Replantation of finger avulsion injuries: a systematic review of survival and functional outcomes.

PURPOSE: Recent studies presenting functional outcomes after replantation of finger avulsion injuries have challenged the historical practice of performing revision amputation for all complete finger avulsion injuries. The aim of this study is to conduct a systematic review of the English literature of replantation of finger avulsion injuries to provide best evidence of survival rates and functional outcomes. METHODS: A Medline literature search yielded 1,398 studies, using key words "traumatic amputation" or "replantation", with limitation to humans and finger injuries. Inclusion criteria re

Robotic Rehabilitation Using the Hybrid Assistive Limb for Drop Fingers in a Patient with Cervical Spondylotic Radiculopathy: A Case Report.

Background: Drop finger may occur in patients with C7 and/or C8 cervical radiculopathy caused by cervical spondylosis. Although surgical decompression of the affected nerve roots is performed in patients with drop finger refractory to conservative treatment, postoperative recovery of drop finger is often unsatisfactory. Furthermore, no effective rehabilitation strategy for improving drop finger has yet been established. Methods: Here, we report a patient with drop finger who underwent a novel postoperative rehabilitation program. A 64-year-old man presented with drop finger of the left hand ca

Transverse bayonet dislocation of the proximal interphalangeal joint.

Complete dislocation of the proximal interphalangeal joint with complete rupture of both the collateral ligaments and volar plate was seen in 8 patients. We call this, "transverse bayonet dislocation of the proximal interphalangeal joint." When treated early, closed manipulation resulted in stable reduction. After a brief period of immobilization of 3 to 5 days with a dorsal aluminum splint, sustained active range of motion exercises were begun by strapping the injured finger to the adjacent finger obtaining good end results in all cases. An untreated dislocation of 5 weeks duration needed ope

Rotational stability test for the diagnosis of radial collateral ligament rupture in the fingers: Anatomical study.

Diagnosing rupture of the radial collateral ligament (RCL) of the finger metacarpophalangeal (MCP) joints is difficult. The aim of this cadaver study was to validate a rotational test for the MCP after RCL transection. With the MCP and proximal interphalangeal joints in flexion, rotation along the axis of the proximal phalanx was applied through an extended distal interphalangeal joint to 36 cadaver fingers. Each finger's pulp described an arc of pronation and supination that was noted on the palm. The test was repeated three times: before transection, after transection of the proper collatera

Therapy for Dupuytren's Disease: Collagenase Therapy-A Long-Term Follow-Up Study.

BACKGROUND: Dupuytren's disease (DD) is a systemic connective tissue disorder of the palm. It particularly affects men of Northern European or Caucasian origin over the age of 55. In addition to the classical surgical therapy via limited fasciectomy, Dupuytren's contracture can also be treated minimally invasively. A relatively new treatment method is the use of collagenase injections (Xiapex) to reduce the contracture of the fingers. The data regarding the long-term success of this therapy are currently limited. METHODS: In this monocentric retrospective study, we examined 35 patients who we

Contralateral transplantation of a finger for restoration of thumb function.

Transplantation of a finger from the contralateral hand for thumb reconstruction is seldom done because of possible psychological problems for the patient. We present two cases in which a previously damaged index finger of the contralateral hand was transplanted. In both patients the metacarpophalangeal joint of the index finger replaced that of the thumb. A powerful pinch to the fingers was achieved and the appearance of both the donor and the recipient hands was considerably improved.

Fibrocartilages in the extensor tendons of the interphalangeal joints of human toes.

The extensor tendons of the fingers and toes form part of the capsule of the interphalangeal joint and press against the proximal phalanx during flexion. Previous work on the fingers has shown that there is a "sesamoid" fibrocartilage on the deep surface of each tendon that labels immunohistochemically for a variety of glycosaminoglycans and collagens. However, we know little about the molecular composition of the tendon in the toes. This question is of special interest, because the mechanics of the interphalangeal joints differ in the upper and lower limbs-the toes balance the forefoot, distr

Wrist and Finger Torque Sensor for the quantification of upper limb motor impairments following brain injury.

This paper details the design of the Wrist and Finger Torque Sensing module (WFTS): a lightweight, portable device that measures isometric wrist and finger flexion and extension joint torques. The WFTS can be used in combination with rehabilitation robots such as the ACT-3D, with isometric measurement stations, or as a stand-alone device. Because many robotic devices are limited in that they involve the hand in isolation, the WFTS is designed to investigate abnormal joint torque coupling at the paretic wrist and fingers in individuals with adult-onset stroke or childhood hemiplegia during 3D a

Does Inclination of the Toe Proximal Phalangeal Head Affect the Outcomes of Vascularized Joint Transfers? Retrospective Study of 22 Joint Transfers.

BACKGROUND: Different shapes of the proximal phalangeal head of toe proximal interphalangeal joints (e.g., oval and circular) are observed in vascularized joint transfers. The difference in shape implies the varying degrees of inclination of the articular surfaces between toes. This study investigated the impact of articular inclination on outcomes after toe joint transfers for finger proximal interphalangeal joint reconstruction. METHODS: Twenty-one patients who underwent vascularized joint transfer from May of 2009 to May of 2018 were included. Their mean age was 33.4 years and mean follow-

Risk Factors for Long-Term Contracture Recurrence after Collagenase Injection for Dupuytren Disease: A Prospective Cohort Study.

In Dupuytren disease, little is known about the long-term outcomes of collagenase injection or risk factors for contracture recurrence. In this prospective study, 159 patients (242 fingers) with Dupuytren disease and active extension deficit (AED) ≥20° in a metacarpophalangeal (MCP) or proximal interphalangeal (PIP) joint were treated with collagenase injection during a 14-month period. At 5 years, 18 patients were deceased, 2 could not be contacted, and 13 had undergone fasciectomy. The remaining 126 patients (199 treated fingers) participated in a follow-up evaluation at 52-96 (mean 65) mont

Distal interphalangeal joint arthrodesis using nitinol intramedullary fixation implants: X-fuse implants for DIP arthrodesis.

Arthrodesis has been shown to be a successful operative procedure for the treatment of intractable pain, instability, and deformity of the distal interphalangeal joint of the fingers, and interphalangeal joint of the thumb. Multiple fixation techniques have been used in the past including Kirschner wires, tension bands, headless screws, or lag screws. Clinical results are generally acceptable, but complication rates have been reported to vary between 10% and 20%. Complications include nonunion, pain, malunion, infection, nerve injury, and protruding hardware. The aim of this study is to descri

A tendon sheath tumour presenting as trigger finger.

The symptom of trigger finger may be caused by a variety of lesions. This report describes a trigger finger caused by a tumour in the tendon sheath. Trigger finger refers to transient arrest of motion in a finger. It may be caused by a variety of lesions such as those of the volar plate (Yancey & Howard, 1962) and joint capsule (Aldred, 1954), structural abnormalities of the metacarpal head (Flatt, 1961), lacerations of the flexor tendons (Janecki, 1976) and 'loose bodies' (Milford, 1971). Triggering as a result of tumour of the tendon sheath does not, however, appear to have been reported.

Three-Chamber Actuated Humanoid Joint-Inspired Soft Gripper: Design, Modeling, and Experimental Validation.

To address the limitations of single-chamber soft grippers, such as constant curvature, insufficient motion flexibility, and restricted fingertip movement, this study proposes a soft gripper inspired by the structure of the human hand. The designed soft gripper consists of three fingers, each comprising three soft joints and four phalanges. The air chambers in each joint are independently actuated, enabling flexible grasping by adjusting the joint air pressure. The constraint layer is composed of a composite material with a mass ratio of 5:1:0.75 of PDMS base, PDMS curing agent, and PTFE, whic

Kinematic performance of a six degree-of-freedom hand model (6DHand) for use in occupational biomechanics.

Upper extremity musculoskeletal disorders represent an important health issue across all industry sectors; as such, the need exists to develop models of the hand that provide comprehensive biomechanics during occupational tasks. Previous optical motion capture studies used a single marker on the dorsal aspect of finger joints, allowing calculation of one and two degree-of-freedom (DOF) joint angles; additional algorithms were needed to define joint centers and the palmar surface of fingers. We developed a 6DOF model (6DHand) to obtain unconstrained kinematics of finger segments, modeled as fru…
