Effectiveness of the saline load test in diagnosis of traumatic elbow arthrotomies.

OBJECTIVES: The purpose of this study was to investigate the amount of fluid required and the sensitivity of the saline load test to identify an intra-articular arthrotomy of the elbow. METHODS: A cadaveric study was conducted using 36 thawed, fresh-frozen forequarter amputations. An elbow arthrotomy was made in the posterocentral arthroscopic portal site with a 4.5-mm trochar. To confirm intra-articular location of the arthrotomy, the trochar was trapped in the ulnohumeral joint. The elbow joint was then loaded with saline mixed with methylene blue. During the injection, the known arthrotomy…

Clinical assessment and point of care ultrasonography: How to diagnose haemophilic synovitis.

BACKGROUND: The ability of clinical tools to identify early joint changes is limited. Synovitis is a fundamental finding in understanding haemophilia activity and the response to its therapies; thus, there is a need for sensitive methods to better diagnose subclinical synovitis early. PURPOSE: Our aim was to compare the frequency with which clinical assessment and ultrasound detected synovial hypertrophy in the most frequently affected joints in patients with haemophilia (elbows, knees and ankles). METHODS: We analysed patients with haemophilia older than 16 years who came to the haemophilia

Joint Function and Arthropathy Severity in Patients with Hemophilia.

BACKGROUND: The Arnold-Hilgartner classification is one of the most popular evaluation systems for the progression hemophilic arthropathy. A previous study reported an association between arthropathy severity and arc range of motion (ROM). However, associations between arthropathy severity and angular ROM and muscle strength remain unclear. AIM: The purpose of this study was to clarify the association between joint function and arthropathy severity in hemophilia. METHODS: We studied the knee, ankle, and elbow joints of 31 patients with hemophilia (PWH). The condition of the affected joints w…

Elbow Flexion Assist Orthosis for Arthrogryposis.

INTRODUCTION: People with arthrogryposis multiplex congenita (AMC) often have muscle weakness in the biceps that makes elbow flexion difficult. An elbow-flexion assist orthosis was designed using the force of springs, combined with a sliding joint, to apply appropriate elbow torque to aid a user in lifting her hand to her mouth. The sliding joint allows an increasing elbow torque despite a decreasing spring force. METHODS: The device was prototyped for a user with AMC. An occupational therapist measured the user's flexion with and without the device. Benchtop torque measurements were also det

[Fractures of internal epicondyle of the humerus with intra-articular incarceration of bone fragments].

The article analyses 41 fractures of the medral epicondyle of the humerus in children with incarceration of the fragment in the cavity of the elbow joint. The clinical and X-ray signs of this complication are described in detail. Open reduction is the optimal method of treatment. During the operation the ulnar nerve is examined, the cavity of the elbow joint is cleaned, and stable osteosynthesis of the medial epicondyle is performed. Restorative treatment can be started in the early postoperative period if a screw-awl has been applied. The function of the elbow joint is restored completely in …

Acute and overuse elbow trauma: radio-orthopaedics overview.

The correct management of acute, subacute and overuse-related elbow pathologies represents a challenging diagnostic and therapeutic problem. While major trauma frequently requires a rapid surgical intervention, subluxation and minor trauma allow taking more time for diagnostics and planning the correct elective treatment after careful clinical and radiological investigation. In these conditions, communication between orthopaedic surgeon and radiologist allow to create a detailed radiology report, tailored to the patient's and surgeon's needs and optimal to plan proper management. Imaging techn

Posteromedial rotatory instability of the elbow: What the radiologist needs to know.

Varus posteromedial rotatory instability of the elbow joint is a relatively new subject described for the first time in 2003. It occurs secondary to axial loading of the elbow with varus force and internal rotation of the forearm. There is usually a specific pattern of osseous and soft tissue injuries that can be recognized on imaging. This includes an anteromedial coronoid fracture and avulsion of the lateral collateral ligament complex from its humeral attachment. Ulnar collateral ligament complex injury is also reported, particularly its posterior bundle which plays an important role in pos…

[RADIAL HEAD FRACTURES - DIAGNOSIS, CLASSIFICATION AND TREATMENT].

Radial head fractures are a common injury of the elbow joint usually occurring after a fall onto an outstretched hand ("FOOSH"). These fractures comprise 30% of all the fractures of the elbow joint in adults and 11% of proximal radius fractures in children. Clinically, these patients present with elbow tenderness and swelling with bruising and pain in pronation and supination. These fractures are usually accompanied by associated injuries such as scaphoid fractures, distal radio-ulnar joint injuries, coronoid process fractures and ligamentous injuries. Diagnosis and classification are based on…

Medial Elbow Pain Syndrome: Current Treatment Strategies.

Medial elbow pain is a common presentation that can be a challenge to appropriately treat for the orthopedic surgeon. Causes include medial epicondylitis, ulnar neuritis, ulnar collateral ligament injury, flexor pronator strain, or snapping medial triceps. A good outcome is typically achieved with adequate treatment of tendon degeneration at the common flexor tendon origin. Mainstay treatment is nonoperative modalities such as stretching, rest, activity modification, therapy, and injections. If nonoperative management fails, intermediate interventions such as extracorporeal shockwave therapy,

Arthroscopic Management of Posterior Elbow Impingement.

BACKGROUND: Posterior elbow impingement is often seen in athletes who engage in overhead sports that involve repetitive elbow extension; for these patients with persistent symptoms, arthroscopic debridement is often recommended. While traditionally performed in the supine or prone position, arthroscopic elbow debridement performed in the lateral decubitus position has distinct advantages that make it our preferred method. INDICATIONS: Arthroscopic debridement in the context of posterior elbow impingement is utilized when conservative management with rest, physical therapy, and nonsteroidal an

[Elbow dislocation fractures].

The treatment of complex elbow dislocation fractures is a challenge to both the treating surgeon as well as to the patient due to the complex bony and soft tissue anatomy of the joint. In order to establish an expedient treatment algorithm, all osseous and ligamentous injuries need to be thoroughly assessed. Furthermore, a detailed knowledge of the joint-stabilizing structures, practicable surgical approaches as well as the possible techniques for fracture fixation and/or arthroplasty are essential to facilitate early rehabilitation of the elbow and avoid injury-related complications. Ipsilateral total shoulder and elbow prosthetic replacement in a patient with severe haemophilia B.

Since factor replacement concentrates became widely available in the 1970s, total prosthetic replacement of joints in haemophilic patients has become increasingly common, with success particularly in the knee and hip. However, there has been no published series of elbow or shoulder replacements large enough to draw conclusions about the advisability of these procedures in this group of patients. We report a case of prosthetic replacement of the shoulder in a 58-year-old man with haemophilia B, and replacement of the elbow joint of the same limb 21 months later. The results are initially very e…
