Aerosolized salbutamol accelerates the resolution of pulmonary edema after lung resection.

BACKGROUND: Ischemia-reperfusion injuries, fluid overload, and cardiac insufficiency may all contribute to alveolar and interstitial lung edema. We hypothesized that aerosolized salbutamol would reduce extravascular lung water and improve oxygenation after lung resection by stimulating epithelial fluid clearance and cardiovascular function. DESIGN: Blinded, randomized, cross-over trial. METHODS: We selected 24 patients with risk factors for lung edema. Aerosolized drugs (salbutamol, 5 mg; vs ipratropium, 0.5 mg) were administered on two consecutive trials, with a 6-h washout period, on the d

Cardiac arrest and pulmonary hypertension in scurvy: a case report.

We report a case of a six-year-old boy who presented after a cardiac arrest, likely due to a pulmonary hypertensive crisis in the setting of vitamin C deficiency. After initially presenting with subacute multifocal bone lesions of unknown etiology, he experienced a pulseless electrical activity cardiac arrest while undergoing a diagnostic procedure under sedation. During his post-arrest convalescence, he developed persistent tachycardia and peripheral edema. An echocardiogram revealed findings consistent with significant pulmonary arterial hypertension, which was found to be responsive to inha

Hemorrhage in cardiac conduction tissue in premature infants.

Six premature infants had hemorrhages that were limited to cardiac conduction tissues. Obstetric complications occurred in each; four were growth retarded, and four had perinatal asphyxia. Two babies survived beyond one week. Four survived less than 24 hours. Bradyrhythmias occurred in four. Extracardiac lesions included infections and renal failure in two infants, hyaline membrane disease in one, hypoplastic lungs in two (one with renal agenesis and placental amnion nodosum), and placental villous edema in two. Cardiac hemorrhage occurred in sinoatrial nodes (n = 1), atrioventricular nodes (n

[Thoracic lymphadenopathy due to vascular transformation of lymph node sinuses associated with upper limb edema in a chronic hemodialysis patient with congestive heart failure].

BACKGROUND: Vascular transformation of lymph node sinuses (VTLS) is a rare disorder characterized by transformation of lymph node sinuses into endothelium-lined capillary-like channels. This phenomenon was originally discovered by accident whilst examining regional lymph nodes draining cancer. However, it has been found in association with other conditions associated with lympho venous congestion and distension, such as congestive heart failure (CHF) or even lymphoadenopathy alone. CASE REPORT: We describe the clinical case of a male dialysis patient with CHF (secondary to ischemic-hypertensi

An Unusual Case of Acquired Dilated Cardiomyopathy Due to Long-Standing Patent Ductus Arteriosus.

Patent ductus arteriosus (PDA) is a common congenital cardiac condition in neonates. Its frequency is substantially higher in preterm infants, and it is frequently associated with other congenital heart defects. Its persistence into adulthood has become increasingly uncommon due to advances in neonatal screening, echocardiographic diagnosis, and timely therapeutic intervention. The hemodynamic burden imposed by PDA depends on the magnitude of the left-to-right shunt and may result in both short- and long-term complications. While small shunts may remain asymptomatic throughout life, moderate a

Improvement in left ventricular ejection fraction and fluid compartment dynamics in patients with heart failure and type 2 diabetes treated with SGLT-2 inhibitors.

OBJECTIVE: To evaluate the longitudinal changes in body fluid compartments and their association with cardiac recovery in patients with concomitant heart failure (HF) and type 2 diabetes mellitus (T2DM) receiving sodium-glucose cotransporter-2 (SGLT-2) inhibitor therapy. METHODS: In this prospective, single-center study, 64 patients diagnosed with both HF and T2DM were initiated on SGLT-2 inhibitor therapy and followed up for 12 months. Clinical and laboratory parameters, along with bioelectrical impedance analysis (BIA), were evaluated at baseline, 6 months, and 12 months. The key analytical

Percutaneous left atrial decompression in patients supported with extracorporeal membrane oxygenation for cardiac disease.

OBJECTIVES: Left atrial decompression using cardiac catheterization techniques has been described at centers with extracorporeal membrane oxygenation programs. Left atrial decompression can decrease cardiogenic edema, minimize ventricular distension, and allow myocardial recovery. We describe Boston Children's Hospital's experience with percutaneous left atrial decompression techniques, acute outcomes, and clinical impact of left atrial decompression in extracorporeal membrane oxygenation patients. SUBJECTS: Patients supported with extracorporeal membrane oxygenation undergoing percutaneous l

Amyloid cardiomyopathy: a hidden heart failure cause that is often misdiagnosed.

BACKGROUND: Congestive heart failure (CHF) is a very common condition, especially in the elderly, characterized by dyspnea, orthopnea, nocturnal paroxysmal dyspnea, and peripheral edema. CLINICAL PRESENTATION AND INTERVENTION: We report the case of a 76-year-old with CHF symptoms for the last 3 months. The ECG and transthoracic echocardiogram were suggestive of CHF due to amyloid cardiomyopathy. After cardiac MRI, a positive Congo red staining of subcutaneous fat aspiration and a negative genetic testing for mutant transthyretin, senile amyloid cardiomyopathy (ATTRw) was expected. CONCLUSION

[Volume expansion during septic shock].

At its very early phase, septic shock is characterized by severe hypovolemia related to abundant fluid losses, increased venous compliance and maldistribution of extracellular fluid (interstitial edema, splanchnic pooling). It results in a drop in venous return and cardiac output. Volume expansion, with crystalloids or colloids produces a hyperdynamic state with high cardiac output and low peripheral resistances. The quantity of liquid administered rather than its composition is the main determinant. Clinical examination and hemodynamic monitoring (Swan-Ganz catheter) appear to provide the mos

Cardiotoxic Effects of Lachesis acrochorda Snake Venom in Anesthetized Wistar Rats.

Ophidism is a public health problem in tropical countries, occurring predominantly in rural areas. In Colombia, among the species responsible for snakebite envenomation, inflicting high mortality, is the Chocoan bushmaster, Lachesis acrochorda, better known locally by the names "verrugosa (warty)" and "pudridora (rot-causing)". In this research, the cardiotoxic effect of the venom of L. acrochorda in male Wistar rats weighing 230 ± 20 g was evaluated. A statistical design of randomized blocks was implemented with three treated groups, injected with lyophilized venom (doses of 3.22 μg/g, 6.43 μ

Manual Lymph Drainage in a Patient with Congestive Heart Failure: A Case Study.

A variety of conditions result in lower extremity edema, such as deep vein thrombosis, cellulitis, venous stasis insufficiency, and congestive heart failure (CHF). A case study is presented to illustrate the dynamics of the lymphatic system, the pathology of CHF, the importance of obtaining a pretreatment differential diagnosis, and how to implement a safe treatment plan. The patient was a 69-year-old overweight woman with bilateral lower extremity lymphedema of almost equal volume (~9,100 mL) of >2 months' duration. She had 11 draining wounds and a reported history of chronic obstructive pulm

Aldosterone: role in edematous disorders, hypertension, chronic renal failure, and metabolic syndrome.

The role of aldosterone has expanded from the hormone's genomic effects that involve renal sodium transport to nongenomic effects that are independent of the effect of aldosterone on sodium transport. The nongenomic effects of aldosterone to increase fibrosis, collagen deposition, inflammation, and remodeling of the heart and blood vessels, however, are markedly increased in the presence of high sodium intake. The genomic effect of aldosterone increases renal sodium transport, but the administration of large doses of aldosterone to normal individuals does not cause edema, relating to the pheno

Clinical practice. Protein-losing enteropathy in children.

Protein-losing enteropathy (PLE) is a rare complication of a variety of intestinal disorders characterized by an excessive loss of proteins into the gastrointestinal tract due to impaired integrity of the mucosa. The clinical presentation of patients with PLE is highly variable, depending upon the underlying cause, but mainly consists of edema due to hypoproteinemia. While considering PLE, other causes of hypoproteinemia such as malnutrition, impaired synthesis, or protein loss through other organs like the kidney, liver, or skin, have to be excluded. The disorders causing PLE can be divided i

Dilation of intra-atrial baffle fenestrations: results in vivo and in vitro.

Spontaneous closure of intra-atrial baffle fenestrations occurred early postoperatively following a Fontan procedure in three patients (ages 0.8-2 yr) and were reopened using transvenous balloon dilation. Following this clinical experience with this complication we studied the effects of high pressure balloons of different sizes on fenestrations created in polytetrafluoroethylene (PTFE) patches. The intra-atrial baffle fenestration was found closed within hours after surgery in two patients and 32 days postoperatively in the other. Increasing edema, chest tube drainage, and renal dysfunction e

Impaired cardiovascular function caused by different stressors elicits a common pathological and transcriptional response in zebrafish embryos.

Zebrafish embryos have been widely used to study the genes and processes needed for normal vertebrate heart development. We recently observed that exposure to 2,3,7,8-tetra-chlorodibenzo-p-dioxin (TCDD) or retinoic acid (RA) produces very similar signs of heart failure in developing zebrafish via divergent molecular pathways. The fact that diverse stressors and mutations cause severe pericardial edema and circulatory collapse in developing zebrafish has been largely unexplored. We hypothesized that unrelated chemicals can trigger a common pathological response leading to the same end-stage hea

Definitive Airway Management in Sepsis-Related Airway Edema Following a Failed Intubation: An Anesthesia-Guided Approach to High-Risk Airway Management.

Severe infection and sepsis can significantly alter airway anatomy through inflammation, edema, and tissue friability, complicating definitive airway management in critically ill patients. These challenges are further magnified in individuals with prior failed intubation or airway trauma, where the margin for error is narrow, and loss of a functioning airway can be catastrophic. We report the case of a critically ill patient with influenza A-associated acute hypoxic respiratory failure and septic shock. Our patient required definitive airway management in the setting of severe airway edema and

In Experimental Dilated Cardiomyopathy Heart Failure and Survival Are Adversely Affected by a Lack of Sexual Interactions.

Nearly one in three people in the U.S. will develop heart failure (HF), characterized by fluid retention (edema) in the lungs and elsewhere. This leads to difficult breathing, deterioration of physical capacity, restriction of normal activities and death. There is little data about the safety and effects of sexual interactions in patients with HF. We tested whether a lack of sexual interactions affected pathophysiological outcomes in a pre-clinical mouse model of dilated cardiomyopathy that recapitulates the progressive stages of human HF. Male mice were randomly given access to, or deprived f

Low-dose methotrexate-induced vulvar edema: A case report.

RATIONALE: Methotrexate (MTX) is an antimetabolite of folic acid, which is used for management of ectopic pregnancy. MTX-related toxicity may include cutaneous mucosal damage, bone marrow suppression, gastrointestinal disorders (gastritis, diarrhea, hematitis), liver and kidney function damage, pulmonary toxicity, cardiac toxicity, and nerve toxicity. However, it is not usual for vulvar edema induced by low-dose methotrexate. PATIENT CONCERNS: In this case report, we described a patient with severe vulvar edema and oral cavity ulceration and scalp ulceration induced by low-dose MTX treatment
