Dermatomyositis associated with chronic myelogenous leukemia.

Dermatomyositis and polymyositis have been well established in association with malignant neoplastic disease. Most commonly, this association has been noted in patients with solid tumors rather than in patients with neoplasms of hematopoietic origin. We describe here a patient with chronic myelogenous leukemia who developed typical dermatomyositis, which responded to therapy with corticosteroids. This concurrence has been reported in only one previous patient to our knowledge. The nature and implications of the association of dermatomyositis and neoplasia are discussed.

[Vasculitis in children's dermatomyositis (author's transl)].

Vascular lesions have been discovered by a number of authors in children's dermatomyositis, when they are not ordinary found in adults. Affecting capillaries, arterioles and veinlets, vasculitis appears as an hyperplasia of intima, then as an endothelial necrosis and formation of thrombi. Usually seen with light microscope, these lesions are sometimes only revealed by electron microscope. These vascular lesions are observed in the muscle and mainly in the perimysial interstitial tissue, in the skin, and in the various visceral localisations of the disease: pulmonary, ocular, renal, nervous and…

Hereditary complement (C2) deficiency with dermatomyositis.

A 60 year old white man in previous good health presented with a 6 month history of progressive muscle weakness. Clinical and laboratory findings were typical of dermatomyositis. Muscle biopsy confirmed the presence of inflammatory myopathy; deposits of immunoglobulin G (IgG), immunoglobulin M (IgM) or third component of complement (C3) were not detected by immunofluorescence. No evidence was found for an associated neoplasm. An unexpected finding was the total absence of serum hemolytic complement activity. Further investigation revealed that the complement defect was attributable to a select…

Paraneoplastic Dermatomyositis Due to Gallbladder Carcinoma.

Dermatomyositis is an inflammatory myopathy that can be sporadic or paraneoplastic. A 72-year-old female presented with a subacute progressive illness characterized by symmetrical proximal weakness of upper and lower limbs, dysphagia, and neck flexor weakness. On examination, she had erythematous, scaly lesions over the periorbital areas, face, and interphalangeal and metacarpophalangeal joints (Gottron's papules). Neurological examination revealed bilateral palatal palsy, proximal predominant upper and lower limb and neck flexor weakness, areflexia with preserved knee jerks, waddling gait, an…

Occult ovarian adenocarcinoma manifesting as dermatomyositis.

A woman in her 60s presented with erythematous lesions predominantly over the joints. After evaluation by dermatology and rheumatology, she was diagnosed with dermatomyositis and initiated on oral steroids and immunosuppressants. She was subsequently referred to gynaecology services for further evaluation of possible malignancies. On pelvic examination, a firm, mobile mass was noted in the right adnexa which was confirmed on imaging. A decision for staging laparotomy was taken. Intraoperatively, a solid mass was observed in the right ovary with no adjacent tumour deposits. Histopathology indic…

Linear juvenile dermatomyositis: A segmental manifestation.

A 7-year-old girl presented with proximal muscle weakness and skin lesions. Physical examination revealed violaceous papules on the right forearm in a blaschkoid distribution. Her symptoms and test results were consistent with juvenile dermatomyositis. An unusual superimposed segmental manifestation of this disease is discussed.

B Cell Lymphocytosis in Juvenile Dermatomyositis.

In this study, we determined if B lymphocytosis may serve as a JDM biomarker for disease activity. Children with untreated JDM were divided into two groups based on age-adjusted B cell percentage (determined through flow cytometry): 90 JDM in the normal B cell group and 45 in the high B cell group. We compared through T-testing the age, sex, ethnicity, duration of untreated disease (DUD), disease activity scores for skin (sDAS), muscle (mDAS), total (tDAS), CMAS, and neopterin between these two groups. The patients in the high B cell group had a higher tDAS (p = 0.009), mDAS (p = 0.021), and n
