[Urethral dilatation with the renal sheath dilator under the ureteroscope to treat male urethrostenosis].

OBJECTIVE: To explore the efficacy of urethral dilatation with the renal sheath dilator under the ureteroscope in the treatment of male patients with urethrostenosis. METHODS: Eighteen male patients with urethrostenosis underwent urethral dilatation with the renal sheath dilator. Under the ureteroscope, a zebra-guide wire was inserted through the stenosed urethra into the bladder and the stenosis was gradually dilated with the renal sheath dilator, followed by placing a Foley catheter of proper size for 1-4 weeks. For children, the renal sheath dilator was selected according to their age, whi…

The epidemiology of idiopathic dilated cardiomyopathy in women: the Washington DC Dilated Cardiomyopathy Study.

We examined possible risk factors for idiopathic dilated cardiomyopathy in women by comparing newly diagnosed cases (N = 61) ascertained from five Washington DC area hospitals with neighborhood controls (N = 122) identified using a random digit dialing technique. We matched the cases and controls on 5-year age intervals and telephone exchange and analyzed the data using conditional logistic regression methods. We found that idiopathic dilated cardiomyopathy was associated with history of asthma, diabetes mellitus, hypertension, and black race. The association with hypertension was particularly

Left atrial mechanical function and clinical performance in non-dilated versus dilated cardiomyopathy.

BACKGROUND: Non-dilated left ventricular cardiomyopathy (NDLVC) is a distinct entity characterized by impaired left ventricular ejection fraction despite normal chamber dimensions. While it may represent an earlier or less fibrotic stage of non-ischemic cardiomyopathy compared to dilated-cardiomyopathy (DCM), its clinical and structural characterization remains incomplete. OBJECTIVES: This study aimed to compare left atrial (LA) phasic strain parameters between patients with NDLVC and those with DCM, all with similarly reduced ejection fractions. Additionally, the relationship between LA func

Peripheral venous dilation using flow-mediated dilation response: A randomized crossover study.

BACKGROUND: Venodilation is crucial in enhancing the success rate of peripheral intravenous cannulation. Flow-mediated dilation (FMD) is a vasodilatory response initiated by temporary ischemia followed by reperfusion. This crossover study aimed to test the hypothesis that FMD induces dilation of the peripheral veins of the forearm. METHODS: Fifteen healthy volunteers underwent the FMD and control conditions in a randomized order. FMD involved a 5-min occlusion of blood flow in the brachial artery, followed by reperfusion, achieved by inflating and deflating a cuff placed on the upper arm. The

Non-dilated left ventricular cardiomyopathy vs. dilated cardiomyopathy: clinical background and outcomes.

AIMS: Non-dilated left ventricular cardiomyopathy (NDLVC) was proposed as a new category of cardiomyopathy that included patients with non-left ventricular (LV) dilatation, LV wall motion abnormality, or LV scar. However, the clinical background and event rates of NDLVC were unclear. The aim of this study was to examine the characteristics and event rates of patients with NDLVC and reduced LV ejection fraction (NDLVC-REF) in comparison with those with dilated cardiomyopathy (DCM). METHODS AND RESULTS: We retrospectively included 363 patients with newly diagnosed non-ischaemic cardiomyopathy a…

Out-patient esophageal dilation: an experience in 130 patients using Savary-Gilliard dilators.

Experience of outpatient esophageal dilation using Savary-Gilliard dilators in 130 patients during a 17 month period is reported. The strictures were radiation induced (48), tumoral stenoses (40), anastomotic (16) and due to other causes (26). Both fluoroscopy and endoscopy were used in 58%, endoscopy alone in 23% and fluoroscopy alone in 19% of patients for placement of the guide wire. One hundred and twenty six stricture dilations (97%) were technically successful. Eighty one (62%) patients could be dilated to 14 mm or more. One hundred and nineteen (94%) patients were dilated in one or two

Treatment of congenital dilatation of the choledochus- reconstruction of the bile duct after resection of the dilated choledochus.

Reconstruction of the way of bile flow was attempted in 21 cases with the resection of the dilated bile duct, and the post-operative courses were observed over a long period. It was found that ascending cholangitis is seldom caused by the hepaticoduodenostomy. It is important, when hepaticoduodenostomy is performed, that the sound section of the wall of the bile duct without stenosis in the hepatic side should be selected. Reconstruction of the way of the bile flow to prevent ascending cholangitis by leading normal bile flow into the duodenum should be devised.

Pneumatic dilation, evaluation of compliance of esophagogastric junction by Pneumatic dilation balloon before POEM.

BACKGROUND AND AIMS: After the introduction of POEM several years ago, the position of pneumatic dilation (PD) in the treatment of achalasia has been challenged. During PD, balloon pressure changes according to the volume of gas injected. This may reflect esophagogastric junction (EGJ) compliance, a parameter recently considered important by POEM operators. In this study, we aimed to assess whether the pneumatic dilation balloon catheter combined with a precise pressure sensor allows for the evaluation of EGJ compliance, and then, help predict the outcome of POEM in the treatment of achalasia.

Integrating genetics and phenotyping in clinical management of dilated and non-dilated left ventricular cardiomyopathy.

Over the past decades, the approach to cardiomyopathies has deeply evolved. It has progressively transitioned from one predominantly based on clinical evaluation and conventional imaging towards an integrated and multidimensional approach that incorporates advanced imaging techniques, tissue characterization, and comprehensive genetic testing. In this perspective, the current classification of cardiomyopathies, from the European Society of Cardiology, has introduced the new entity of non-dilated left ventricular cardiomyopathy (NDLVC) that encompasses a spectrum of diseases from hypokinetic no

Esophageal dilation with EsoFLIP is faster than CRE balloon dilation combined with EndoFLIP in children.

BACKGROUND: Controlled radial expansion (CRE) balloon dilators are traditionally used to dilate esophageal strictures during an esophagogastroduodenoscopy (EGD). EndoFLIP is a diagnostic tool used during an EGD to measure important parameters of the gastrointestinal lumen, capable of assessing treatment before and after dilation. EsoFLIP is a related device that combines a balloon dilator with high-resolution impedance planimetry to provide some of the luminal parameters in real time during dilation. We sought to compare procedure time, fluoroscopy time, and safety profile of esophageal dilati

Characterization and Long-Term Prognosis of Postmyocarditic Dilated Cardiomyopathy Compared With Idiopathic Dilated Cardiomyopathy.

Dilated cardiomyopathy (DC) is the final common pathway of different pathogenetic processes and presents a significant prognostic heterogeneity, possibly related to its etiologic variety. The characterization and long-term prognosis of postmyocarditic dilated cardiomyopathy (PM-DC) remain unknown. This study assesses the clinical-instrumental evolution and long-term prognosis of a large cohort of patients with PM-DC. We analyzed 175 patients affected with DC consecutively enrolled from 1993 to 2008 with endomyocardial biopsy (EMB) data available. PM-DC was defined in the presence of borderline

Review of dilated cardiomyopathies. Dilated cardiomyopathies and altered prothrombotic state: a point of view of the literature.

Heart failure is an enormously important clinical problem that, if not faced, may overwhelm health care resources. Primary and secondary cardiomyopathies cause the majority of cases of clinical heart failure, which is thus better approached from the utility point of view of myocardial failure. Furthermore, the risk of thromboembolic complications presenting in such disease may be higher than in ischemic cardiomyopathy. Intracardiac thrombi and mural endocardial plaques (from the organization of thrombi) are present at necropsy in more than 50% of patients with dilated cardiomyopathy (DCM). Sev

Prognostic Significance of Cystic Biliary Dilatation in Primary Sclerosing Cholangitis.

BACKGROUND/AIMS: Primary sclerosing cholangitis (PSC) is a chronic, progressive disease characterized by inflammatory and fibrotic strictures in the intrahepatic and/or extrahepatic bile ducts. Typical cholangiographic findings include multifocal, annular, short bile duct strictures with minimal upstream dilatation. However, in some patients, this dilatation progresses to cystic dimensions. The current study aimed to investigate whether the clinical features and prognosis of patients with cystic dilatation (CD) of the bile ducts differ from those of patients with classic cholangiographic PSC. …

Acute Respiratory Failure From Massive Esophageal Dilatation due to Achalasia.

Massive esophageal dilation from achalasia can cause acute respiratory failure via lung compression. Prompt recognition and urgent esophageal decompression can rapidly reverse hypoxia and be lifesaving. After stabilization, temporizing or definitive achalasia therapies should be pursued to prevent recurrence.
