Magnetic resonance imaging of the elbow: a structured approach.

CONTEXT: The elbow is a complex joint and commonly injured in athletes. Evaluation of the elbow by magnetic resonance imaging (MRI) is an important adjunct to the physical examination. To facilitate accurate diagnosis, a concise structured approach to evaluation of the elbow by MRI is presented. EVIDENCE ACQUISITION: A PubMed search was performed using the terms elbow and MR imaging. No limits were set on the range of years searched. Articles were reviewed for relevance with an emphasis of the MRI appearance of normal anatomy and common pathology of the elbow. RESULTS: The spectrum of common

An olecranon chondral flap and osteochondral coronoid fracture in a spontaneously reduced elbow dislocation in a child.

Elbow injuries in children are very common and radiographs are often difficult to interpret because of the radiolucency of the cartilaginous anlage and the progressive appearance of multiple secondary ossification centres. Elbow dislocations are rare injuries in children. Coronoid fractures can occur during dislocation or relocation of the elbow and can be the only hallmark of a severe injury. The understanding of the mechanics of these injuries has undergone considerable evolution over the past decade. Intra-articular chondral flap fractures are a traumatic elevation of the hyaline cartilage …

Clinical effects of shadow throwing instruction on improving the "elbow dropped" position: an examination of reproducibility during net and partner throwing drills.

[Purpose] This study investigated whether shadow throwing-based form instruction improves the "elbow dropped" position during net and partner throwing drills among youth baseball players who have recovered from throwing related shoulder or elbow injuries. [Participants and Methods] Forty-six right-handed male players (ages: 11-15 years) who exhibited the "elbow dropped" position during shadow throwing were analyzed after returning to throwing without pain following recovery from throwing-related shoulder or elbow injuries. The "elbow dropped" position during shadow throwing, net throwing, and …

Double-locking precontoured plating system for malunited fractures of the distal end of humerus.

BACKGROUND: Distal humeral malunions are uncommon injuries, often associated with limited elbow motion, pain, instability, weakness, and sometimes ulnar neuritis. The complex anatomy of the elbow joint makes this condition one of the most complex elbow injuries to treat. MATERIALS AND METHODS: Four patients were treated by the same surgeon between 2011 and 2013 using a double-locking precontoured plating system for malunited intra-articular or extra-articular fractures of the distal end of the humerus. RESULTS: At a mean 3 years of follow-up, a significant improvement in the elbow motion and…

Terrible triad of the elbow--role of the coronoid process: a case report.

Posterior dislocation of the elbow joint with fracture of both the radial head and coronoid process, the so-called 'terrible triad', is a complex and difficult-to-treat injury. We report a case of a 34-year-old man with terrible triad of the elbow and associated injuries secondary to a fall. The patient was treated with radial head osteosynthesis, resection of the coronoid process fragments, and stabilisation of the ulnohumeral joint by Kirschner wire transfixation. The sequence of treatment steps is provided. The priority for treating multiple injuries should be established. The integrity of

Upper extremity injuries in adolescent tennis players.

The majority of adolescent tennis players will have their playing temporarily interrupted by an injury, with lower extremity problems predominating. Although upper extremity injuries occur less frequently, they tend to be more troublesome. Injuries discussed in this article include "King Kong arm," slipped capital humeral epiphysis, "Osgood-Schlatter disease" of the shoulder, tennis elbow, stress synovitis and elbow flexion contracture, and friction burns and collagen stress fractures of the rotator cuff.

Diagnostic accuracy of ultrasonography for diagnosis of elbow fractures in children.

BACKGROUND: Elbow injuries and fractures are a common pathology in the pediatric emergency unit. X-ray and CT scan of the elbow are the standard diagnostic procedures, which increase exposure to radiation in children. Previous studies have shown that fractures can also be visualized by ultrasound (US); thus, this study aimed to evaluate the diagnostic accuracy of elbow US compared with radiography for the diagnosis of elbow fracture in children. MATERIALS AND METHODS: This was a prospective observational study of patients aged 2-14 years that presented to emergency departments with a suspecte…

A Rare Case Report of Split Hand and Foot Malformation.

INTRODUCTION: Elbow dislocations are commonly encountered by orthopedic surgeons all over. Management is straightforward and the result is generally satisfactory. However, in a small subset, associated injuries like convergent or divergent dislocations at superior radioulnar joints are present. These associated injuries are often missed initially leading to their delayed treatment and suboptimal elbow function. We report one such rare case of Convergent dislocation at the elbow. Although reported from other parts of the world till date, there has been none reported from India. It is extremely

Thrombosis of the Brachial Artery After Closed Elbow Dislocation.

Posterior dislocation of the elbow joint is the second commonest large joint dislocation that can be experienced due to various traumatic incidents. Although it may be associated with fractures and vascular lesions, in this case report, we describe a patient who encountered a posterolateral elbow dislocation following a fall on their arm with an extended elbow. This dislocation was followed by delayed thrombosis of the brachial artery, necessitating a revascularization surgery. For optimal patient care, physicians should remain vigilant, being cautious about potential vascular injuries both be

Ulnar neuropathy and medial elbow pain in women's fastpitch softball pitchers: a report of 6 cases.

BACKGROUND: Elite-level women's fastpitch softball players place substantial biomechanical strains on the elbow that can result in medial elbow pain and ulnar neuropathic symptoms. There is scant literature reporting the expected outcomes of the treatment of these injuries. This study examined the results of treatment in a series of these patients. METHODS: We identified 6 female softball pitchers (4 high school and 2 collegiate) with medial elbow pain and ulnar neuropathic symptoms. Trials of conservative care failed in all 6, and they underwent surgical treatment with subcutaneous ulnar ner

Differences in outcome and complications between 2 distinct complex traumatic elbow instability patterns. A retrospective comparison of terrible triad and olecranon fracture dislocations.

BACKGROUND: Terrible triad (TT) of the elbow and olecranon fracture dislocation (OFD) injuries are a combination of 2 different types of unstable fracture dislocation injuries classified under traumatic elbow instability patterns. Despite the differences in injury mechanisms, similarities in the structures disrupted and treatment principles make it meaningful to compare outcomes and complications after surgical treatment of these injuries. METHODS: We reviewed data of 91 patients who underwent surgical treatment for TT injuries and olecranon fracture dislocations and were assessed for inclusi

Current concepts in the management of complex elbow trauma.

Complex instability of the elbow, also known as fracture dislocation of the elbow, is one of the most challenging injuries of the musculoskeletal system. A clear understanding of the biomechanics of the joint with special focus on the relative contributions of the articular components and the ligamentous constraints is critical to the proper management of these injuries. The accurate recognition and implications of associated injuries offers a rationale for approaching these difficult problems by appropriately addressing the elements of the trauma. Further, doing so in a timely fashion enhance

Common and Less Well-known Upper-limb Injuries in Elite Tennis Players.

A recent increase in epidemiology studies on injuries in elite tennis players has helped to shed light on the types of injuries these athletes sustain. This article reviews the common upper-limb injuries in elite players and includes less well known, but important, injuries. A search was conducted to identify current relevant studies involving elite tennis players. Injury frequency rates, injuries by regions, and types of injuries together with a list of commonly reported injuries in the upper limb were established. This list was then reviewed and refined by a sports medicine physician who car

Magnetic Resonance Imaging Findings After Elbow Dislocation: A Descriptive Study.

BACKGROUND: The elbow is one of the most commonly dislocated joints, and dislocation is usually accompanied with an assortment of soft tissue injuries. The purpose of this study was to retrospectively analyze and describe the patterns of ligamentous, tendinous, and muscular injuries in patients with an acute elbow dislocation and subsequent magnetic resonance image (MRI) evaluation. METHODS: From 2008 to 2020, 235 patients clinically diagnosed with an elbow dislocation were seen in the department, of which only 19 underwent an MRI of the affected elbow. Twelve patients met inclusion criteria,

How much valgus instability can be expected with ulnar collateral ligament (UCL) injuries? A review of 273 baseball players with UCL injuries.

BACKGROUND: The dynamic stress radiograph of the elbow was designed to help the clinician better define valgus laxity and instability in the throwing athlete. However, no large study has quantified the amount of laxity in athletes with ulnar collateral ligament (UCL) injuries. We hypothesized that valgus stress radiographs in patients with UCL injuries will demonstrate a significantly greater amount of valgus stress opening of the dominant elbows compared with the nondominant elbow. METHODS: Bilateral elbow static and stress radiographs that were taken as part of our standard preoperative wor

Open elbow dislocation associated with distal ischemia in children about one case and review of the literature.

Elbow's dislocations of the child are rare injuries. Its associated injuries such as the opening and the distal ischemia are an extreme therapeutic emergency. One case of open elbow dislocation with distal ischemia in a 10-year-old child is reported. In the clinical examination, there is a deformity of the elbow, a wound showing the cartilaginous distal humerus with the brachial artery under tension on the trochlea that creates a beginning distal ischemia and hypoesthesia in the territory of the median nerve. The surgical health care included a careful debridement, a reduction and scanning of

Ulnar Collateral Ligament Injuries in Throwing Athletes: Diagnosis, Management, and Treatment-An Evidence-Based Review.

There is substantial interest in the medial ulnar collateral ligament of the elbow. This is due to the growth of throwing-associated injuries in the past 30 years. This is, in part, due to the length of time to rehabilitate this type of injury in this athlete population. It is vital to understand how to appropriately diagnose and manage medial ulnar collateral ligament injuries in throwing athletes based on the injury severity. This manuscript will review the anatomy of the elbow, risk factors for medial ulnar collateral ligament injuries, management approaches, and future directions for preve

The Skeletally Immature and Newly Mature Throwing Athlete.

Injuries to the shoulder and elbow in the pediatric and adolescent throwing athlete are common. Both knowledge of throwing mechanics and understanding of normal bone development in the immature skeleton are key to the diagnosis, treatment, and potential prevention of these common injuries. Pathologic changes from chronic repetitive trauma to the developing shoulder and elbow manifest as distinctly different injuries that can be predicted by the skeletal maturation of the patient. Sites of vulnerability and resulting patterns of injury change as the child evolves from the skeletally immature li

The Unstable Elbow: Current Concepts in Diagnosis and Treatment.

Elbow instability is common and may occur after a variety of injuries, including falls or direct blows. Instability can be classified as either acute or chronic. Acute instability is classified as simple (without fracture) or complex (with associated fracture). Chronic instability is classified as a chronically dislocated or recurrently unstable elbow. Recurrent instability commonly presents as isolated medial or lateral collateral ligament insufficiency. A chronically dislocated elbow is often more complex, involving both osseous and ligamentous injuries. The treatment of simple dislocations
