Necrotizing fasciitis is a severe, fulminant infection most commonly encountered in patients with diabetes mellitus, alcohol abuse, and intravenous drug abuse. The infection can spread-unrecognized along fascial planes beneath seemingly normal skin. The relatively benign appearance of the extremity is misleading and often results in delay in diagnosis and increased morbidity or death. Immediate aggressive surgical debridement through extensile incisions in combination with antibiotic therapy is necessary for control of these limb- and life-threatening, soft-tissue infections. Gas gangrene, or …

Necrotizing fasciitis is a rare but life-threatening infection of the subcutaneous tissue. In addition to Group A streptococci, polymicrobial infection with various aerobic and anaerobic gram positive or gram negative bacteria is frequently observed. The present case illustrates that a high level of clinical suspicion is necessary for early diagnosis of this disease and confirms the value of computed tomography in patients with sepsis of unknown origin. Because of rapid progression of the disease early debridement of necrotic tissue is mandatory for survival.

OBJECTIVES: 1. To determine the sex distribution of Plantar fasciitis in Harare. 2. To determine the presence or absence of a calcaneal spur and its role in causation and hence treatment. 3. To determine unilateral or bilateral involvement. 4. To determine the response to steroidal injection. 5. To determine the role of occupation and/or activities if any.
DESIGN: Patients presenting with heel pad pain were carefully examined, the age, sex and occupation recorded. They were followed up at two weekly intervals following treatment.
SETTING: This prospective study was carried out at a private s…

Necrotizing fasciitis is a relatively rare, potentially life-threatening infection involving the subcutaneous tissues. We report a case of group A streptococcal necrotizing fasciitis/myositis in which CT played an important role in differential diagnosis.

The authors present the results of treatment provided to 11 patients with necrotizing fasciitis who were after a surgical operation and after administration of antibiotics treated in a hyperbaric chamber. A total of 8 patients (82%) recovered completely. The authors discuss the theory of action of hyperbaric oxygen on microorganisms and draw attention to the possible reduction of mortality of this serious disease when using hyperbaric oxygenation. The latter is considered an important auxiliary method which supplements surgical treatment, rational antibiotic therapy and in particular careful i…

A case of necrotizing fasciitis of the vulva arising in a leukemic patient during a chemotherapy nadir is presented. The nature of the process was not recognized initially, and the patient was treated with intravenous antibiotics. After clinically evident necrosis developed, prompt surgical debridement was carried out. Clinical improvement followed and the wound healed well. This case suggests that chemotherapy with its attendant immunosuppression is a predisposing factor in the development of necrotizing fasciitis and warrants a high index of suspicion.

Necrotizing fasciitis is a serious infectious disease characterized by a necrotizing cellulitis of the subcutaneous tissue. The mortality has not changed over the last 50 years, probably because the condition is not recognized early enough to permit effective treatment by debridement. We report 3 cases illustrating different clinical courses.

Five patients with necrotizing fasciitis are reported. Streptococci were isolated in all but one patient, in whom Staphylococcus albus was responsible. Other organisms were also isolated in culture. Surgical debridement and skin grafting were needed in all our patients, in addition to systemic antibiotics and topical treatment, comprising Eusol paraffin soaks. One patient developed this condition after a skin biopsy; another had chronic venous leg ulcer. There was no predisposing cause in the remaining three patients.

Eosinophilic fasciitis or Shulman's syndrome is linked to scleroderma, systemic sclerodermia as various types of localized sclerodermia, by a nosological relationship analyzed by the authors. But, excluding the one concerning eosinophilic fasciitis and deep morphea, the authors seem to believe that the current differences are still justified, since the clinical picture and the levels of the histological lesions vary from one entity to the other. The nosological debate concerning eosinophilic fasciitis demonstrates also that a histologically recognized fasciitis has no complete specificity. By …

Necrotizing fasciitis of the neck (NFN) is a relatively rare, fulminating infectious process of the cervicofacial tissues which may cause sudden and unexpected death. Although often the result of a dental infection, injuries of the soft tissues of the neck may also initiate rampant cellulitis, and recognition of the underlying etiology of such cases is necessary to determine properly the manner of death. Five cases of NFN are presented with a review of the causative factors and usual bacteriology, and specific factors of medicolegal interest are addressed.

The mortality of acute appendicitis increases sixfold if perforation occurs. We have reported a case of perforated appendix complicated by necrotizing fasciitis of the abdominal wall and retroperitoneum. We believe this complication has not been previously described in the English literature.

Plantar fasciitis is a common cause of pain, particularly in runners and certain other athletic groups. This syndrome must be distinguished from certain other conditions, such as the tarsal tunnel syndrome and achillodynia. Conservative therapy including rest, orthotics, heel cups, anti-inflammatory agents, and icing reduce symptoms in most patients. A few athletes may need surgery to continue running. The authors released the plantar fascia and excised areas of mucinoid degeneration in 15 athletes. Fourteen returned to full athletic activity.

Eosinophilic fasciitis, originally reported as a syndrome distinct from scleroderma, appears now to be an early inflammatory variant of scleroderma. No less than one half of the cases reported as eosinophilic fasciitis have convincing features of scleroderma, including Raynaud's phenomenon, esophageal dysmotility, restrictive lung disease, diffuse hyperpigmentation, synovitis, flexion contractures, dermal sclerosis, colonic diverticula, scleroderma kidney, positive latex fixation test, and the presence of serum antinuclear antibodies (ANA). Clinical presentations of scleroderma range from isol…

A patient with an extensive necrotizing infection of the endopelvic fascia is presented. Cultures and tissue sections revealed the organisms responsible to be a mixed infection of Bacteroides, Staphylococcus, and Streptococcus. Hypertension and diabetic vascular disease were felt to contribute to the initiation of the disease process in this unusual site.

PurposeIf the pain persists for a long time in the treatment of plantar fasciitis (PF) or if there is no response to treatment, central sensitization (CS) may develop and the pain may transform into nociplastic pain (NP). This study aimed to evaluate the frequency of CS and NP in patients with PF.MethodsThis cross-sectional study was undertaken between November 2023 and March 2024. The Foot Function Index (FFI) scale, which evaluates the foot's functionality, was applied to the patient group. The Visual Analog Scale (VAS), which evaluates pain intensity; the Pain-DETECT scale, which evaluates …

BACKGROUND: Monofocal necrotizing fasciitis (MONF) involves a single site in a rapidly progressing infection and necrosis of the fascia and surrounding soft tissue. Synchronous multifocal necrotizing fasciitis (SMNF), the simultaneous development of NF in multiple noncontiguous sites, is rarely reported. This study aimed to compare the clinical characteristics and outcomes between patients with SMNF and MONF, and to determine the risk factors of SMNF.
METHODS: Our retrospective case-control study compared the clinical characteristics and outcomes, between January 2006 and January 2013, of pat…

Introduction Early operative debridement of necrotising fasciitis is a major outcome determinant. Identification and diagnosis of such patients can be clinically difficult. The Laboratory Risk Indicator for Necrotising Fasciitis (LRINEC) score first published in 2004 is based on routinely performed parameters and offers a method for identifying early cases. No literature review has yet been performed on the application of such a score. Methods A systematic review of English-language literature was performed from 2004 to 2014 to identify articles reporting use of LRINEC score and the incidence …
