Highly time-resolved 4D MR angiography using golden-angle radial sparse parallel (GRASP) MRI.

Current dynamic MRA techniques are limited by temporal resolution and signal-to-noise penalties. GRASP, a fast and flexible MRI technique combining compressed-sensing, parallel imaging, and golden-angle radial sampling, acquires volumetric data continuously and can be reconstructed post hoc for user-defined applications. We describe a custom pipeline to retrospectively reconstruct ultrahigh temporal resolution, dynamic MRA from GRASP imaging obtained in the course of routine practice. GRASP scans were reconstructed using a custom implementation of the GRASP algorithm and post-processed with Me

Low grade oncocytic tumors of the kidney: a clinically relevant approach for the workup and accurate diagnosis.

Renal oncocytoma and chromophobe renal cell carcinoma were accepted as unique renal tumors in the late 1990s. Since their formal description, criteria for diagnosis have evolved and additional distinct tumor subtypes originally considered as one these two entities are now recognized. The last two decades have witnessed unprecedented interest in the spectrum of low grade oncocytic renal neoplasms in three specific areas: (1) histologic characterization of tumors with overlapping morphologic features between oncocytoma and chromophobe renal cell carcinoma; (2) description of potentially unique e

Peripheral Nerve Anatomy Revisited: Modern Requirements for Neuroimaging and Microsurgery.

Detailed anatomic investigation of peripheral nerve topography underlies the correct application of intraoperative neuromonitoring (IONM) and ultrasonography, both well-established methods to prevent nerve palsy during surgical operations and to elucidate pathomechanisms in disease. In this study, we analyzed the anatomy of selected peripheral nerves in the head and neck regions to improve the outcome of endocrine and migraine surgeries. Anatomic dissections of 204 hemilarynges were performed to study the topography of the inferior laryngeal nerve (ILN). Measurements were taken from the lower

Titanium Acetabular Component Deformation under Cyclic Loading.

Acetabular cup deformation may affect liner/cup congruency, clearance and/or osseointegration. It is unclear, whether deformation of the acetabular components occurs during load and to what extent. To evaluate this, revision multi-hole cups were implanted into six cadaver hemipelvises in two scenarios: without acetabular defect (ND); with a large acetabular defect (LD) that was treated with an augment. In the LD scenario, the cup and augment were attached to the bone and each other with screws. Subsequently, the implanted hemipelvises were loaded under a physiologic partial-weight-bearing moda

Determining constitutive behavior of the brain tissue using digital image correlation and finite element modeling.

Detailed knowledge about the mechanical properties of brain can improve numerical modeling of the brain under various loading conditions. The success of this modeling depends on constitutive model and reliable extraction of its material constants. The isotropy of the brain tissue is a key factor which affects the form of constitutive models. In this study, compression tests were performed on different parts of the sheep brain tissue. Also, the digital image correlation (DIC) method was utilized to investigate the direction dependency of brain parts considering their microstructures. To this ai

Angiographic embolization followed by piecemeal resection of giant posterior mediastinal schwannoma: Case report and concise review.

INTRODUCTION: Posterior mediastinal masses present unique diagnostic and therapeutic challenges, particularly when large highly vascularized tumors extend toward or emanate from the spinal cord. The rare nature of these tumors precludes the development of standardized management algorithms, underscoring the importance of case reports. PRESENTATION OF CASE: A 57 year old female presented with exertional dyspnea and right chest pressure. Chest radiography followed by computed tomography (CT) scan demonstrated a 13 cm posterior mediastinal mass involving the T7 vertebral body. CT-guided percutan

Application of exercise transcutaneous oxygen pressure measurements for detection of proximal lower extremity arterial disease: a case report.

Proximal claudication is secondary to ischemia caused by peripheral artery disease (PAD), whereas proximal pseudo-claudication is secondary to other disease processes such as hip arthritis, spinal stenosis, neuropathy, and so forth. The differentiation between the two can be challenging. Exercise transcutaneous oxygen pressure measurement (exercise-TcPO2) allows noninvasive detection of flow-reducing lesions in the proximal arteries and tributaries of the lower extremity arterial tree. We present the first case report in the United States using an exercise-TcPO2 algorithm. A 71-year-old diabet

Regional variations in fascicle curvatures within a muscle belly change during contraction.

During muscle contraction, the fascicles curve in response to changes in internal pressures within the muscle. Muscle modelling studies have predicted that fascicles curve to different extents in different regions of the muscle and, as such, curvature is expected to vary along and across the muscle belly. In the present study, the local variations in fascicle curvature within the muscle belly were investigated for a range of contractile conditions. B-mode ultrasound scans of the medial and lateral gastrocnemii muscles were collected at five ankle positions-ranging from dorsiflexion to plantarf

Amyotrophic lateral sclerosis and degenerative cervical myelopathy: phenotype-based diagnostic pitfalls, investigative mismatch, and practical clinical reasoning.

Differentiating amyotrophic lateral sclerosis (ALS) from degenerative cervical myelopathy (DCM) remains difficult because the two disorders can converge clinically while diverging biologically. ALS is a progressive motor neuron disease, whereas DCM is a potentially treatable compressive myelopathy; however, both may present with upper-limb weakness, hand wasting, hyperreflexia, gait disturbance, and cervical MRI abnormalities. This narrative review examines ALS-DCM overlap through the concept of explanatory sufficiency: whether the available clinical, imaging, and electrophysiological evidence

Hemostatic Use of Over-the-Scope Clips in Non-Variceal Upper Gastrointestinal Bleeding: A Narrative Review.

The over-the-scope clip (OTSC) system is increasingly used for the endoscopic hemostasis of bleeding non-variceal upper gastrointestinal lesions. OTSCs provide secure, full-thickness tissue compression. The Stop the Bleeding Trial (STING-1) was a German multicenter randomized controlled trial (RCT) that compared OTSC to standard treatment (through-the-scope clips and contact thermal devices) for refractory bleeding ulcers. The rate of further bleeding was significantly reduced with the use of OTSC (19 of 33 patients [57.6%] in the standard therapy group and 5 of 33 patients [15.2%] in the OTSC

An Intrinsically Explainable Approach to Detecting Vertebral Compression Fractures in CT Scans via Neurosymbolic Modeling.

Vertebral compression fractures (VCFs) are a common and potentially serious consequence of osteoporosis. Yet, they often remain undiagnosed. Opportunistic screening, which involves automated analysis of medical imaging data acquired primarily for other purposes, is a cost-effective method to identify undiagnosed VCFs. In high-stakes scenarios like opportunistic medical diagnosis, model interpretability is a key factor for the adoption of AI recommendations. Rule-based methods are inherently explainable and closely align with clinical guidelines, but they are not immediately applicable to high-

Clinical course of a tailgut cyst transformed to retrorectal mucinous adenocarcinoma: A case report.

INTRODUCTION AND IMPORTANCE: Tailgut cysts (TGC) are rare retrorectal lesions originating from embryological remnants of the hindgut. Malignant transition is rare; treating it with an unplanned diagnostic algorithm can complicate the clinical course. CASE PRESENTATION: We report a case of retrorectal mucinous adenocarcinoma arising from a TGC in a 63-year-old female. She had experienced constipation, recurrent urinary tract infections, and difficulty voiding for three months. A total hysterectomy was performed elsewhere for a presumed leiomyoma. A transrectal biopsy was negative, but partial

Unified Framework for Oculomotor Nerve Reconstruction: Tractography-Based Anatomical Assessment.

BACKGROUND AND PURPOSE: The oculomotor nerve (OCN) innervates the eye muscles and can be affected by inflammatory, compressive, and pathological conditions. Diffusion MRI (dMRI) tractography shows the potential ability to describe the trajectory of the OCN. However, reconstruction of the OCN in the cavernous sinus is still challenging due to the complex tissue environment at the skull base. METHODS: In this study, we integrated anatomical knowledge to propose a unified framework for OCN tractography, using 45 dMRI datasets from the Human Connectome Project subjects aged 22-36 years and data f

Management and treatment of perioperative hypersensitivity.

PURPOSE OF REVIEW: Perioperative hypersensitivity reactions are rare but potentially catastrophic events. This review acts to summarize recent recommendations for both immediate and poststabilization management of suspected reactions, alongside practical advice for anaesthetists who may be faced with these events. RECENT FINDINGS: Prompt treatment is essential but may be hampered by delay in recognition. This can occur because there are multiple differential diagnoses for the observed clinical signs as well as variations in clinical presentation. Resuscitation is dependent on the use of adren
