Ectopic Thymic Tissue Presenting as an Epiglottic Mass Compromising a Neonatal Airway: A Case Report.

An epiglottic mass (EM) is rarely found in neonates and poses life-threatening airway complications. We present the case of an infant urgently transferred from Belize via the World Pediatric Project with a lingual EM. The EM was misdiagnosed twice. The patient's home country of Belize initially misdiagnosed the EM as an elongated uvula and then again as an esophageal polyp. The true nature of the upper airway mass (UAM) was only discovered intraoperatively on rigid bronchoscopy as a lingual, mobile EM. Pathology of the lingual EM showed fibrovascular tissue with an area of exuberant capillary

Defining Epiglottic Collapses Patterns in Obstructive Sleep Apnea Patients: Francia-Lugo Classification.

Obstructive Sleep Apnea (OSA) is characterized by repetitive collapse of the upper airway during sleep. Drug-Induced Sleep endoscopy (DISE) is used to identify the collapse site. Among the possible sites of collapse, the epiglottis occurs more frequently than previously described. In this study, we reviewed DISE findings and classified different epiglottic collapse patterns. We found 104 patients (16.4%) with epiglottis collapse (primary 12.5% and secondary 3.9%). We described the following patterns of epiglottis collapse: Anterior-Posterior (AP) collapse with rigid component "trapdoor type" (

A Lot to Choke on: Case of Adult Epiglottitis with Concurrent Peritonsillar Abscess in a Patient with a Sore Throat.

BACKGROUND: In the winter months, one often sees a large increase in the volume of patients presenting to emergency departments with acute pharyngitis. While most cases of acute pharyngitis are benign, a rare minority can be life threatening. CASE REPORT: We report a case of epiglottis with a concomitant peritonsillar abscess (PTA) in an adult who presented to the emergency department with a sore throat. Computed tomography (CT) scan showed epiglottitis with a developing left PTA. The patient was treated with broad-spectrum antibiotics, high-dose steroids, and underwent multiple laryngoscopie…

Foreign Body Granuloma of the Epiglottic Vallecula Mimicking Malignancy on MRI and FDG PET/CT.

A 66-year-old man complained of foreign body sensation of the throat and progressive dysphagia without any trigger. Videolaryngoscopy showed a whitish lesion in the epiglottic vallecula. Neck enhanced MRI showed a lesion in epiglottic vallecula with remarkable enhancement. FDG PET/CT showed intense FDG uptake of the lesion with SUVmax of 28. Malignant tumor was suspected. The patient underwent microlaryngoscopic surgery. Histopathologic findings of the specimen were consistent with foreign body granuloma.

Epiglottic masses identified on CT imaging: A case report and review of the broad differential diagnosis.

Epiglottic masses may be cystic, granulomatous, infectious, benign or malignant neoplastic, or manifestations of a systemic disease. When large in size, the airway may become obstructed, and when accompanied by suspicious features such as cartilaginous invasion, extension to the pre-epiglottic or para-glottic spaces, or lymphadenopathy, the radiologist must consider malignancy as a primary differential diagnosis. However, when only benign features are identified, the differential diagnosis is broad. We present a 65-year-old female with an incidental 1 cm exophytic, pedunculated, papillomatous

Clinical Features and Outcome of Dogs with Epiglottic Retroversion With or Without Surgical Treatment: 24 Cases.

BACKGROUND: Published information describing the clinical features and outcome for dogs with epiglottic retroversion (ER) is limited. HYPOTHESIS/OBJECTIVES: To describe clinical features, comorbidities, outcome of surgical versus medical treatment and long-term follow-up for dogs with ER. We hypothesized that dogs with ER would have upper airway comorbidities and that surgical management (epiglottopexy or subtotal epiglottectomy) would improve long-term outcome compared to medical management alone. ANIMALS: Twenty-four client-owned dogs. METHODS: Retrospective review of medical records to i

Epiglottic cyst incidentally discovered during screening endoscopy: a case report and review of literature.

From the endoscopists' point of view, although the main focus of upper gastrointestinal endoscopic examination is the esophagus, stomach, and duodenum (usually bulb and 2nd portion including ampulla of Vater), the portions of the upper airway may also be observed during insertion and withdrawal of the endoscope, such as pharynx and larynx. Thus, a variety of pathologic lesions of the upper airway can be encountered during upper endoscopy. Among these lesions, an epiglottic cyst is relatively uncommon. The cyst has no malignant potential and mostly remains asymptomatic in adults. However, if la

Tracheotomy--a satisfactory method in the treatment of acute epiglottitis. A clinical and functional follow-up study.

A follow-up study, including clinical examination, X-ray and function tests, of 27 children with acute epiglottitis treated with tracheotomy at the ENT-Department, Mölndal Hospital during 1971-1975 has been performed. The function tests were an important part of the investigation and included measurement with the He-dilution technique, flow--volume curves and the forced oscillation technique. No child had any detectable tracheal stenosis or any other serious per- or postoperative complication. The only complication at all was one ugly scar on the neck. The tracheotomized patient needs very lit…

Simultaneous purulent pericarditis and acute epiglottitis from Haemophilus influenzae in an immunocompetent adult: a case report.

OBJECTIVES: Haemophilus influenzae can cause severe and invasive disease. While purulent pericarditis and epiglottitis are known to be rare complications of Haemophilus influenzae, we report a rare case of their simultaneous occurrence. METHODS: We report a 70-year-old female who presented with a sore throat and dyspnea to the emergency department, was found to have epiglottitis, and then rapidly deteriorated, developing shock and acute respiratory failure. A point-of-care ultrasound was performed, revealing a pericardial effusion with tamponade. RESULTS: Emergent pericardiocentesis yielded …

Adult Epiglottitis as an Often Overlooked, Life-threatening Condition Requiring Special Airway Consideration; a Case Report.

After the implementation of vaccination against Haemophilus Influenza Type B (Hib), adult patients comprise the majority of patients with acute epiglottitis. Its presentation with stridor mandates swift recognition and intervention to prevent airway compromise and mortality. Here, we present a case of a 44-year-old male with diabetes mellitus who presented with acute onset of shortness of breath and stridor following days of throat soreness and fever. Initial treatment for suspected anaphylaxis provided partial relief, and emergency department evaluation revealed characteristic signs of acute …

Elevated risk of acute epiglottitis in patients with chronic obstructive pulmonary disease: A nationwide cohort study.

OBJECTIVE: In individuals with epiglottitis, chronic obstructive pulmonary disease (COPD) is a common comorbidity; however, the impact of COPD under such circumstances is not well documented. Therefore, we performed this population-based study to determine whether, in adults, COPD is a risk factor for epiglottitis. METHODS: In this retrospective matched-cohort study, data obtained from the Taiwan National Health Insurance Research Database were analyzed. We identified all patients newly diagnosed as having COPD in 2000-2011 and performed frequency matching and propensity-score matching for ev…

Epiglottitis in Sydney before and after the introduction of vaccination against Haemophilus influenzae type b disease.

BACKGROUND: Acute epiglottitis due to infection with Haemophilus influenzae type b (Hib) is much less common in children following the introduction of Hib vaccination; however, adult epiglottitis cases have not decreased. In addition, epiglottitis hospitalizations are consistently more numerous than notifications and the reason for this is not clear. AIMS: To more accurately describe the clinical, aetiological and epidemiological features of epiglottitis and to ascertain the accuracy of hospitalization data in an era of widespread Hib vaccination. METHODS: Medical records in 11 public hospit…

Lateral epiglottic collapse in obstructive sleep apnea is associated with laterally directed pharyngeal collapse.

STUDY OBJECTIVES: Epiglottic collapse can obstruct the airway in patients with obstructive sleep apnea in an anteroposterior or lateral direction. The present study investigates the concept that lateral or concentric pharyngeal collapse patterns may remodel the epiglottis and predispose it to lateral collapse. To do so, we hypothesized that the presence of-any form of laterally directed pharyngeal collapse, eg, oropharyngeal lateral wall collapse or complete concentric collapse of the soft palate, is associated with increased odds for having concurrent lateral epiglottic collapse (E-lat). Surgical Treatment Options for Epiglottic Collapse in Adult Obstructive Sleep Apnoea: A Systematic Review.

The critical role of epiglottis in airway narrowing contributing to obstructive sleep apnoea (OSA) and continuous positive airway pressure (CPAP) intolerance has recently been revealed. This systematic review was conducted to evaluate available surgical treatment options for epiglottic collapse in adult patients with OSA. The Pubmed and Scopus databases were searched for relevant articles up to and including March 2022 and sixteen studies were selected. Overall, six different surgical techniques were described, including partial epiglottectomy, epiglottis stiffening operation, glossoepiglottop

Airway problems related to laryngeal mask airway use associated with an undiagnosed epiglottic cyst.

This report describes airway management difficulties in a patient with an asymptomatic and undiagnosed epiglottic cyst. Mask ventilation after induction of general anesthesia was uneventful. Insertion of an LMA resulted in inadequate ventilation. Fiberoptic intubation through an intubating LMA was impossible because the LMA pushed the cyst into the laryngeal inlet. Fibreoptic intubation, assisted by the use of a direct laryngoscope without the LMA was uneventful. Preoperative craniocervical scans performed for assessment of other conditions may reveal asymptomatic pathology in the airway.

Xylazine-induced attenuation of dorsal displacement of the soft palate associated with epiglottic dysfunction in a horse.

An 18-month-old Quarter Horse filly was evaluated because of dorsal displacement of the soft palate associated with epiglottic dysfunction that caused exercise intolerance and an abnormal respiratory noise. The abnormality of the epiglottis was corrected by use of a sedative dose of xylazine hydrochloride. There was no familial predisposition to hyperkalemic periodic paralysis, which may cause similar laryngopharyngeal signs, and the horse did not respond to administration of acetazolamide. There was no known history of trauma that could have caused neuritis, which has also been suggested as a

Epiglottic augmentation by use of polytetrafluoroethylene to correct dorsal displacement of the soft palate in a standardbred horse.

A 2-year-old 400-kg Standardbred colt with a history of exercise intolerance was determined to have dorsal displacement of the soft palate (DDSP). Standing endoscopy revealed a soft, flaccid, and hypoplastic epiglottis and arytenoid cartilages that moved symmetrically and abducted fully. Lateral laryngeal radiography was used to calculate a corrected thyroepiglottic length of 6.48 cm. Videoendoscopy during exercise on a high-speed treadmill at standardized treadmill speeds was performed weekly for 3 consecutive weeks. The soft palate dorsally displaced at each examination. A sternohyoideus/ste
