[Allergological diagnostics and current allergens in occupational dermatology].

A broad spectrum of various substances may cause occupational allergic contact dermatitis. Often, it is difficult to identify potentially or truly relevant allergens at the workplace. Therefore, the consulting physician must have a comprehensive knowledge of possible allergens in different occupations to perform specific diagnostics and to initiate efficient secondary prevention by allergen avoidance. In this review, we give some practically relevant general comments on patch testing in occupational dermatitis, followed by a discussion of important occupational allergens in those occupational

Epidemiology of occupational skin disease in Singapore 1989-1998.

This is an epidemiologic study of occupational skin disease in Singapore. All patients diagnosed with occupational dermatoses in the National Skin Centre, Singapore, over the 10-year period 1989-1998 were studied retrospectively. Irritant contact dermatitis was found to be more common than allergic contact dermatitis. The major sources of occupational dermatitis in Singapore were the metal/engineering, building/construction, electrical/electronics and transport industries. The main irritants were detergents/wet work, solvent and oil/grease. The main allergens were chromate, rubber chemicals an

Contact allergy in the cleaning industry: analysis of contact allergy surveillance data of the Information Network of Departments of Dermatology.

BACKGROUND: Occupational contact allergy is a common problem in the cleaning industry. OBJECTIVES: To identify the most frequent occupation-associated allergens and time trends in contact allergy in female cleaners. METHODS: We analysed the patch test data concerning 803 female cleaners, who were evaluated for occupational contact dermatitis in 45 dermatological departments contributing to the Information Network of Departments of Dermatology (IVDK) from 1996 to 2009. Female patients, except cleaners, with occupational dermatitis (n = 14494) and female controls without occupational dermatiti

Etiology of hand dermatitis among patients referred for patch testing.

BACKGROUND: Hand dermatitis affects many people and has numerous causes. Few reports have looked at the etiology of hand dermatitis across broad demographics. OBJECTIVES: The purposes of this investigation were to (1) determine the causes of hand dermatitis in the population patch-tested at a dermatologic referral clinic, (2) report occupations frequently associated with hand dermatitis, and (3) indicate which substances were the more common allergens among individuals evaluated by patch testing for hand dermatitis. METHODS: A retrospective analysis of patch-test data on 1,034 patients who w

Occupational dermatoses.

PURPOSE OF REVIEW: All terms referring to job-related (occupational) rashes are clearly defined. Ranking as the second most common occupational disorder, the importance of occupational dermatoses is illustrated. RECENT FINDINGS: 90-95% of occupational dermatoses result from a contact-type dermatitis. Until recently, 4/5 cases were believed to be of the irritant, rather than allergic type of contact dermatitis, however, with more extensive patch testing, more than half the cases were found to be allergic. SUMMARY: The direct causes of occupational dermatoses: chemical, mechanical, physical, a

Clinical and epidemiological features of textile contact dermatitis: an Italian multicentre study.

BACKGROUND: The prevalence of occupational and non-occupational textile dermatitis seems to be increasing, probably because of changed textile manufacturing techniques. OBJECTIVE: Evaluation of the clinical features and epidemiology of textile contact dermatitis (TCD) in Italy. METHODS: Demographic data, clinical history aspects, atopy and positive patch test reactions to occupational and non-occupational allergens were investigated in 277 textile dermatitis patients. RESULTS: Contact dermatitis was the most frequent clinical presentation (95.3%). TCD was more common in females, in the four

Epoxy-based production of wind turbine rotor blades: occupational dermatoses.

Occupational dermatoses were investigated in a factory producing rotor blades for wind turbines by an epoxy-based process. In a blinded study design, 603 workers were first interviewed and thereafter clinically examined. Based on a history of work-related skin disease, clinical findings of dermatitis, or both, 325 (53.9%) of the workers were patch tested with a specially profiled occupational patch-test series and the European standard patch-test series. Calculated on all investigated workers, 17.1% of the workers were diagnosed with occupational dermatoses caused by work. Occupational allergi

Contact dermatitis among marking nut workers in rural India.

BACKGROUND: Informal processing of Semecarpus anacardium (marking nut) provides livelihood to many women in rural India. The nut contains potent phenolic compounds with strong irritant and allergenic properties, yet occupational contact dermatitis related to this exposure remains poorly documented in community settings. AIMS: To describe the prevalence, clinical severity and occupational determinants of contact dermatitis among female informal workers involved in marking nut processing in rural Vidarbha, India. METHODS: A community-based cross-sectional study was conducted among all eligible

[Secondary lymphedema of the hand as a complication of recurrent erysipelas in irritant contact dermatitis].

A 54 year old female hairdresser suffered from occupational irritant contact dermatitis of the hands. As a complication of hand dermatitis, secondary lymphoedema developed following recurrent erysipelas. Fissures occurring in the course of the hand dermatitis presumably enabled entry of streptococci. Treatment of this complication of occupational dermatitis was reimbursed by the Employers Liability Insurance.

Improving Recovery of Irritant Hand Dermatitis in Healthcare Workers With Workplace Interventions During the COVID-19 Pandemic.

INTRODUCTION: Occupational hand dermatitis is common among healthcare workers, with increased incidence during the COVID-19 pandemic. Irritant contact dermatitis accounts for the majority of occupational hand dermatitis and is largely due to frequent contact with hand hygiene products. Long-term prognosis of occupational contact dermatitis is often very poor. This study aims to identify and implement suitable workplace interventions to aid in the recovery of occupational irritant hand dermatitis among healthcare workers during the COVID-19 pandemic. METHODS: A quality improvement (QI) project

[Overview of five years of occupational dermatology: The role of atopy].

BACKGROUND: The management of occupational dermatitis is often complex and relies on skills in both dermatology and occupational medicine. A bidisciplinary occupational dermatology consultation was set up in 2002 at Brest CHU. The aim of this study is to provide an assessment of 5 years of activity and to discuss the role of atopy. PATIENTS AND METHODS: From October 2002 to October 2007, 245 patients attended this consultation. 145 files concerning proven occupational dermatosis were studied (age, sex, occupation, dermatosis, allergens and history of atopy). One hundred and five patients comp

Contact dermatitis is an unrecognized problem in the construction industry: Comparison of four different assessment methods.

BACKGROUND: A high contact dermatitis symptoms prevalence has been observed in Dutch construction workers. METHODS: Contact dermatitis was diagnosed by an expert panel using questionnaire data and photographs of 751 subjects' hands. A subset was evaluated by two occupational physicians. Their diagnoses were compared to those of the expert panel. In addition, two self-reported questionnaire-based assessment methods were compared to the expert panel evaluation. Associations between contact dermatitis and determinants were assessed using log-binomial regression analysis. RESULTS: Contact dermat

Carriers of filaggrin gene (FLG) mutations avoid professional exposure to irritants in adulthood.

BACKGROUND: Loss-of-function mutations in the filaggrin gene (FLG) are associated with xerosis, atopic dermatitis, and early onset of hand eczema. Irritant exposure is a risk factor for occupational hand eczema, and FLG mutations increase the risk of occupational irritant contact dermatitis on the hands in hospital cohorts. It is unknown whether FLG mutations affect the level of irritant exposure. OBJECTIVES: To evaluate whether exposure to occupational irritants was dependent on FLG mutations, atopic dermatitis, and age at hand eczema onset. METHODS: Randomly chosen Danish adults completed

An epidemiological comparison between occupational and non-occupational hand eczema.

Thirty-four per cent of 2110 patients with eczema attending a contact dermatitis clinic had hand eczema. Fifty-five per cent (395 of 721) had contact dermatitis and 45% (326 of 721) had endogenous/unclassifiable eczema. Thirty-five per cent of cases (217 of 721) were occupational eczema. An epidemiological comparison of patients with occupational and non-occupational hand eczema (defined as eczema occurring on the hands up to the wrist line) was made. Hand eczema was more common in males in both groups, and there was a significantly larger proportion of males in the occupational group (65%) th

Environmental aspects of occupational dermatology.

In most cases occupational contact dermatitis is primarily caused by direct localised skin contact with chemical or physical agents, but its development may also depend on wider aspects of the occupational environment. Firstly, allergens may widely contaminate surfaces in sufficient quantity to elicit dermatitis; this is not as often true of irritants. Secondly, occupational contactants may enter the air as molecules, particles or droplets and elicit dermatitis. This can occur with irritants as well as allergens. Thirdly, low humidity of the air itself can cause dermatitis, as well as promotin
