The pathogenesis of the two forms of hypertensive pulmonary vascular disease.

Autopsy observations suggested that lesions of hypertensive pulmonary vascular disease (HPVD) due to elevated venous pressure differ from those with arterial hypertension only. Clinical and pathologic features were reviewed in patients from the Hopkins autopsy files with proved pulmonary hypertension; 50 had venous HPVD due to left-sided congestive heart failure, 50 had arterial HPVD due to congenital malformations, and 15 had idiopathic pulmonary hypertension (IPH). The two forms of HPVD have consistent distinctive histologic changes. In venous HPVD intimal fibroelastosis (IFE) develops in ve

Perivascular Basal Echogenicity in the Fetal Heart: An Unconventional Marker of Maternal Autoimmune Antibodies.

Fetal congenital heart block is a widely recognized complication of pregnancies with anti Ro/La antibodies. Less common presentations, including dilated cardiomyopathy, myocarditis, valvular insufficiency, and endocardial fibroelastosis, have also been described in these fetuses. We present two cases where recognition of isolated basal echogenicity of the fetal heart, an unconventional manifestation of anti-SSA antibodies during pregnancy, led to a suspicion and ultimately diagnosis of antibody positivity in the mother. These cases highlight the importance of being aware of this rare manifesta…

An expanded phenotype of maternal SSA/SSB antibody-associated fetal cardiac disease.

OBJECTIVES: Conventional manifestations of fetal Sjogren's antibodies (SSA/SSB) associated cardiac disease include atrioventricular block (AVB), transient sinus bradycardia, endocardial fibroelastosis (EFE) and dilated cardiomyopathy. We describe other manifestations of cardiac disease. METHODS: We describe three fetuses with unique myocardial and conduction system disease. RESULTS: One had isolated EFE with subsequent mitral and tricuspid valve chordal avulsion, the second had sinoatrial and infrahissian conduction system disease, and in both, neonatal progression to life threatening diseas…

Congenital aneurysm of the muscular interventricular septum.

Congenital aneurysms of the muscular interventricular septum are rare. We report the diagnosis of such an aneurysm by fetal echocardiography at 19 weeks' gestation and the subsequent follow-up through early neonatal life, with the ultimate progression to a dilated cardiomyopathy requiring cardiac transplantation. This case highlights the early diagnosis of this rare finding and the unusual clinical course to include the confounding features of endocardial fibroelastosis, left ventricular noncompaction, and a lymphocytic infiltrate demonstrated on pathologic examination. Most infants with conge…

[Sudden death in a 4-month-old infant associated with anomalous origin of the left coronary artery].

We describe the autopsy findings of a 4 months-old boy, who died suddenly after an episode of high airway infection of 3 days time. Autopsy examination showed cardiomegaly (80 g) with widening of the left cavities and thick and white endocardial surface, besides a severe thinning of the cardiac apex at the level of the left ventricle. Left coronary origin was in the pulmonary artery trunk. Histologically, the myocardium shows endomyocardial fibroelastosis and also multiple and extensive areas of old and recent infarcts in the left ventricle. The collateral coronary arteries, were increased in

[Myocardial infarct in children. The anatomicoclinical aspects].

Nine cases of myocardial infarction in the newborns (8 cases aged between 7 days and 6 months) and infants (1 case aged 12 years and 6 months) were followed up for 15 years. The etiology was different: mediocalcosis of the coronaries, congenital abnormalities of the heart marked hypertrophy of left ventricular myocardium (endocardial fibroelastosis or nonobstructive hypertrophic cardiomyopathy) severe hypoxia (severe acute bronchopneumopathies), and AAR, cardioarticular form, with multiple attacks in the infant. The ECG aspects were not specific in most children and clinically the prevailing f…

A cardiomyopathy ("sudden death syndrome") of adult hens.

The lesions associated with a syndrome of sudden death in adult hens is described. The syndrome occurred primarily in broiler-breeder hens and in three flocks of commercial egg-laying strains. All flocks were housed on deep litter. Deaths occurred from 24 to 26 weeks of age until approximately 30 weeks of age. Congestion and oedema of viscera occurred consistently and concentric or eccentric myocardial hypertrophy associated with left atrial endocardial fibroelastosis occurred in many birds. The cause of the syndrome has not been determined. The findings of cardiac failure, myocardial hypertro…

[Mucolipidosis type II (I-cell disease) with unusually severe heart involvement].

Mucolipidosis II (I-cell disease), an autosomal recessive inborn error of mucolipid metabolism with defective transport of lysosomal enzymes is described in a young infant. Besides the typical findings as a result of the "lysosomopathy", this case presented an unusual and distinct cardiac involvement: a cardiomyopathy with dilatation of the left ventricle and signs of an endocardfibroelastosis.
