Schwannoma presenting with anteromedial knee pain and bone defect.

Tumors of the peripheral nerve sheath including schwannomas are known for their rarity and can present with neurological deficit. We report an interesting case of schwannoma of the infrapatellar branch of saphenous nerve presenting with anteromedial knee pain and proximal tibial metaphyseal defect. A 26-year-old man presented with pain and swelling (for the past 2 months) in the anteromedial aspect of knee. Investigations revealed scalloping lesion in the anteromedial aspect of proximal tibia with sclerosed margins. Surgical exploration identified an encapsulated tumor lying over the defect in

Femoral artery puncture site pseudoaneurysm formation following uterine artery embolization for symptomatic fibroids: a case report.

BACKGROUND: Uterine artery embolization (UAE) is a safe and durable alternative to hysterectomy for the treatment of fibroids. Complications arising in the femoral artery puncture site such as infection, bleeding, and hematoma formation have been reported. This is the first report of pseudoaneurysm formation following UAE. CASE: A 46-year-old primiparous woman with multiple symptomatic fibroids became hypertensive during transfemoral UAE. The vascular sheath removal was delayed until blood pressure had normalized. Manual compression was required to control bleeding from the right groin punctu

[Hypophosphatemic oncogenic osteomalacia].

The first case of oncogen osteomalacia in Hungary is reported, to draw the attention of the medical profession to it and to present the new data about its pathomechanism. Pathological hip fracture caused by hypophosphataemic osteomalacia due to isolated renal phosphate wasting was found in a previously healthy 19 years old sportsman. In spite of daily 1.5 micrograms calcitriol treatment and phosphate supplementation, hypophosphataemia persisted for 13 years and he needed regular indometacin medication for his bone pain. During that time an 1.5 cm gingival tumour was found and radically removed

[Haemophilic pseudo-tumours and iliac haematomas: radiological and clinical findings (author's transl)].

Between 1971 and 1977, 648 patients with haemophilia were seen; of these 38 had pseudo-tumours and 163 iliac haematomas. Amongst the latter 153 were cured by the administration of the appropriate factor. In ten the symptoms persisted. In the early stages the diagnosis cannot be made radiologically. Large haematomas produce indirect evidence of their presence by displacing the bladder. Particularly with small haematomas, computer tomography has proved a reliable and accurate diagnostic measure. Without treatment, a haemophilic pseudo-tumour may develop. Its radiological differential diagnosis i

Clinical, functional, and quality-of-life outcomes after arthroplasty versus osteosynthesis for metastatic lesions of the proximal femur: a 12-month observational cohort study.

BACKGROUND: Metastatic lesions of the proximal femur frequently result in severe pain, impaired mobility, and pathological fractures. Surgical intervention aims to restore skeletal stability, allow early mobilization, and improve quality of life. This study compared clinical, functional, and quality-of-life outcomes following endoprosthetic reconstruction (arthroplasty) versus intramedullary osteosynthesis in patients with metastatic proximal femoral disease. METHODS: This single-center observational cohort study included 54 patients treated with either arthroplasty (n = 27) or intramedullary

Ultrasound of the adult hip.

The adult hip is a new and challenging site for evaluation with ultrasound (US). Clinically, diseases involving the hip region may be difficult to diagnose without the help of imaging. The hip region is a crossroad for numerous vascular, nervous, and muscular structures that pass between the trunk and the lower extremity. Thus, inflammatory processes and neoplasms may spread to and from the hip, buttock, thigh, pelvis, and retroperitoneum, and trauma may also effect the adjacent areas of the trunk and thigh. Because it is a crossroad, many conditions that are not specific to the hip occur in t

Osteoid osteomas of the hip: a well-recognized entity with a proclivity for misdiagnosis.

OBJECTIVES: The diagnosis of osteoid osteomas (OO) about the hip can be challenging as presenting symptoms can mimic other, more common, periarticular pathologies. Our aims were to identify the most common misdiagnoses and treatments, mean delay in diagnosis, characteristic imaging features and provide tips for avoiding diagnostic imaging pitfalls for patients with OO of the hip. METHODS: We identified 33 patients (34 tumors) with OO about the hip who were referred for radiofrequency ablation between 1998 and 2020. Imaging studies reviewed included radiographs (n = 29), CT (n = 34), and MRI (

A pituitary cup biopsy is more accurate than reamings for histological diagnosis of intramedullary lesions during nailing of impending and pathologic fractures: a retrospective matched cohort analysis.

INTRODUCTION: Histologic diagnosis of the lesion is important while intramedullary nailing of a pathologic or impending fracture of a long bone. A biopsy can be performed extramedullary using a targeted sampling device such as a pituitary rongeur or intramedullary from the bone removed from the reamer during preparation of the intramedullary canal. The purpose of this study is to compare a cup pituitary rongeur vs. a reaming biopsy from the intramedullary canal during the treatment of pathologic bone lesions with an intramedullary nail. METHODS: An IRB approved retrospective case control stud

Prevention of Pathological Fracture of the Proximal Femur: A Systematic Review of Surgical and Percutaneous Image-Guided Techniques Used in Interventional Oncology.

Background and objectives: Patients suffering from bone metastasis are at high risk for pathological fractures and especially hip fractures. Osteolytic metastases can induce a high morbidity rate (i.e., pain, facture risk, mobility impairment), and operation on them can be difficult in this frail population having a reduced life expectancy. Several medical devices have been investigated for the prevention of these pathological hip fractures. Materials and Methods: To investigate these solutions, a literature review and a meta-analysis of primary studies was performed. Data sources included ele

Intermittent internal fixation with a locking plate to preserve epiphyseal growth function during limb-salvage surgery in a child with osteosarcoma of the distal femur: a case report.

Limb shortening is a problem associated with surgery for osteosarcoma of the lower extremity in adolescents, as the tumors frequently occur near the epiphysis. Herein we report the use of a less invasive stabilization system (LISS) and an intermittent fixation method to preserve the growth function of epiphysis in an 11-year-old patient with an osteosarcoma of the distal femur. The 11-year-old male presented with left knee enlargement and pain for 2 weeks, and magnetic resonance imaging (MRI) and biopsy were consistent with osteosarcoma of the left distal femur. After preoperative chemotherapy,

Giant cell tumor complicating Paget disease of long bone.

Giant cell tumor (GCT) is a rare complication of Paget disease of bone. It usually occurs in the skull or pelvic bones of patients with long-standing polyostotic disease. This report describes a 62-year-old patient who presented with monostotic Paget disease of the distal femur complicated by GCT. He had a 2-year history of discomfort and pain in his left knee. Conventional plain films and MRI demonstrated the characteristic bone changes of Paget disease and an associated lytic lesion involving the epiphyseal and metaphyseal regions of the distal femur. A diagnostic curettage showed the charac

Case report 603. Giant cell reparative granuloma of the distal end of right femur.

A GCRG of the femur in a 57-year-old man was described. He had undergone local radiotherapy and systemic chemotherapy for T-cell lymphoma of the jaw and scalp diagnosed 4 weeks previously. Since radiologically the lesion suggested an aggressive process, malignant lymphoma was the most probable preoperative diagnosis. In the differential diagnosis giant cell tumor, brown tumor of hyperparathyroidism, benign fibrous histiocytoma, and malignant fibrous histiocytoma were discussed in further detail. This histologically documented case of GCRG of the femur without any underlying process appears to

[Alloplasty of the hip joint and the proximal part of the femoral shaft].

The possibilities of the use of untypical prostheses of the hip in cases with lesion of the proximal femoral end together with a part of the shaft have been presented. A hip prosthesis with the upper part of the femur was implanted in two cases for neoplastic disease, and in one case for pseudoarthrosis of the femur within the intertrochanteric region. Good early clinical and radiological results was obtained and the total range of motion in the implanted joint was 210 degrees. After operation the patients did not complain about pain and they walked with the aid of a cane.
