Can continuous glucose monitoring-derived glycemic variability predict retinal neurodegeneration earlier than HbA1c in patients without clinically apparent retinopathy?

Early screening for diabetic retinopathy (DR) has long relied on the detection of fundus vascular lesions. However, accumulating evidence indicates that retinal neurodegeneration may occur before overt microvascular abnormalities become clinically visible. At the stage of no clinically apparent diabetic retinopathy (NDR), glycated hemoglobin A1c (HbA1c), by virtue of its averaging nature, is unable to adequately reflect the acute metabolic stress imposed by short-term glycemic variability (GV) on the retinal neurovascular unit (NVU). The increasing use of continuous glucose monitoring (CGM) ha…

Closed-Loop Bioelectronic Artificial Pancreas Patch for Continuous Monitoring and Regulation of Blood Glucose in Diabetic Rats and Pigs.

Integrated artificial pancreas devices with continuous glucose monitoring (CGM) and closed-loop insulin delivery are crucial for diabetes management. However, design challenges remain, including miniaturization, low cost, stability, and low power consumption. Current commercial products are expensive ($3,000 to $8,000 USD), and bulky, typically exceeding 100 cm3. Here, a closed-loop bioelectronic artificial pancreas patch is presented for continuous blood glucose regulation in diabetic rats and pigs. By designing transiently dissolvable microneedle arrays incorporated with microtube arrays, in…

Safety and efficacy of continuous glucose monitoring devices in individuals with diabetes undergoing hyperbaric oxygen therapy: a scoping review.

INTRODUCTION: Continuous glucose monitoring devices (CGMs) have emerged as an effective approach to optimise glycaemic control for individuals living with diabetes mellitus. Despite CGMs offering improved patient satisfaction and quality of life, they have been primarily validated for outpatient and home use. This has posed a challenge for patients and providers who wish to incorporate CGMs into clinical settings such as hyperbaric oxygen therapy (HBOT). Those with advanced diabetes mellitus who have diabetic foot ulcers that are refractory to treatment are among the most prevalent users of HB…

Performance of flash continuous glucose monitoring and glycemic marker correlations in Chinese pregnant women with non-type 1 diabetes.

BACKGROUND: Maternal diabetes significantly increases the risk of adverse maternal and neonatal outcomes. Traditional self-monitoring of blood glucose is often invasive and limited in its ability to capture glycemic variability. Flash continuous glucose monitoring (FCGM) offers a promising alternative; however, its reliability and correlation with biochemical markers such as hemoglobin A1c (HbA1c) and glycated albumin (GA) in pregnant women with gestational diabetes mellitus (GDM) and type 2 diabetes mellitus (T2DM) remain underexplored. AIM: To evaluate the performance of the FreeStyle Libre…

Hypoglycemia prevention via pump attenuation and red-yellow-green "traffic" lights using continuous glucose monitoring and insulin pump data.

BACKGROUND: Hypoglycemia has been identified as a primary barrier to optimal management of diabetes. This observation, in conjunction with the introduction of continuous glucose monitoring (CGM) devices, has set the stage for achieving tight glycemic control with systems that adjust the insulin pump settings based on measured glucose concentrations. Because system safety and system reliability are key considerations, there is a need for algorithms that reduce the risk of hypoglycemia in closed-loop, open-loop, and advisory-mode systems. More specifically, the algorithm presented here is formul…
