Toenail onychomycosis is a common disease with limited treatment options, as treatment failures and relapses frequently are encountered. Many patients experience long-term disease that affects multiple toenails and causes substantial discomfort and pain. Although many patients prefer topical therapies, treatment efficacy with ciclopirox and amorolfine lacquers has been disappointing. Efinaconazole solution 10% is a new triazole antifungal agent specifically developed for the treatment of onychomycosis. Efinaconazole has shown a broad spectrum of antifungal activity in vitro and is more potent

A combination of topical and oral antifungals is widely used to treat onychomycosis, but treatment failure is common and oral drugs may cause toxicity and potential drug interactions. For this reason, new approaches and strategies should be considered. The following case shows that photodynamic therapy (PDT) may represent an alternative noninvasive approach to treatment of onychomycosis.

Trichophyton raubitschekii was originally described as a distinct dermatophyte species but is now classified as a variant of Trichophyton rubrum. The variant raubitschekii differs from common strains of T. rubrum with regard to morphology, physiology, epidemiology and patterns of infection. It is mostly found in Africa, Asia and South America, but the literature and our observations reveal that seven patients (four men, three women) with T. rubrum var. raubitschekii-infections were meanwhile seen in Germany. All of them were born in Africa and they suffered from tinea corporis or tinea pedum,

BACKGROUND: The Weil osteotomy has been reported to be a clinically effective treatment of metatarsalgia and intractable plantar keratosis. The plantar inclination of the metatarsal influences the effect of the osteotomy but has never been studied in detail.
METHODS: This study examined five fresh or fresh-frozen cadaver specimens. The data obtained from the specimens was used to model the geometry of the Weil osteotomy. The effect of thick saw blades on the Weil osteotomy was determined.
RESULTS: The inclination of the second metatarsal averaged 24.6 (range 19 to 31) degrees. The displaceme

An 80-year-old woman was referred to our hospital with a spontaneously appearing bullous dermatosis limited to the soles of both feet, causing an intense pruritus. She also suffered from Parkinson's disease and depression, and had been treated with levodopa, benserazide, and mirtazapine for several years. On clinical examination, we found several tense and hemorrhagic bullae, with a diameter of up to 3 cm, at both plantar sites and multiple, confluent, dyshidrotic vesicles ( Figs 1 and 2). The rest of the skin, including the mucous membranes and palms, was normal. The first clinical diagnosis

Three patients with onychomycosis due to Trichophyton soudanense with an "endonyx" type of nail invasion are presented. Endonyx onychomycosis is clinically characterized by a diffuse milky-white discolouration of the affected nail, in the absence of nail bed hyperkeratosis or onycholysis. Nail plate surface and nail thickness are normal. The pathology shows a contrast between the great number of fungal hyphae visible in the nail plate and the absence of fungal elements in the nail bed. The nail bed does not show inflammatory changes or hyperkeratosis and adheres strictly to the lowermost layer

INTRODUCTION: Plantar hidradenitis, one of several possible causes of painful papulo-nodular lesions of the foot in children and young adults, was recently described as the presence of characteristic lesions of the eccrine sweat glands.
CASE REPORT: A 15-year-old boy consulted for a sudden-onset painful eruption on the sole of the right foot. Physical examination revealed papulo-nodular erythromato-violet infiltrated lesions located on the anterior part of the right planta. Histology examination showed dense neutrophil inflammatory infiltration predominating at the dermo-hypodermic junction a

A 29-year-old Thai woman had draining sinus tracts, tumefaction, and granules on the plantar aspect of the foot. Phialophora verrucosa was isolated from the lesion. P. verrucosa is a major agent of chromoblastomycosis, which is known to rarely cause subcutaneous phaeohyphomycosis. This dematiaceous fungus has not been previously reported to cause mycetoma. This case illustrates the clinical spectrum of disease of this fungus. The salient features of mycetoma and management options are presented.

Ten patients who presented with nonmosaic plantar warts were treated with a new polymeric matrix patch delivery system containing salicylic acid USP, 21 percent. Clinicians then assessed the potential value of this patch system in terms of efficacy, length of treatment time required, patient compliance, and tolerance.

Based on a case of our own observation, we present the characteristic features of a rare skin disease: hyperkeratosis follicularis et parafollicularis in cutem penetrans, i.e. Kyrle's disease. The clinical picture shows follicular and interfollicular hyperkeratotic horn-like plugs with a typical distribution pattern. The classical histological features are epidermolysis and hyperkeratoses advancing towards the dermis. Today, this disease is regarded as a genodermatosis with recessive heredity and is best treated with systemic and topical application, resp., of vitamin A acid.
