BACKGROUND: The German DRG (diagnosis-related groups) system forms the basis for billing inpatient hospital services. It includes not only the case groups (G-DRGs), but also additional and innovation payments. This paper analyzes and evaluates the relevant developments of the G-DRG System 2010 for orthopedics and traumatology from the medical and classification perspectives.
METHODS: Analyses of relevant diagnoses, medical procedures and G-DRGs in the versions 2009 and 2010 based on the publications of the German DRG institute (InEK) and the German Institute of Medical Documentation and Infor

In a conservative political climate, it is very important that social workers and others who are concerned with social welfare understand how to affect social change through legislative advocacy. This article presents a case study in successful intervention using the passage by the Congress of the Ryan White Comprehensive AIDS Resources Emergency (CARE) Act of 1990. The major influences in the passage of this landmark bill were building a diverse coalition that provided a very broad base of advocacy; securing bipartisan support; using recognizable personalities to call attention to the problem

In fiscal years 1979 and 1980, the Genetic Diseases Service Branch of the Health Services Administration funded 34 state genetic service programs in the United States through monies from the National Genetic Diseases Act (PL 94-278) of 1976. During the two-year period, 131,818 at-risk individuals were provided with genetic counseling. Analyses of amniotic fluid were performed on 42,003 specimens, with 436 abnormal fetuses (1%) detected. Utilization ratios of amniocentesis for advanced maternal age ranged from a low of 5% to a high of 49%. Screening programs for inborn errors of metabolism repo

This analysis of the health system of the Caribbean Netherlands reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance on the islands of Bonaire, St Eustatius and Saba (the BES islands). Since the dissolution of the Netherlands Antilles in 2010, residents of the BES islands live in special municipalities of the Netherlands, each with its own government (public entity). The Ministry of Health, Welfare, and Sport (VWS) in The Hague, through the Department of Care and Youth Caribbean Netherlands (ZJCN), oversees the health sy

When a healthy infant dies suddenly and unexpectedly, it is critical to correctly determine if the death was caused by child abuse or neglect. Sudden unexpected infant deaths should be comprehensively investigated, ancillary tests and forensic procedures should be used to more-accurately identify the cause of death, and parents deserve to be approached in a nonaccusatory manner during the investigation. Missing a child abuse death can place other children at risk, and inappropriately approaching a sleep-related death as maltreatment can result in inappropriate criminal and protective services

BACKGROUND: Over 20 million Americans are currently or have been incarcerated. Most are from medically underserved populations; 1 in 3 African American men and 1 in 6 Latino men born in 2001 are projected to go to prison during their lifetime. The amount of funding from the National Institutes of Health (NIH) to understand and improve the health of persons involved with the criminal justice system is unknown.
OBJECTIVE: To describe NIH funding for research on the health and health care needs of criminal justice-involved persons.
DESIGN: Review of NIH grants (2008-2012) in the RePORT (Researc

Most nonprofits make program decisions based on a mission rather than a strategy. They rally under the banner of a particular cause, be it "fight homelessness" or "end hunger." And since their causes are so worthwhile, they support any programs that are related--even tangentially--to their core missions. It's hard to fault people for trying to improve the state of the world, but that approach to making decisions is misguided. Acting without a clear long-term strategy can stretch an agency's core capabilities and push it in unintended directions. The fundamental problem is that many nonprofits

One goal of the scientific research enterprise is to improve the lives of individuals and the overall health of societies. This goal is achieved through a combination of factors, including the composition of research portfolios. In turn, this composition is determined by a variety of scientific and societal needs. The recent history of polio research highlights the complex relations between research policy, scientific progress and societal benefits. Here, we briefly review the circumstances leading to the possibility of eradication of poliovirus, evaluate the research environment that emerged

For many poor Americans, having a decent home and suitable living environment remains a dream. This lack of adequate housing is not only a burden for many of the poor, but it is harmful to the larger society as well, because of the adverse effects of inadequate housing on public health. Not only is the failure to provide adequate housing shortsighted from a policy perspective, but it is also a failure to live up to societal obligations. There is a societal obligation to meet the housing needs of everyone, including the most disadvantaged. Housing assistance must become a federally-funded entit

BACKGROUND: In 2006, a local collective combating homelessness set up an 'experimental squat' in an abandoned building in Marseille, France's second largest city. They envisioned the squat as an alternative to conventional health and social services for individuals experiencing long-term homelessness and severe psychiatric disorders. Building on what they learned from the squat, some then joined a larger coalition that succeeded in convincing national government decision-makers to develop a scientific, intervention-based programme based on the Housing First model. This article analyses the pol

BACKGROUND: Ghana's imaging services face rising demand, uneven digital infrastructure, and limited access to advanced modalities. Artificial intelligence (AI) could improve diagnostic accuracy, workflow efficiency, and access, but real-world adoption is early.
OBJECTIVE: Assess Ghana's readiness to adopt AI in medical imaging, identify pathways and barriers, and propose a phased, context-specific roadmap.
METHODS: A review of peer-reviewed and grey literature (2012-March 2025) using PubMed, IEEE Xplore, Scopus, Google Scholar, and Ghanaian institutional documents. The review emphasizes imag

There is increasing recognition that without stronger health systems, efforts to improve global health and Universal Health Coverage cannot be achieved. Over the last three decades, initiatives to strengthen health systems in low-income countries have attracted huge investments in the context of achieving the Millennium Development Goals, the Sustainable Development Goals, as well as Universal Health Coverage. Yet, health system inadequacies persist, especially at the subnational level. Our paper presents a political economy analysis featuring a three-district case study in Uganda, where distr

BACKGROUND: Hospital Information Systems (HIS) are becoming increasingly vital to the digital transformation of healthcare; nevertheless, their implementation in low and middle-income countries is frequently obstructed by structural, financial, and governance issues. In Iraq, the extensive adoption of HIS is still at an early stage, with limited empirical evidence concerning institutional readiness. This study thus examined the organizational readiness for HIS implementation in the public hospitals located in Al-Amara, Iraq, and identified the structural and organizational determinants that sh

OBJECTIVES: Onchocerciasis remains a major public health problem in the Democratic Republic of Congo (DRC), one of the countries with the highest burden globally. Despite more than two decades of community-directed treatment with ivermectin (CDTI), progress toward elimination has been slower than expected. This review examines the main epidemiological, operational, and governance-related barriers that continue to hinder elimination efforts.
METHODS: We conducted a structured narrative review informed by World Health Organization (WHO) frameworks for evaluating neglected tropical disease progr
