Bone Fractures Numerical Analysis in a Femur Affected by Osteogenesis Imperfecta.

This work presents a non-invasive methodology to obtain a three-dimensional femur model of three-year-old infants affected with Osteogenesis Imperfecta (OI) type III. DICOM® Files of a femur were processed to obtain a finite element model to assess the transverse, the oblique, and the comminuted fractures. The model is evaluated under a normal walking cycle. The loads applied were considered the most critical force generated on the normal walking cycle, and the analyses considered anisotropic bone conditions. The outcome shows stress concentration areas in the central zone of the diaphysis of

Distal clavicle fractures with "superior intact cortical bone": minimally invasive surgery.

INTRODUCTION: Distal clavicle fractures represent 12%-26% of all clavicle fractures. For unstable cases, surgical fixation is the preferred method of treatment. To date, there is still controversy regarding the best fixation method with a high reoperation and complication rate reported. The purpose of this article is to describe a minimally invasive method for reduction and stabilization of displaced distal clavicle fractures, using cortical buttons. SURGICAL TECHNIQUE: After standard preoperative preparation, a 3-cm incision is made at the coracoclavicular area. Using both coracoid and clavi

Contribution of bone mineral density in stress fractures of elite athletes.

INTRODUCTION: The stress fracture occurs in a healthy bone with a lack of adaptation to sustained, intensive and repetitive physical activity. However, bone mineral density is strongly correlated with bone strength and fractures may occur more easily on osteoporotic bone. AIM: To evaluate the bone densitometric profile of elite athletes with a history of stress fracture and to determine the interest in preventing this type of fracture. METHODS: cross-sectional study comparing a group of elite athletes to a control group matched by sex, age and sports discipline. Bone mineral density is measu

Diagnostic performance of brain computed tomography to detect facial bone fractures.

OBJECTIVE: Most patients with head trauma require brain computed tomography (CT) in the emergency department. However, the requirement for facial CT remains controversial. The aim of this study was to evaluate the diagnostic value of brain CT alone for detection of facial fractures and its ability to determine the requirement for additional facial CT. METHODS: This retrospective multicenter study was conducted in two tertiary hospitals in Seoul, Republic of Korea, between January 2014 and December 2015. Data were collected from the medical records of adult patients (aged over 18 years) who ha

Factors that predict instability in pediatric diaphyseal both-bone forearm fractures.

The aim of this study was to determine the factors that may predict failure of closed reduction and casting of diaphyseal forearm fractures in children. Demographic and radiographic data of children with closed reduction and casting of these fractures in the emergency department were evaluated. Of 174 patients with adequate follow-up to union, 19 (11%) required a repeat procedure. Risk factors for repeat reduction included translation of 50% or more in any plane, age more than 9 years, complete fracture of the radius, and follow-up angulation of the radius more than 15° on lateral radiographs

[Fragility Fractures in Hemodialysis Patients. Uremic toxins-related abnormal bone metabolism.].

Uremic toxins are increased with deterioration of kidney function and the levels are associated with progression of uremic syndrome. Uremic toxins are associated with various chronic kidney disease(CKD)-related systemic disease, and bone is one of major targets. Recent clinical and basic studies show that protein-bound uremic toxins, such as indoxyl sulfate(IS)and p-crecyl sulfate(PCS), parathyroid hormone, and renin-angiotensin-aldosterone system(RAAS)are associated with bone fragilities. IS and PCS induce skeletal resistance to PTH in osteoblasts and osteoclasts as well as impairment of bone

Treatment of Long Bone Fractures in Calves with Ilizarov External Fixator.

In this study, the advantages and disadvantages of Ilizarov external fixator in the treatment of long bone fractures in calves were investigated. For this purpose, 26 calves of different breeds, ages and genders, having metacarpal (n = 12), metatarsal (n = 5) and antebrachial (n = 9) fractures underwent surgery under general anaesthesia. Frames, which were prepared before surgery in accordance with the radiographic findings, were applied under open reduction in 18 cases, and under closed reduction in eight cases. In the postoperative period, all calves except one started to bear some weigh on

Intramedullary Nailing System in Unstable Both Bone Diaphyseal Fractures in Children.

Background Forearm fractures in pediatric population is usually managed conservatively. Unstable fractures need operative intervention like closed or open intramedullary nailing or open reduction and internal fixation with plates and screws. However, there is no consensus regarding the method of treatment according to age group. Objective To standardize the use of elastic nailing system as effective form of treatment in terms of function, cost and lower complications as compared to widely used titanium nails in developed countries.Method Sixty eight pediatric patients with both bone forearm di

Inflammatory Microenvironment Persists After Bone Healing in Intra-articular Ankle Fractures.

BACKGROUND: Post-traumatic osteoarthritis (PTOA) is responsible for the majority of cases of ankle arthritis. While acute and end-stage intra-articular inflammation has previously been described, the state of the joint between fracture healing and end-stage PTOA remains undefined. This study characterized synovial fluid (SF) composition of ankles after bone healing of an intra-articular fracture to identify factors that may contribute to the development of PTOA. METHODS: Of an original 21 patients whose SF was characterized acutely following intra-articular ankle fractures, 7 returned for pla

Steroidal contraceptives and bone fractures in women: evidence from observational studies.

BACKGROUND: Age-related decline in bone mass increases the risk of skeletal fractures, especially those of the hip, spine, and wrist. Steroidal contraceptives have been associated with changes in bone mineral density in women. Whether such changes affect the risk of fractures later in life is unclear. Hormonal contraceptives are among the most effective and most widely-used contraceptives. Concern about fractures may limit the use of these effective contraceptives. Observational studies can collect data on premenopausal contraceptive use as well as fracture incidence later in life. OBJECTIVES

Comparison of MRI, CT and bone scintigraphy for suspected scaphoid fractures.

PURPOSE: The best diagnostic modality for confirmation of the diagnosis of a scaphoid fracture that is not visible on the initial radiograph (occult scaphoid fracture) is still subject of debate. The aim of this study was to compare the accuracy of magnetic resonance imaging (MRI), computed tomography (CT) and bone scintigraphy (BS) for the diagnosis of these occult scaphoid fractures. PATIENTS AND METHODS: In a study period of 12 months, 33 consecutive patients with a clinically suspected scaphoid fracture without a fracture on the scaphoid radiographs were evaluated with MRI, CT and BS. Abdominal solid organ injury in trauma patients with pelvic bone fractures.

BACKGROUND: We analyzed the clinical progression of trauma patients with pelvic bone fractures so to determine the risk factors associated with sustaining concurrent abdominal solid organ injuries. METHODS: This study was a retrospective chart review. Subjects were categorized based on injury type: solid organ versus non-solid organ injury groups. These study groups were compared based on demographics, treatments, and clinical outcomes. Potential risk factors that may contribute to the occurrence of abdominal solid organ injury in trauma patients with pelvic bone fractures were evaluated. RE

Osteoblast-derived WNT16 represses osteoclastogenesis and prevents cortical bone fragility fractures.

The WNT16 locus is a major determinant of cortical bone thickness and nonvertebral fracture risk in humans. The disability, mortality and costs caused by osteoporosis-induced nonvertebral fractures are enormous. We demonstrate here that Wnt16-deficient mice develop spontaneous fractures as a result of low cortical thickness and high cortical porosity. In contrast, trabecular bone volume is not altered in these mice. Mechanistic studies revealed that WNT16 is osteoblast derived and inhibits human and mouse osteoclastogenesis both directly by acting on osteoclast progenitors and indirectly by in

A systematic review of bone mineral density and fractures in phenylketonuria.

INTRODUCTION: Our objective was to systematically review and analyze published data on bone mineral density (BMD) and fracture rates in patients with phenylketonuria (PKU), and relationships between BMD and phenylalanine levels. METHODOLOGY: We searched PubMed, CINAHL, and Cochrane databases from January 1966 to November 2013 for studies of spine BMD or fracture in PKU and control subjects. We excluded studies assessing skeletal health by ultrasound or peripheral quantitative computer tomography. Both authors reviewed abstracts for inclusion, and read full text papers to extract data. Computed tomographic findings of cerebral fat embolism following multiple bone fractures.

Fat embolism to the lungs and brain is an uncommon complication following fractures. Few reports with descriptions of computed tomographic (CT) findings of emboli to the brain or cerebral fat embolism are available. We report a case of cerebral fat embolism following multiple skeletal fractures and present its CT findings here.

Intercalated bone PEG in the treatment of non-united scaphoid fractures.

INTRODUCTION: Untreated ununited scaphoid fractures will almost inevitably progress to radiographic and symptomatic osteoarthritis of the wrist. This may lead to subsequent morbidity and lifelong disability especially in young males in which the fracture scaphoid is more common. PATIENTS AND METHODS: Twenty-one patients presenting non-united fracture scaphoid were operated upon by using disto-proximal bone peg technique with average time between injury and operation as 11 months (6-18 months). All of them male with average age of 26 years (17-35 years). There were 17 patients with fracture wa

Tympanic plate fractures in temporal bone trauma: prevalence and associated injuries.

BACKGROUND AND PURPOSE: The prevalence of tympanic plate fractures, which are associated with an increased risk of external auditory canal stenosis following temporal bone trauma, is unknown. A review of posttraumatic high-resolution CT temporal bone examinations was performed to determine the prevalence of tympanic plate fractures and to identify any associated temporal bone injuries. MATERIALS AND METHODS: A retrospective review was performed to evaluate patients with head trauma who underwent emergent high-resolution CT examinations of the temporal bone from July 2006 to March 2012. An unusual case of hypercortisolism with multiple weight-bearing bone fractures.

Glucocorticoid excess, either from exogenous exposure or through endogenous overproduction, is a common cause of secondary osteoporosis. We report a 52-year-old woman presenting with multiple stress fractures of the lower extremities, despite various osteoporosis therapeutic regimens. Investigations led to the diagnosis of hypercortisolism of pituitary origin. Pituitary surgery was unsuccessful, justifying a treatment of ketoconazole. In the absence of densitometric osteoporosis, assessment of bone microstructure using high resolution peripheral quantitative computed tomography revealed altera
