OBJECTIVE: To analyse the diagnostic value of laparoscopy in patients with tubal infertility.
METHODS: The morphological appearance of fallopian tubes and surrounding tissues of 1120 cases with proven tubal infertility were observed under laparoscopy. Hydrotubation was performed at the same time in each case.

OBJECTIVES: To investigate the localization of manganese superoxide dismutase (Mn-SOD) and copper-zinc superoxide dismutase (Cu, Zn-SOD) in the human ovary and fallopian tube, and to examine the role of superoxide radicals and SODs in the human ovulatory process.
METHODS: Using immunohistochemical methods, we studied the localization of SODs in 22 human ovaries, in 18 fallopian tubes, and in aspirated granulosa cells. We measured, by enzyme-linked immunosorbent assay, the concentrations of SODs in follicular fluid taken from 94 IVF patients.

From 1978 through 1993 four patients underwent surgical treatment of primary fallopian tube rumors. Mean age was 62.5 years. A different histologic pattern was found in each cases: transitional, endometrioid, serous papillary and malignant mixed mesodermal tumor. Prognostic significance of histologic patterns is underlined together with grade and stage; all our cases were included in stage IA. Surgical treatment was, in all cases, hysterectomy with bilateral salpingo-oophorectomy; in two cases an adjunctive chemotherapy followed. A second look was performed in two cases. We have developed an insemination method using a large (4 ml) volume of the inseminate. This method incorporates ovarian stimulation, isolation of an optimal number of motile spermatozoa and, finally, Fallopian tube sperm perfusion (FSP). In a clinical study, 139 couples with various causes of infertility, had a total of 239 treatment cycles. Hormone-responsive adenylyl cyclase (AC) activity in biopsies from normal human Fallopian tubes was studied. Enzyme activity with a Km of 0.15 mmol/L and a maximum velocity of 13.8 pmol/mg.min in the basal condition was demonstrated. The addition of prostaglandin E1 (PGE1), PGE2, PGF2a, vasoactive intestinal polypeptide, isoproterenol, and terbutaline increased enzyme activity, with no change in the Km. Forty-three cases of primary tubal adenocarcinoma were treated at Memorial Sloan-Kettering Cancer Center between 1979 and 1989. Thirty-eight patients who received cisplatin-based combination chemotherapy following primary surgery were reviewed. The mean patient age was 59 years, with 86% postmenopausal. Distribution by stage was as follows: I, 3 (7%); II, 4 (11%); III, 27 (71%); and IV, 4 (11%). Median follow-up from the time of diagnosis was 62 months (range, 5-132). The association of tubal infertility and ectopic pregnancy with Chlamydia trachomatis infection was investigated using a case-control study design. Although culture methods failed to document active chlamydial infection in the majority of cases, serology revealed a significant association of Chlamydia trachomatis antibody with tubal infertility and ectopic pregnancy. Thirteen of 18 (72%) women with tubal factor infertility and 18 of 32 (56%) women with ectopic pregnancy had antibodies to Chlamydia trachomatis as compared to 11 of 49 (22%) normal pregnant controls. This case report describes the expulsion of an IUD (Gravigard) through the Fallopian tube. Such a phenomenon has been suggested earlier but never reported. It is concluded that the shape of the IUD is of importance for such a route of expulsion.

A rare genital anomaly is described, consisting of a right hemiuterus with a rudimentary horn and a remotely located left fallopian tube, totally disconnected from the pelvic genitalia, and ruptured by an ectopic pregnancy. An associated finding was a unilateral hyperstimulated ovary (post HMG-HCG therapy), confined to an accessory ovary on the left. The case is presented in detail, and the unusual anatomical and functional features are discussed.
