Hematuria in an Adult with Congenital Heart Disease.

BACKGROUND: Adults with congenital heart disease (CHD) in the United States now outnumber children with CHD, due in part to the improvement in surgical and medical management. This growing population may present postoperatively to the emergency department (ED) with nonspecific complaints from unforseen complications secondary to cardiac intervention. CASE REPORT: We describe a 39-year-old male who presented to the ED with hematuria and dysuria after he underwent percutaneous device ventricular septal defect (VSD) closure 10 days before. Upon initial evaluation, laboratory results confirmed a …

The urethral smear as a tool in diagnosing adenovirus-induced urethritis.

Adenovirus is a recognised cause of non-gonococcal urethritis, and is not uncommonly associated with extragenital signs and symptoms. This case report describes a patient with symptoms of conjunctivitis, meatitis and urethritis. The urethral smear revealed almost exclusively monocytes microscopically, raising the suspicion of a viral aetiology. Results confirmed the presence of adenovirus in both the eyes and urethra. Despite waning reliance on the urethral smear in sexual health clinics, it can still be an important diagnostic tool in assessing the aetiology of non-specific urethritis. Findin

[Systemic adverse effects of topical ocular instillation of atropine in two children].

A medication-related cause must be sought when unusual symptoms occur. Topical treatments, including eye drops, whose side effects are more common in exposed children, need to be verified. We report here the cases of two children who developed systemic symptoms after the administration of atropine-based mydriatic eye drops. A 6-month-old boy was admitted to the emergency department with acute urine retention lasting 36h. Investigations identified only eye drop treatment 3h before the onset of symptoms, with 2 drops of homatropine 1 %, as a cause. No other urinary retention was observed during

[What to do with a normal urinary strip in case of lower urinary tract symptoms?].

Patients very often consult for lower urinary tract symptoms, that do not necessarily equate to common cystitis. When urinary leucocytes and nitrites are absent, the urinary strip has a very good negative predictive value and makes the diagnosis of a lower urinary tract infection very unlikely. One then has to search for other diagnoses and to clarify the nature of the symptoms, irritating or obstructive ones, their duration and to correlate them to the patient's age and gender. In sexually active young patients, infectious diseases predominate, such as uretritis or vaginitis, while, with age,

An unusual cause of renal allograft dysfunction: graft papillary necrosis.

A 43-year-old nondiabetic man, 5 years post-renal transplantation, presented complaining of oliguria, fever and dysuria of 1-day duration. Graft ultrasound did not reveal any obstructive changes. Graft function did not improve in spite of 3 days of antibiotics. On the fourth day he passed fleshy material in urine subsequent to which his urine output improved and fever recovered. His graft function settled near to the previous baseline. Histological analysis of the material revealed necrosed renal papillary tissue. Renal papillary necrosis in allograft is uncommon and generally reported in the …

Androgen deprivation-mediated cytoreduction before interstitial brachytherapy for prostate cancer does not abrogate the elevated risk of urinary morbidity associated with larger initial prostate volume.

PURPOSE: We examined whether prostate volume reduction after a short course of androgen deprivation (AD) lowered the risks of acute and chronic urinary morbidity related to radioactive seed implantation for low-risk prostate cancer. METHODS AND MATERIALS: Eighty-one patients received AD for cytoreduction before interstitial brachytherapy alone. Urinary morbidity was carefully assessed for all patients during a median followup of 53 (range, 23-78) months after treatment. Outcomes were then compared with those of a control group of 81 patients who were matched 1:1 based on identical prostate vo

The tinkle tale.

The clinical features of urinary calculi are well documented. Several textbooks state the features of passage of a stone per urethra including severe dysuria, straining and strangury. We report on three patients in whom attention was drawn to the passage of a stone by the sound made as the stone hit the bedpan during urination.

[Combination therapy in the management of urinary disorders after transurethral resection of the prostate].

INTRODUCTION: Surgery in patients with large prostates due to benign prostatic hyperplasia (BPH) results in severe dysuria manifesting as frequent or urgent need to urinate and urinary incontinence. Traditionally, these conditions are treated with alpha blockers. Recently, alpha-blockers and the anticholinergic combination have been used showing additional advantages. This study aimed to investigate the comparative effectiveness of monotherapy versus combination therapy in the treatment of postoperative dysuria in BPH patients with large prostates undergoing transurethral surgery. MATERIALS A…

A Case of Xanthogranulomatous Pyelonephritis Associated with Renoduodenal Fistula.

Xanthogranulomatous pyelonephritis (XGP) is a rare form of pyelonephritis associated with repeated infection, chronic inflammation, and obstruction. Various fistulas, including those to the intestine, are a known association with XGP. Here, a 55-year-old woman with a history of multiple previous renal calculi presented with dysuria and back pain. Contrast-enhanced computed tomography (CT) revealed a soft tissue density in her renal pelvis and perirenal space consistent with XGP along with a tract connecting the upper pole of her right kidney to the second portion of the duodenum. This finding …

An Organic Intravesical Foreign Body Caused by Penetrating Trauma that was Missed during Initial Management.

We report a case of an intravesical foreign body that was incompletely removed endoscopically and that defied diagnosis with current diagnostic tools. A 65-year-old man visited Korea University Anam Hospital complaining of dysuria and a sensation of residual urine. His medical history included an intravesical foreign body caused by penetrating trauma, and he had undergone endoscopic removal of foreign bodies 1 year previously. After additional remnant intravesical foreign bodies were found, he had undergone additional endoscopic removal and his urinary symptoms subsided. After 2 years, however…

Renal calyceal microlithiasis: clinical presentation may precede sonographic evidence.

Calyceal microlithiasis (CM) is characterized by the sonographic finding of hyperechogenic spots less than 3 mm in diameter in renal calyces, and it may be the first step in calculus formation. From January 1992 to January 1998, we have observed 216 children with CM. The present report deals with 34 of them in whom renal sonography was negative at first observation and the diagnosis of CM was made only on repeated sonographic examinations. The presenting symptoms were recurrent abdominal pain, dysuria, and hematuria occurring alone or in combination. Half of the patients had hypercalciuria. A …

Etiology of benign prostatic hyperplasia.

The natural history of benign prostatic hyperplasia (BPH) involves two phases. The first, or pathologic phase, is composed of two stages, microscopic and macroscopic, neither of which produces symptomatic clinical dysuria. Nearly all men throughout the world eventually develop microscopic BPH if they live long enough. In only about 50% of men, however, does microscopic BPH grow to produce a macroscopic enlargement of the gland, suggesting that additional factors are required for BPH progression. The second, or clinical, phase of BPH involves the progression of pathologic BPH to its clinical fo…

The dipslide in diagnosis of urinary tract infections.

Urinary tract infections (UTI's) are common in medicine. Symptoms may include fever, chills, frequency, and dysuria. Asymptomatic UTI's are also common, with a prevalence of one percent in school girls and ten percent in pregnant women. Pyuria, dysuria, and frequency may be absent in patients with UTI's or present in patients without UTI's. Therefore, a UTI must be bacteriologically diagnosed as greater than 100,000 organisms/ml, usually of a single organism, in a properly obtained urine specimen. The dipslide is a simple, convenient, inexpensive device for the quantitative diagnosis of a UTI.…
