Transcutaneous electrical stimulation of thigh muscles in the treatment of detrusor overactivity.

OBJECTIVE: To investigate the clinical effects on detrusor overactivity of a new method of transcutaneous reciprocal electrical stimulation of the thigh muscles. PATIENTS AND METHODS: Nineteen patients with detrusor overactivity, comprising 14 with detrusor hyperreflexia (DH) and five with idiopathic detrusor instability (IDI), were studied. Electrical stimulation was applied alternately to the quadriceps and hamstring muscles of one or both legs through surface electrodes for 20 min. The treatment was given once a day for 14 days and then the patients were evaluated urodynamically. RESULTS:

Long-lasting reductions of spasticity induced by skin electrical stimulation.

We studied the effects of electrical stimulation of the skin on upper extremity spasticity in nine hemiparetic stroke subjects. The effects were quantified by comparing reflex torque responses elicited during ramp and hold angular perturbations of the elbow recorded before and after low-intensity skin stimulation. Electrical stimulation was applied to skin over the biceps muscle for a period of ten minutes at a 20 Hz frequency, pulse duration 0.1 ms, with an intensity level below motor threshold but above sensory threshold. In seven of the nine subjects, stimulation of skin over spastic muscle

Visual perception elicited by electrical stimulation of retina in blind humans.

OBJECTIVE: To evaluate the feasibility of bypassing damaged photoreceptors and electrically stimulating the remaining viable retinal layers to provide limited visual input to patients who are blind because of severe photoreceptor degeneration. METHODS: In the operating room with the patient under local anesthesia, focal electrical stimulation of the retinal surface with brief biphasic pulses was performed using small probes inserted through the sclera. The procedure was performed in five subjects who had little or no light perception. Three subjects had retinitis pigmentosa, one had age-relat

The sniff-like aspiration reflex evoked by electrical stimulation of the nasopharynx.

Forced inspiratory efforts resembling the sniff-like aspiration reflex (AR) were evoked by single-shocks or trains of electrical impulses to the dorsal pharyngeal and nasal mucosal surfaces in 12 anaesthetized spontaneously breathing cats. The strongest ARs determined by diaphragmatic (DIA) EMG and peak-inspiratory flows were elicited from the dorsolateral nasopharyngeal wall close to the torus tubalis. Highly responsive sites were also detected in the posterolateral nasal cavity and in the rostrolateral oropharynx. Repetitive stimulation at threshold strength evoked ARs exclusively in the lat

Reflex contractions of the ischiocavernosus muscles following electrical and pressure stimulations.

In a previous study, we have demonstrated that voluntary muscular contractions of the ischiocavernosus muscles (IC) correlate with changes in intracavernous pressure and, therefore, with penile rigidity. The purpose of our current research project was to verify whether reflex contractions of the IC muscles exist and under what conditions. Our results confirm that reflex contractions occur following electrical or pressure stimulations. Following electrical stimulation, the mean latency of the reflex contractions was 67.5 ms for the IC muscles and 34.9 ms for the bulbocavernosus muscles (BC). Fo

The neurophysiological basis of bladder inhibition in response to intravaginal electrical stimulation.

Intravaginal electrical stimulation (IVS) induces a profound bladder inhibition and is successful in the treatment of incontinence due to detrusor instability. In this experimental study in cats, direct recordings of the efferent activity in thin hypogastric and pelvic nerve filaments to the bladder were used to analyze the neuronal mechanisms underlying this bladder inhibition. A longlasting reflex discharge, with a latency of 35 to 50 msec., was evoked in the hypogastric nerve by IVS. The reflex discharge was unaffected by imposed changes in intravesical pressure or by micturition contractio

Development of a universal control unit for functional electrical stimulation (FES).

In collaboration with the College of Engineering the author has developed a laboratory, or clinic, based, battery operated "universal" control system, designed to improve disabled gait in upper motor neuron disabilities, especially stroke, hemiplegia, and cerebral palsy, by applying several channels of FES (Functional Electrical Stimulation) to the lower limb muscles while the patient is walking. The timing of the FES pulses, which can be applied to as many as six of the patient's muscles, is determined by potentiometer controlled one-shot timers, which are triggered by any of three switches i

Electrical stimulation of tooth pulp in humans. II. Qualities of sensations.

Individual premolar teeth of 22 normal volunteers were stimulated via intradentinal electrodes using brief electrical pulses. Questionnaires were used to determine the qualities of sensations produced by this stimulation. Operationally defined 'innocuous' sensations predominated between 1 and 10 dB SL, and were absent above 30 dB SL. Similarly defined 'painful' sensations were observed throughout the range of suprathreshold intensities but were most common above 20 dB SL. These findings demonstrate that non-painful sensations can arise from electrical tooth pulp excitation in circumstances in

Identification of the motor laryngeal nerves - a new electrical stimulation technique.

Head and neck surgeons are familiar with the technique of identifying motor nerves in the head and neck region by using electrical stimulation especially in the identification of the facial and the spinal accessory nerves. The identification of the motor laryngeal nerves by electrical stimulation intra-operatively has been described; but, the difficulty of visualization of intrinsic laryngeal muscle movement has prevented the wide spread use of this technique. This paper will introduce a simple, safe and reliable method to allow the surgeon to recognize true vocal cord movement while stimulati

Relief of pain by subcutaneous electrical stimulation after ocular surgery.

Electrical stimulation in the treatment of postoperative ophthalmologic pain was applied in 25 patients. In 21, stimulation was applied by means of subcutaneously implanted wires and in four patients the therapy was administered by externally placed electrodes. The subcutaneous electrodes were introduced near the supraorbital and infraorbital nerves at the end of the surgical procedures, and stimulation was immediately begun. Duration of treatment varied between one and three days. The nerve stimulator is a portable, battery-operated apparatus. The implanted electrodes avoid skin resistance an

Electrical stimulation for voiding dysfunction after spinal cord injury.

Incontinence and frequency of voiding were present after spinal cord injuries in 18 patients. A hyperreflexic bladder and/or pelvic floor weakness was found in these patients. Functional electrical stimulation resulted in relief or improvement of symptoms in 9 of the 11 patients in whom this procedure was used. An increase in anal sphincter pressure with functional electrical stimulation was a more reliable criterion than an increase in maximum urethral pressure in the selection of patients for the procedure.

Delayed drinking in response to electrical and thermal stimulation of the medial forebrain.

Water intake in response to electrical and thermal stimulation of the medial forebrain was studied in the goat. When the frontal wall of the third cerebral ventricle was included in a field of bipolar electrical stimulation a dipsogenic response was obtained after discontinuation of the stimulation. Release of antidiuretic hormone (ADH) was apparently also elicited. The water intake was roughtly proportional to the amount of current which had been applied during the stimulation period. Water consumption in response to stimulation attenuated the dipsogenic effect of subsequent stimulation, as d

Mechanisms for closure of the human urethra during intravaginal electrical stimulation.

The effect of intravaginal electrical stimulation (IVS) on the urethral pressure profile (UPP) before and during succinylcholine blockade or spinal anaesthesia was studied in patients with stress incontinence and in patients with clinically normal urethral function. During succinylcholine blockade, the UPP was lowered to 74% and during spinal anaesthesia to 39% of the original maximal UPP. IVS could not influence the UPP during succinylcholine blockade, while the UPP could be restored during spinal anaesthesia when the stimulation strength was increased 3 to 4 times. The following conclusions

Long-term intravaginal electrical stimulation in urge and stress incontinence.

Using a new device for intravaginal electrical stimulation (IVS), 24 women with pure stress incontinence, mixed urge and stress incontinence or urge incontinence due to detrusor instability, were treated. To minimize mechanical discomfort, the electrode carrier was made of flexible material and inflatable. Furthermore, this design facilitated fixed, constant positioning of the electrodes at individually tested sites to produce maximum response at voltage well tolerated by the patient. The stimulation frequency was adjusted to suit the cause of incontinence: 20 to 50 Hz in stress incontinence,

Electrical bone-growth stimulation in an experimental model of delayed union.

An experimental model has been devised for the consistent production of delayed bone healing of the tibia in adult dogs. A double-blind trial, with bias eliminated, was used to evaluate the use of a commercially available direct-current bone-growth stimulator with this model. The stimulator produced a statistically significant acceleration of bone healing at four weeks in the experimental model. Osteogenesis was normal, and no dysplastic, inflammatory, or neoplastic changes were found. This research has shown that electrical stimulation of bone is safe and augments bone formation. The bone-gro
