Narrowed sinus rhythm electrograms in the zone of uniform slow conduction are helpful to identify VT isthmus location, shape, and orientation.

BACKGROUND: Although the sinus rhythm uniform slow conduction zone is an indicator of the reentrant ventricular tachycardia (VT) sources, it cannot alone be used for delineation. OBJECTIVE: This study aimed to use substrate mapping to determine VT isthmus characteristics. METHODS: In 31 canine postinfarction experiments, an infarct and border zone formed in the anterior left ventricle after left anterior descending coronary artery ligation. In each experiment, 3 to 5 days after infarction, sustained monomorphic reentrant VT was reproducibly inducible via premature electrical stimulation. Epi

The V1-V3 transition index as a novel electrocardiographic criterion for differentiating left from right ventricular outflow tract ventricular arrhythmias.

PURPOSE: The aim of this study was to develop a new electrocardiographic criterion for differentiating the origin of outflow tract ventricular arrhythmias (OT-VAs) with precordial transition in lead V3. METHODS: A total of 147 consecutive patients with OT-VAs displaying precordial transition in lead V3 who underwent successful catheter ablation in the right ventricular outflow tract (RVOT) (n = 118) or left ventricular outflow tract (LVOT) (n = 29) were included in this study. The V1-V3 transition index was defined as the sum of S-wave amplitude in lead V1 and V2 during premature ventricular

Contractile and electrophysiologic characterization of optimized self-organizing engineered heart tissue.

BACKGROUND: Engineered heart tissue (EHT) is being developed for clinical implantation in heart failure or congenital heart disease and therefore requires a comprehensive functional characterization and scale-up of EHT. Here we explored the effects of scale-up of self-organizing EHT and present detailed electrophysiologic and contractile functional characterization. METHODS: Fibers from EHT were generated from self-organizing neonatal rat cardiac cells (0.5×10(6) to 3×10(6)/fiber) on fibrin. We characterized contractile patterns and measured contractile function using a force transducer, and

Double-Balloon Versus Selective Vein of Marshall Ethanol Infusion for Mitral Isthmus Block in Atrial Fibrillation.

BACKGROUND: Achieving durable mitral isthmus (MI) block remains challenging in persistent atrial fibrillation (PeAF). Vein of Marshall ethanol infusion (VOMEI) facilitates MI block, but the impact of ethanol delivery technique on lesion formation and procedural outcomes is not well established. METHODS: In this prospective, single-center study, 106 patients with PeAF were assigned to three groups: RF-only (n = 57), selective VOMEI (n = 34), and double-balloon VOMEI (n = 15). Electroanatomic mapping was performed before and after ethanol infusion. The primary procedural endpoint was acute bidi

Rate-Dependent Pacemap Matching in Scar-Related Ventricular Tachycardia: Impact of "TR Fusion" Phenomenon.

BACKGROUND: The impact of varying rates of pacemapping (PM) rates on QRS morphology and PM score matching in patients with scar-related ventricular tachycardia (VT) has not been systematically assessed. OBJECTIVES: In this study, the authors sought to assess the variability in PM score matching at different pacing rates. METHODS: During substrate mapping for VT ablation, PM was performed at cycle lengths (CLs) of 600 ms, 500 ms, 400 ms, 300 ms, and VT CL. PM scores were compared for the entire QRS, the first half (H1) of QRS, and the second half (H2) of QRS to examine the influence of the pr

Acute electrical and hemodynamic effects of multisite left ventricular pacing for cardiac resynchronization therapy in the dyssynchronous canine heart.

BACKGROUND: Multisite left ventricular (multi-LV) epicardial pacing has been proposed as an alternative to conventional single-site LV (single-LV) pacing to increase the efficacy of cardiac resynchronization therapy. OBJECTIVE: To compare the effects of multi-LV versus single-LV pacing in dogs with left bundle branch block (LBBB). METHODS: Studies were performed in 9 anaesthetized dogs with chronic LBBB using 7 LV epicardial electrodes. Each electrode was tested alone and in combination with 1, 2, 3, and 6 other electrodes, the sequence of which was chosen on the basis of practical real-time

Different relations between infarct size and occluded bed size in barbiturate-anesthetized versus conscious dogs.

The relation between infarct size and occluded bed size in barbiturate-anesthetized (n = 32) and conscious (n = 34) dogs was compared using models of the left anterior descending (n = 43) and circumflex (n = 23) coronary arteries with 2 day old infarcts. Infarct and occluded bed (postmortem coronary arteriography) masses were measured by computerized planimetry of weighed left ventricular rings. For either type of occlusion, infarcts were larger in anesthetized than in conscious dogs (56 versus 33% occluded bed, p less than 0.001), with greater slopes of the linear regressions between infarct

Novel Metric Using Laplacian Eigenmaps to Evaluate Ischemic Stress on the Torso Surface.

The underlying pathophysiology of myocardial ischemia is incompletely understood, resulting in persistent difficulty of diagnosis. This limited understanding of underlying mechanisms encourages a data driven approach, which seeks to identify patterns in the ECG data that can be linked statistically to disease states. Laplacian Eigen-maps (LE) is a dimensionality reduction method popularized in machine learning that we have shown in large animal experiments to identify underlying ischemic stress both earlier in an ischemic episode, and more robustly, than typical clinical markers. We have now e

Body surface isopotential mapping of the entire QRST complex in the Wolff-Parkinson-White syndrome. Correlation with the location of the accessory pathway.

Body surface potential maps were recorded during sinus rhythm and during atrial pacing at the time of electrophysiologic studies in 42 patients with Wolff-Parkinson-White syndrome. The locations of the accessory pathways were determined by epicardial mapping during surgery in 34 patients and by multicatheter endocavitary electrophysiologic studies in eight additional patients. During delta wave inscription, the shape and extension of areas of the negative and positive potentials on the thorax correlated better with the preexcitation site (69% of patients) than with the localization of the mini…

High-density mapping-guided corrective HIS bundle pacing after failed CRT upgrade for persistent left superior vena cava.

In patients with persistent left superior vena cava (PLSVC), transvenous device implantation for cardiac resynchronization therapy (CRT) may be challenging. We present a complex case with successful, high-density electroanatomic mapping (EAM) guided corrective His bundle pacing (CHBP) following failed CRT upgrade in a patient with PLSVC, congenital heart disease, and pacing-associated heart failure. CHBP restored physiological conduction in left bundle branch block with complete conduction block leading to clinical improvement and cardiac remodeling. The presented case supports the growing evi

Ablation compared with drug therapy for recurrent ventricular tachycardia in arrhythmogenic right ventricular cardiomyopathy: Results from a multicenter study.

BACKGROUND: The comparative efficacy of antiarrhythmic drug (AAD) therapy vs ventricular tachycardia (VT) ablation in arrhythmogenic right ventricular cardiomyopathy (ARVC) is unknown. OBJECTIVE: We compared outcomes of AAD and/or β-blocker (BB) therapy with those of VT ablation (with AAD/BB) in patients with ARVC who had recurrent VT. METHODS: In a multicenter retrospective study, 110 patients with ARVC (mean age 38 ± 17 years; 91[83%] men) with a minimum of 3 VT episodes were included; 77 (70%) were initially treated with AAD/BB and 32 (29%) underwent ablation. Subsequently, 43 of the 77 p

The "hidden" concealed left-sided accessory pathway: An uncommon cause of SVT in young people.

BACKGROUND: Concealed left-sided accessory pathways (CLAP) are a cause of supraventricular tachycardia (SVT) in the young. Most are mapped with right ventricular (RV) apical/outflow pacing. Rarely, alternative means of mapping are required. We review our experience from three pediatric electrophysiology (EP) centers with a rare form of "hidden" CLAP. METHODS: All patients <21 years undergoing EP study from 2008 to 2014 with a "hidden" CLAP (defined as an accessory pathway [AP] for which RV pacing at cycle lengths [CL] stable for mapping did not demonstrate eccentric retrograde conduction) wer

Regional Left Ventricular Electrical Activation and Peak Contraction Are Closely Related in Candidates for Cardiac Resynchronization Therapy.

OBJECTIVES: This study determined the relationship between the timing of left ventricular (LV) electrical activation and peak contraction at potential LV pacing locations in candidates for cardiac resynchronization therapy (CRT). BACKGROUND: Targeting the LV lead to the region of latest electrical activation or the segment of latest peak contraction has both been shown to improve CRT response. Whether these regions correspond within CRT patients is uncertain. METHODS: Twenty-eight consecutive CRT candidates underwent intraprocedural coronary venous electroanatomic mapping using EnSite NavX.

Therapeutic efficacy of environmental enrichment on behavioral, endocrine, and synaptic alterations in an animal model of maternal immune activation.

BACKGROUND: Cardiac optical mapping is an imaging technique that measures fluorescent signals across a cardiac preparation. Dual optical imaging of voltage-sensitive and calcium-sensitive probes allows for simultaneous recordings of cardiac action potentials and intracellular calcium transients with high spatiotemporal resolution. The analysis of these complex optical datasets is both time intensive and technically challenging; as such, we have developed a software package for semi-automated image processing and analysis. Herein, we report an updated version of our software package (KairoSight

ML and MAP estimation of parameters for the Kalman filter and smoother applied to electrocardiographic imaging.

In electrocardiographic imaging (ECGI), one solves the inverse problem of electrocardiography (ECG) to reconstruct equivalent cardiac sources based on the body surface potential measurements and a mathematical model of the torso. Due to attenuation and spatial smoothing within the torso, this inverse problem is ill-posed. Among many regularization approaches used in the ECG literature to overcome this ill-posedness, statistical techniques have received great attention because of their flexibility to represent the data, and ability to provide performance evaluation tools for quantification of u

Endogenous hydrogen peroxide in paraventricular nucleus mediates sympathetic activation and enhanced cardiac sympathetic afferent reflex in renovascular hypertensive rats.

An enhancement of the cardiac sympathetic afferent reflex (CSAR) contributes to sympathetic activation in renovascular hypertension. Angiotensin II in the paraventricular nucleus (PVN) augments the CSAR and increases sympathetic outflow and blood pressure. The present study aimed to determine whether endogenous hydrogen peroxide in the PVN mediated the enhanced CSAR, sympathetic activity and the effects of angiotensin II in the PVN in renovascular hypertension induced by the two-kidney, one-clip method (2K1C) in rats. At the end of the fourth week, the rats underwent sino-aortic and vagal dene

Wolff-Parkinson-White ablation after a prior failure: a 7-year multicentre experience.

AIMS: Catheter ablation for Wolff-Parkinson-White syndrome (WPW) can be challenging and is associated with failure in approximately 1-5% of cases. We analysed the reasons for failure. METHODS AND RESULTS: All patients (89 patients, 28 +/- 16 years old) referred for WPW ablation after a prior failure were studied. Reasons for the prior failure as well as for the acute success were analysed. The repeat procedure was successful in 81 (91%) patients. Multiple (2.7 +/- 0.9) or large accessory pathways (APs) were seen in 13 patients. For left lateral APs, inaccurate mapping and lack of transseptal
