alaskan community's development of a harm reduction managed alcohol program (MAP) in the context of a city-run quarantine site for individuals experiencing homelessness. The MAP was developed to support quarantining by COVID-19-exposed or COVID-positive individuals who also experienced chronic homelessness, a severe alcohol use disorder, and heightened health risks related to potentially unsupported alcohol withdrawal. Responding to heat-related health risks: the urgency of an equipoise between emergency and equity.

In the summer of 2024, there were higher temperatures than usual in several parts of India. Temperatures in Delhi, a huge city with millions of residents, broke several previous records. Low-income households have dwellings that do not offer much protection from the heat, and individuals struggle to access basic amenities, such as water. With accumulating evidence on consequent impacts for mortality and morbidity, governance structures are seeking to respond in a timely and efficient manner. There is a need to recognise that heat-related illnesses and deaths are not best addressed merely as an
