[Hurler syndrome. Early diagnosis and successful enzyme replacement therapy: a new therapeutic approach. Case report].

Mucopolysaccharidosis type I (MPS I) is a lysosomal storage disorder due to alpha-L-iduronidase deficiency. Its severe prognosis has been significantly improved by enzyme replacement therapy using recombinant human alpha-L-iduronidase (laronidase). We report the case of a boy who was diagnosed at 19 months of age with Hurler's disease, the most severe form of MPS I, and received thereafter a treatment by laronidase, resulting in clinical and biological improvement. The aim of this case report is to draw physicians' attention on the presenting signs of Hurler's disease, in order to enable an ea

Ultrastructural features of gaucher disease treated with enzyme replacement therapy presenting as mesenteric mass lesions.

The classical ultrastructural features of Gaucher disease include large numbers of intracytoplasmic, membrane-bound lysosomal inclusions containing characteristic tubular structures on an electron-lucent background, representing the periodic acid schiff (PAS)-positive Gaucher cells identifiable on light microscopy. Following enzyme replacement therapy (ERT), many of the manifestations of the condition are ameliorated, but persistent mesenteric lymphadenopathy has been reported, the ultrastructural features of which previously have not been described. Two children, aged 4 and 8 years old, respe

A phase I/II clinical trial of enzyme replacement therapy in mucopolysaccharidosis II (Hunter syndrome).

OBJECTIVE: To evaluate the safety and explore the efficacy of idursulfase (recombinant human iduronate-2-sulfatase) treatment for mucopolysaccharidosis II (MPS II). STUDY DESIGN: Twelve patients were enrolled into a randomized, double-blind, placebo-controlled trial for 24 weeks followed by an open-label extension study. Three groups of 4 patients were enrolled sequentially, with 3 patients in each group receiving idursulfase and 1 patient receiving placebo. The first group received idursulfase at 0.15 mg/kg infused every other week with the 2nd and 3rd groups receiving 0.5 and 1.5 mg/kg, res

Outcome after three years of laronidase enzyme replacement therapy in a patient with Hurler syndrome.

Enzyme replacement therapy (ERT) with laronidase, recombinant alpha-L-iduronidase, for mucopolysaccharidosis type I (MPS I) has been clinically available since April 2003. Pre-approval studies were performed on patients with the more attenuated forms of MPS I, Hurler-Scheie and Scheie syndromes. The clinical efficacy of laronidase on the severe form of MPS I, Hurler syndrome, is not well known. We present a patient with Hurler syndrome who has been treated with laronidase for 3 years. Clinically, the patient demonstrated improvement in urinary glycosaminoglycan (GAG) levels and hepatomegaly, b

[Manifestation of Fabry disease in a heterozygous female patient. New perspectives using enzyme replacement therapy].

HISTORY AND ADMISSION FINDINGS: A 32-year-old woman with bilateral corneal opacities was at 9 years of age diagnosed to have reduced activity of the enzyme alpha-galactosidase A. She was admitted to our hospital because of skin lesions that had developed over the last 8 years. INVESTIGATIONS: The clinical features included angiokeratomas on the buttocks, hips, and periumbilical region, whorl-like corneal opacity (cornea verticillata), and mitral valve prolapse. Activity of alpha-galactosidase A was reduced to about a quarter of the normal value. Histological examination revealed lipid deposit

Overcoming the blood-brain barrier with high-dose enzyme replacement therapy in murine mucopolysaccharidosis VII.

Enzyme replacement therapy (ERT) effectively reverses storage in several lysosomal storage diseases. However, improvement in brain is limited by the blood-brain barrier except in the newborn period. In this study, we asked whether this barrier could be overcome by higher doses of enzyme than are used in conventional trials. We measured the distribution of recombinant human beta-glucuronidase (hGUS) and reduction in storage by weekly doses of 0.3-40 mg/kg administered i.v. over 1-13 weeks to mucopolysaccharidosis type VII mice immunotolerant to recombinant hGUS. Mice given up to 5 mg/kg enzyme

The first 5 years of clinical experience with laronidase enzyme replacement therapy for mucopolysaccharidosis I.

Mucopolysaccharidosis I (MPS I, McKusick 25280) is caused by the deficiency or absence of the lysosomal enzyme, alpha-L-iduronidase (EC 3.2.1.76). This inherited disease causes progressive cellular, tissue and organ damage across the entire phenotypic spectrum. Disabling, multi-organ disease is the rule, and generally results in death between the first and fourth decades of life. Recently, laronidase (Aldurazyme) [Genzyme], a specific recombinant human alpha-L-iduronidase) became commercially available as long-term enzyme replacement therapy. Results from the Phase I/II and III extended clinic

Morphological changes in muscle tissue of patients with infantile Pompe's disease receiving enzyme replacement therapy.

Pompe's disease (glycogen storage disease type II) is an autosomal recessive myopathy caused by lysosomal alpha-glucosidase deficiency. Enzyme replacement therapy (ERT) is currently under development for this disease. We evaluated the morphological changes in muscle tissue of four children with infantile Pompe's disease who received recombinant human alpha-glucosidase from rabbit milk for 72 weeks. The patients were 2.5-8 months of age at entry. Prior to treatment, all patients showed lysosomal glycogen storage in skeletal and smooth muscle cells, vascular endothelium, Schwann cells, and perin

The practical aspects of combination therapy with angiotensin receptor blockers and angiotensin-converting enzyme inhibitors.

Chronic lung disease (CLD) or bronchopulmonary dysplasia is a recognized sequel of preterm birth. With improving survival of infants at lower gestational ages, the incidence is on the rise. Pathological features of CLD include alveolar maldevelopment, with or without areas of pulmonary fibrosis. Assisted ventilation, infection/inflammation, oxygen administration, and fluid overload are the major risk factors in the evolution of CLD.Interventions, including the treatment of maternal infection, administration of prenatal glucocorticoids, and postnatal surfactant replacement therapy, improve the

Myopathy and growth failure in debrancher enzyme deficiency: improvement with high-protein nocturnal enteral therapy.

Seven patients with debrancher enzyme deficiency and childhood or adolescent onset myopathy, four of whom also had growth failure, received long-term treatment consisting of high-protein enteral infusion overnight and high-protein feeds during the day for periods varying from 8 to 42 months. All patients demonstrated improvement in physical activity and endurance. Improvement in muscle strength was documented in five patients, with reversal of myopathic EMG patterns to normal in two patients and reversal of abnormal ECG findings to normal in one patient. All four patients with growth failure s

Dual Enzyme-Loading MnO2 Nanosystem for Cascade Chemodynamic Therapy With Enhanced Penetration in Superficial Tumors.

The therapeutic effectiveness of superficial tumors is currently limited by the operational difficulty of traditional surgery. Localized target delivery of nanomedicine provides a promising solution for superior antitumor effect in superficial tumors, but its delivery efficiency can be greatly hampered by the dense tumor stroma. Compared with external modulation against the tumor extracellular matrix by physical methods, internal regulation of the tumor microenvironment via enzyme enables more direct degradation of the tumor extracellular matrix and robust antitumor efficacy. Herein, we report

Electro-activated nanozyme for in situ tumor vaccination: Synergistic multi-enzyme catalysis and electrodynamic therapy.

Oral squamous cell carcinoma (OSCC) is a highly aggressive malignancy with poor prognosis and limited treatment options. To address the challenges of conventional therapy resistance, antioxidant defence, and immune evasion, we developed a multifunctional nanozyme platform, MIL-100(Fe)@Pt@R837@HA (HMPR), that integrates electrodynamic therapy (EDT), chemodynamic therapy, and immunotherapy. The multi-enzyme activities of HMPR allow for the oxygen generation, hydroxyl radicals production, and glutathione depletion, thereby alleviating hypoxia and triggering both apoptosis and ferroptosis. Platinum nanozymes system with tunable enzyme activity for on-demand supply therapy of infected wounds.

The rapid healing of bacterially infected wounds remains a major clinical challenge. The pathological microenvironment-characterized by bacterial infection, reactive oxygen species (ROS) accumulation, persistent inflammation, and impaired tissue repair-severely impedes this process. In this work, we constructed a photothermally triggered, microenvironmental pH-regulating platinum nanozyme system, ACC@LPDAPt nanoparticles (NPs), leveraging the pH-dependent peroxidase-like and catalase-like activities of platinum nanozymes. This system achieves enzyme-like activity conversion by modulating the w

Iatrogenic Pan-Craniosynostosis Due to Alkaline Phosphatase Enzyme Replacement Therapy Using Asfotase Alfa for Hypophosphatasia.

Hypophosphatasia is a relatively rare congenital bone metabolic disease, occurring in approximately 1 in 100,000 to 150,000 births. It is diagnosed in infancy by low bone calcification, rickets-like symptoms visible on X-ray, and decreased serum alkaline phosphatase (ALP) levels in the blood. Prognosis varies by type, and while treatment has not been long established, ALP enzyme replacement therapy has been developed and has achieved improved outcomes since 2015. Pan-craniosynostosis, as an unfavorable side effect of this symptomatic therapy, is not well known even among experienced craniofaci

A polymeric triple-enzyme nanoparticle by light activatable crosslinking for cascade catalytic therapy of osteosarcoma.

Multi-enzymatic cascade reactions play fundamental roles in biosynthesis, energetic metabolism, cell signaling, proliferation and apoptosis in living cells. Such reactions have been proposed for cancer therapy via overproducing reactive oxygen species, depleting nutrients for tumor starvation, or remodeling immunosuppressive microenvironment, however, co-delivery of multiple enzymes with optimal cascade catalytic activity remains challenging. Here, we propose a polymer nanoparticle loaded with glucose oxidase (GOx), lactate oxidase (LOx), and chloroperoxidase (CPO) for synergistic cancer catal
