[Approaches to the hip in the child. II. Wide approaches (author's transl)].

A wide exposure of the hip joint is often necessary in the treatment of congenital dislocation of the hip: Salter's or Olliers's approach, and in that of epiphysiolysis: Dunn's approach. This description of these approaches completes a review of the surgical approaches which are used in the child.

Radiology in early slipped femoral capital epiphysis.

In a study of 38 hips showing only minor abnormality on the AP radiograph, it was found that the most reliable sign of an early slip was blurring of the junction between the metaphysis and the growth plate. The diagnosis could not be made on the AP film alone in 11% of cases and it is recommended that a routine lateral radiograph is obtained when slipped epiphysis is a clinical possibility. The importance of the radiologist in the early diagnosis of the condition is stressed and the causes of diagnostic delay are considered.

Acute chondrolysis complicating slipped capital femoral epiphysis.

Radiographs of 20 patients with acute chondrolysis complicating a slipped capital femoral epiphysis were reviewed with emphasis on the radiographic features and differential diagnosis. There was bilateral disease in five instances; radiographic follow-up ranged from 6 months to 5 years. The onset of chondrolysis occurred after surgery in 18 of the 25 abnormal hips. Acute necrosis of the articular cartilage is characterized radiographically by (1) persistent juxtaarticular osteoporosis; (2) progressive narrowing of all or a portion of the cartilage space; and (3) erosion of the subchondral cort

Slipped femoral capital epiphysis as a sequela to childhood irradiation for malignant tumors.

Five cases of slipped femoral capital epiphysis were noted in children with prior irradiation for malignancy whose femoral head and neck had been included in the radiation portal. Three of the 5 were patients who had lymphoma. As a result of increasing survival rates for these types of patients, this previously unrecognized radiation-related growth disturbance may be more likely to be seen in the future.

High-grade slippage of the lumbar spine in a rat model of spondylolisthesis: effects of cyclooxygenase-2 inhibitor on its deformity.

STUDY DESIGN: Radiographic and histologic evaluation of spondylolisthesis in a rat model. OBJECTIVES: To investigate the effects of etodolac, a cox-2 inhibitor, on the severity of spondylolisthesis in a 4-week-old rat model. SUMMARY OF BACKGROUND DATA: Spondylolisthesis occurs associated with spondylolysis in some pediatric patients. In such patients, the percent of forward slippage varies individually ranging between 0% and 100%. The factors determining the severity of forward slippage have not been clarified as yet. In earlier studies, we found that growth plate stress fracture was the bas

[Limb emergencies in children].

The three main categories of limb emergencies are trauma, infection and the painful hip. Conventional radiography is used as the initial screening imaging modality that can sometimes be complemented with sonography. Computed tomography and MRI are rarely needed. Familiarity with conditions specific to the pediatric population is essential to the diagnosis and management of these patients. Traumatic injuries in children involve growing bones with large cartilaginous component and biomechanical resistance different from that in adults. Lim infections in children are affected by the characteristi

Comparison between anterior cuneiform osteotomy and Dunn procedure in the surgical management of unstable severe slipped femoral epiphysis: A case-control study of 41 patients.

BACKGROUND: There is little consensus on the best treatment algorithm for unstable severe slipped capital femoral epiphysis (SCFE). Subcapital osteotomy, which is one of the surgical options, is performed either anteriorly (anterior cuneiform osteotomy, CO) or laterally with trochanteric osteotomy (Dunn procedure, DP). The CO is technically easier and decreases operating time. Moreover, because the DP was the standard in our department before it was replaced by the CO, we had a series of consecutive patients. Therefore, we did a retrospective case-control study in unstable, severe SCFEs treate

Clinical outcome after transfixation of the epiphysis with Kirschner wires in unstable slipped capital femoral epiphysis.

Treatment of slipped capital femoral epiphysis (SCFE) is still controversial. Agreement has not yet been reached on the appropriate time to perform surgery, the necessity of repositioning manoeuvres, the type of implants for stabilisation, or the need for prophylactic treatment of the contralateral side. In this retrospective study, we present 29 patients with unstable (acute and acute-on-chronic) SCFE treated by internal fixation of the epiphysis with three or four Kirschner wires both therapeutically on the affected side and prophylactically on the not (yet) affected side. After hardware rem

Combined Surgical Dislocations and Proximal Femoral Osteotomies for Treatment of Complex Proximal Femoral Deformities: A Systematic Review.

BACKGROUND: Complex proximal femoral deformities such as slipped capital femoral epiphyses (SCFE) or residual Perthes disease pose challenging clinical problems due to the severity of pathological femoral head and neck changes from long-standing childhood disease. Utilizing the combination of surgical dislocation (SD) and proximal femoral osteotomy (PFO) is an option to address both the intra-articular pathologies as well as the maximal correction of the proximal femoral deformities. The purpose of this systematic review was to report clinical and radiographical outcomes for patients undergoin

Bilateral Proximal Femur Interval Fractures with a Slipped Femoral Capital Epiphysis After Fixation: A Case Report.

CASE: This case report, with 3 years of follow-up, describes a 9-year-old boy who suffered sequential Delbet type IV proximal femoral fractures. Three months postoperatively he had a unilateral slipped capital femoral epiphysis and had to undergo further surgery to address this. CONCLUSION: Proximal femur fractures are rare in childhood, and a slipped capital femoral epiphysis after fixation of a Delbet type 4 fracture has only been described once before in the literature. This case report highlights underlying risk factors of subsequent slipped capital femoral epiphysis after a Delbet type I

Osseoscopy-Assisted Femoral Head Restoration and Vascularized Graft for Slipped Capital Femoral Epiphysis: A Case Report.

CASE: A 13-year-old obese adolescent boy with hypothyroidism developed left femoral head avascular necrosis with collapse after slipped capital femoral epiphysis. We restored the femoral head congruity with the aid of osseoscopy and vascularized free fibular graft (VFFG). Four years after surgery, the patient had painless hip range of motion and femoral head contour was maintained. CONCLUSION: This case highlights a novel use of an arthroscopic camera inside the femoral head to visualize and debride the area of collapse, fill the cavity with cancellous bone graft, and use a VFFG to restore ar

Who Benefits From Allowing the Physis to Grow in Slipped Capital Femoral Epiphysis?

BACKGROUND: The globally acknowledged treatment for mild to moderate slipped capital femoral epiphysis (SCFE) is single screw in situ fixation, also used for prophylactic contralateral fixation. The Free-Gliding Screw (FG; Pega Medical) is a 2-part free-extending screw system designed to allow the growth of the proximal femur. We aimed to analyze the relationship between skeletal maturity and potential growth of the proximal physis and remodeling of the femoral neck using this implant. MATERIALS AND METHODS: Females below 12 years and males below 14 years undergoing in situ fixation for stabl

Development and Validation of SCFE Percutaneous Pinning Surgical Simulation.

BACKGROUND: In situ screw fixation with a single percutaneously placed femoral screw remains widely accepted for femoral head fixation in adolescent patients with slipped capital femoral epiphysis (SCFE). Given the potential risks involved with this procedure, a simulation whereby surgical skills could be refined before entering the operating room may be of benefit to orthopaedic trainees. METHODS: We developed a synthetic model for the simulated treatment of SCFE. Five orthopaedic attendings and twenty trainees were recorded performing an in situ percutaneous fixation on the SCFE model. Time

Mechanical Failure of 2 Cannulated Screw Fixation for Unstable SCFE: A Case Report.

CASE: A 13-year-old obese boy presented with an acute-on-chronic unstable left slipped capital femoral epiphysis (SCFE). He underwent in situ surgical fixation with two 6.5-mm fully threaded cannulated screws. At 6 months, he presented with mechanical failure of both screws. He underwent screw removal, revision in situ fixation, a peritrochanteric flexion and internal rotational osteotomy, and an open femoroplasty. The osteotomy healed at 6 weeks. The femoral physis took an additional year to close. CONCLUSION: This case highlights an uncommon complication of in situ pinning of SCFE, discusse

[Early and late complications and their management in slipped capital femoral epiphysis].

BACKGROUND: Slipped capital femoral epiphysis (SCFE) remains a challenge for the treating surgeon. First of all, SCFE should be diagnosed as early as possible. The earlier the diagnosis is made in adolescents, the lower the slip angle will be. Mild slips show more favourable long-term courses than moderate and severe SCFE. COMPLICATIONS: With increasing slip angle, the risk of complications increases. The complications of SCFE are diverse, the most severe of which are avascular necrosis (AVN) and chondrolysis. AVN is more common in surgically treated than in non-operatively managed patients a
