BACKGROUND: This study assessed the feasibility of high-resolution magnetic resonance imaging of the pancreas by means of an endoscopically inserted endoluminal magnetic resonance receiver coil.
METHOD: A 0.032-inch diameter internal magnetic resonance imaging receiver coil was endoscopically inserted into the pancreatic duct in 4 pigs through the accessory channel of a standard duodenoscope to obtain high-resolution magnetic resonance images by using T1- and T2-weighted sequences.

Patients with villous tumors of the ampulla Vater usually present with jaundice, intermittent or constant, but may seek care for abdominal pain, intestinal hemorrhage, or pancreatitis. Because villous tumors may harbor carcinoma in 30 to 50 per cent of cases, appropriate management may require radical resection. Duodenal ulcer patients with stigmata of recent hemorrhage are at high risk of rebleeding. A 70-year-old man was admitted with upper GI bleeding. Endoscopy showed a large duodenal bleeding mass. The aspect of the tumor was macroscopically similar to the liver surface. The diagnosis of hepatocellular carcinoma was made by aspiration cytology guided by abdominal computed tomographic scan. The postmortem examination revealed an extensive primary hepatic tumor which caused an ulcer in the first part of the duodenum and protruded into the lumen. This unusual presentation of a hepatocellular carcinoma has not been previously reported.

The upper gastrointestinal tract was investigated endoscopically in 30 patients with paracoccidioidomycosis. In 22 patients who shown filling defects in first and second duodenum, only 2 cases of benign tumors were detected by both radiology and endoscopy; the remaining cases were diagnosed as duodenitis and pseudolesions. In duodenum, the great rarity of benign tumors compared with frequency of pseudolesions doesn't allow evaluation of the accuracy of roentgen examinations. The aim of this study was to correlate endoscopic, radiological and pathological observations in 30 patients with Crohn's disease, with involvement of the stomach and/or duodenum. The incidence of gastroduodenal involvement in our patients with Crohn's disease is about 3%. 22 of the 30 patients had intrinsic lesions of the stomach and/or duodenum; in 8 patients the lesions seemed to be produced by involvement from contiguous diseased bowel. RATIONALE: Endoscopic papillectomy is the standard treatment for noninvasive tumors of the ampulla of Vater. However, reports on the endoscopic resection of large tumors at the major duodenal papilla are rare. The "two-step" endoscopic approach described herein represents the first such report for a giant lesion.
