Estimated economic costs of obesity to U.S. business.

OBJECTIVE: To estimate the economic costs of obesity to U.S. business. METHODS: Standard epidemiologic methods for risk attribution and techniques for ascertaining cost of illness were used to estimate obesity-attributable expenditures on selected employee benefits, including health, life, and disability insurance and paid sick leave by private-sector firms in the U.S. in 1994. Data were obtained from a variety of secondary sources, including the National Health Interview Survey, reports from the Bureau of Labor Statistics and other federal agencies, and the published literature. Economic evaluation of benazepril in chronic renal insufficiency.

A prospective, randomized, double-blind trial recently demonstrated that treating patients with chronic renal insufficiency with benazepril significantly decelerates the rate of progression of the disease. We tested the hypothesis that preventative treatment with the angiotensin converting enzyme (ACE) inhibitor benazepril in patients with chronic renal insufficiency is cost-effective. A Markov chain model was used that considered regular treatment, hemodialysis, continuous ambulant peritoneal dialysis, transplantation, rejection and death. Clinical trial data were used to estimate the effects

Economical test methods for developmental neurobehavioral toxicity.

The assessment of behavioral changes produced by prenatal or early postnatal exposure to potentially noxious agents requires both the designing of ad hoc tests and the adaptation of tests for adult animals to the characteristics of successive developmental stages. The experience in designing tests is still more limited than in the adaptation of tests, but several tests have already proven their usefulness; some examples are the suckling test, the homing test, and evaluations of dam-pup and pup-pup interactions. Functional observational batteries can exploit the development at specified postnat

Economic evaluation of prenatal diagnosis: a methodological review.

This paper discusses the role of economic evaluation in prenatal diagnosis. As the availability of new and improved techniques of prenatal diagnosis increases, so does the relevance of economic evaluation. The various methods of economic evaluation are discussed in the context of amniocentesis and chorionic villus sampling. It is argued that in view of the potentially wide range of benefits from prenatal diagnosis, more sophisticated measures of benefit should be incorporated into economic evaluations. Further development of utility-based measures and monetary valuation is suggested.

Vitamin A status of socio-economically backward children.

A comprehensive survey was carried out to asses the Vitamin A status of pre-school (0-6 yrs). and school age (6-12 yrs.) children of socio-economically backward families from slums of Bombay and its suburbs. The Vitamin A, protein, calories and iron from the rice and dal based diet was found to be below recommended dietary allowances (RDA). Among the 1956 children surveyed 20% of the children showed low (< 20 micrograms/dl) serum vitamin A levels. 4.8% of the children were suffering from one or the other signs of Vitamin A deficiency. [Economic evaluation and decision making in environmental health].

Due to the increasing deterioration of the environment and its possible consequences for the health of the population the design and implementation of policies for the control of economic activities according to criteria for the protection of human health and the environment itself have become an unquestionable need. Such policies would include the economic feasibility of existing alternatives for protection. Due to the huge interests involved, however, other factors, both social and political should also be taken into consideration. Economic evaluation has been seen as a promising foundation

Ischaemic stroke in New Zealand: an economic study.

AIM: To quantify and describe the annual cost of ischaemic stroke to New Zealand society during 1992. METHODS: A prevalence or cross sectional approach employing incremental estimation was used to estimate costs from the perspective of society. The study used unit record hospital and mortality data. RESULTS: Direct costs ranged from $93 million to $140 m and loss of production was between $6 m and $14 m. Total quantified costs were estimated to lie between $99 m and $154 m. Hospital and continuing care costs when combined make up 90% of all quantified costs. CONCLUSION: Ischaemic stroke aff

Medical and economic consequences of gang-related shootings.

Treatment costs for victims of gang violence have fueled the withdrawal of hospitals from trauma networks. Not included in such tallies are the medical resources that these seriously ill and medically indigent patients divert from other areas. We examined the surgical care requirements, costs incurred, and outcomes at a Level I trauma center. Local law enforcement records were matched with hospital admissions over a 1-year period to identify with hospital admissions over a 1-year period to identify casualties of gang violence. Of 191 gunshot wound admissions, 107 (56%) were gang related. The m

The epidemiological and economic consequences of AIDS clinical trials.

The Food and Drug Administration (FDA) must manage the delicate balance between speed and safety in the evaluation and approval of new drugs. To explore how these competing obligations might be formalized with respect to the AIDS epidemic, we present a simple decision-theoretic optimization model of the clinical trials process against a backdrop of HIV transmission. Our framework sheds light on such issues as the economic consequences of decisions, the potential savings of a new therapy, the costs to society of delay, the value of better information, and how and when to undertake a clinical tr

Medical and economic parameters of motorcycle-induced trauma.

A retrospective study was conducted on all patients injured in a motorcycle accident who were admitted to the authors' institution during a one-year period. The 71 patients evaluated averaged 26 years of age; 79% were men, 75% were not wearing a helmet, and 24% were legally intoxicated. Sixty-six percent required surgical intervention and 36% a second procedure. There were 167 fractures, with an average of 2.4 per patient. The 27 patients requiring a blood transfusion averaged 10.5 units per patient. Motorcyclists not wearing a helmet had an increased risk of head injury (p less than .01). Tho

An "economic" theory of addiction, hypomania, and sensation seeking.

Research into the relationship between mania, sensation seeking, and substance abuse has shown that hypomanics and sensation seekers use substances frequently and often for the purpose of adjusting their moods. This essay develops an "economic" theory of addiction that attempts to account for those phenomena. The theory also supports the notion that sensation-seeking behavior and variants of manic behavior share many of the characteristics of the addictive behaviors. Implications for the treatment of addictive behaviors are discussed. Data from drug users in the military (Navy) are presented t

Screening for congenital dislocation of the hip: an economic appraisal.

The direct costs of screening for congenital dislocation of the hip (CDH) are compared with the treatment costs resulting from no screening in a cost-effectiveness analysis in British Columbia. Under certain conditions the costs associated with screening and subsequent conservative treatment for 6 to 15 positive cases of CDH/1000 liveborn infants were considerably lower than the costs of either open or closed reduction of the hip for 1.5 infants with CDH per 1000 infants not screened. When adjustments were made to the assumptions about screening costs, rates with which cases were missed and ho

[Therapeutic and economic aspects of dracunculosis in a Parisian hospital].

We studied 18 cases of dracunculosis treated in our department of Internal Medicine in Paris. The mean duration of the hospital stay is 38 days (15 to 90). The cost of this stay, calculated on the basis of the daily charge, accounts for 93.3 % of the total cost. The longest stays are due to serious subcutaneous infections at the site of the worm and to multiple or complex associated diseases. Average charge for laboratory test, roentgenograms or other investigations amounts to 4.4 % of the cost of the hospital stay. A significant part of these investigations is warranted by cellulitis and the
