[Ultrasonography of the nose and paranasal sinuses].

UNLABELLED: Pathological changes of the sinuses and the nasal bones are rarely examined sonographically. However, both acute and chronic sinusitis in maxillary and frontal sinuses and fractures of the nose could all be diagnosed correctly by diagnostic A-mode ultrasound, which is of clinical importance if children or pregnant patients are examined. The aim of the study was to analyse the reliability of A-mode ultrasonography in diagnosing sinusitis and nasal bone fractures, compared to results of CT, X-ray and clinical symptoms. MATERIAL AND METHODS: 86 patients were included in the study: 20

Diagnosis and treatment of sinusitis.

Maxillary and ethmoid sinusitis may occur at any age. Frontal sinus infections first appear in adolescence. The bacteria that cause sinusitis are similar to those associated with otitis media. Streptococcus pneumoniae and Hemophilus influenzae are recovered from 50 to 70 percent of isolates. Anaerobes are also important pathogens in adults. Fungal sinusitis usually occurs only in diabetics or immunosuppressed individuals.

Outcomes in Surgical Treatment of Mucocele in Frontal Sinus.

The aim of the study was to correlate several studies dating from 1997 to 2015 to identify the most effective treatments for mucocele in the frontal sinus (with/without other paranasal sinuses), considering successful outcomes and recurrence. We aimed to conduct a literature review for articles published between 1997 and 2015. For this, we accessed articles in the SciELO database, as well as LILACS, PubMed, and Google Scholar databases. Were identified 32 work-related injuries in the paranasal sinuses; 2 of these were not related to mucoceles or mucopyocele, 4 had no relation to the frontal si

Medical management versus surgical intervention of pediatric orbital cellulitis: the importance of subperiosteal abscess volume as a new criterion.

PURPOSE: To investigate age and frontal sinusitis as indications for the surgical management of pediatric orbital cellulitis with subperiosteal abscess (SPA) and to create an SPA volume criterion that would favor nonsurgical management. METHODS: A retrospective chart review was performed to find all patients age 18 years and younger who presented to Hasbro Children's Hospital with orbital cellulitis secondary to sinusitis with an SPA from 2005 to 2009. SPA volume was measured using a CT ruler at the largest axial, coronal, and sagittal dimensions. Student t testing was used for statistical an…

Monostotic fronto-orbital fibrous dysplasia with convulsion--case report.

A 28-year-old man presented with monostotic fronto-orbital fibrous dysplasia associated with convulsions. Signs of meningeal irritation were observed. Computed tomography (CT) showed right frontal sinusitis, and destruction from the inner to outer table with expansion of the diploic space. T1- and T2-weighted magnetic resonance imaging showed an abnormal low-intensity mass, with heterogeneous gadolinium enhancement. Although the meningitis resolved, signs of infection continued for 2 months due to sinusitis. Treatment of the right frontal sinusitis was undertaken, accompanied by open biopsy. T…

Fever, frontal sinus mass, and CSF pleocytosis in a 44-year-old man.

Intracranial complications arising from frontal sinusitis occur infrequently. However, they can progress with such rapidity that the clinical situation becomes far advanced before they are recognized. Radiographic imaging techniques may not be definitive early in the course of these complications. The infectious disease service was asked to evaluate a middle-aged man with acute global headache and nasal discharge for two weeks. CSF pleocytosis (3,600 WBC/mm3) was documented on lumbar puncture, and a dense mass was noted on sinus radiographs. At surgery, a large bony lesion was found extending …

Clinical and Radiological Significance of Anatomical Variations in Paranasal Sinuses: A Retrospective CT-Based Study.

Background and objective Anatomical variations of the paranasal sinuses influence sinus drainage and airflow, contributing to chronic rhinosinusitis. This study aimed to assess the prevalence of these variations and their association with sinus infections by using CT findings. Methods A retrospective CT-based analysis of 75 patients (aged 25-70 years) was conducted. Anatomical variations, including nasal septal deviation (NSD), concha bullosa, agger nasi cells, Haller cells, and Onodi cells, were recorded. The association between these variations and affected sinuses was analyzed using the chi

Cadaveric validation study of computational fluid dynamics model of sinus irrigations before and after sinus surgery.

BACKGROUND: Investigations into the distribution of sinus irrigations have been limited by labor-intensive methodologies that do not capture the full dynamics of irrigation flow. The purpose of this study was to validate the accuracy of a computational fluid dynamics (CFD) model for sinonasal irrigations through a cadaveric experiment. METHODS: Endoscopic sinus surgery was performed on 2 fresh cadavers to open all 8 sinuses, including a Draf III procedure for cadaver 1, and Draf IIb frontal sinusotomies for cadaver 2. Computed tomography maxillofacial scans were obtained preoperatively and po

Isolated oculomotor nerve palsy secondary to acute sinusitis.

Sinusitis is a common condition, but only very rarely accompanied by isolated cranial nerve palsies. We describe a case of a 64-year-old male with a two-day history of left-sided ptosis associated with one week of nasal congestion and frontal sinus pain. Examination revealed ptosis with left pupil mydriasis. Uncontrasted computed tomography and angiography of the head demonstrated neither intracranial vascular abnormalities nor acute lesions; however, it did show mucosal thickening in the left frontal sinus, ethmoid air cells and left maxillary sinus, indicating potential obstruction of the le

Cholesterol granuloma of the frontal sinus: a case report.

Cholesterol granulomas are common in the mastoid antrum and air cells of the temporal bone. In the paranasal sinuses, especially in the frontal sinus, they have occasionally been mentioned in the literature. The pathogenesis is unknown, but the majority of the authors support the concept of airway obstruction in the cells well pneumatised of temporal bone and paranasal sinuses. The authors report a case of cholesterol granuloma of the frontal sinus treated with radical surgical techniques, and they also recommend an endoscopic approach to frontal sinus to restore or enlarge the nose-frontal ca

Use of balloon catheter dilation and steroid-eluting stent in light and severe rhinosinusitis of frontal sinus: a multicenter retrospective randomized study.

OBJECTIVE: Frontal sinus surgery has an increased rate of re-stenosis, if compared to other sinuses. It depends mainly on recurrent inflammation and abnormal scarring at the frontal recess and its reduction represents one of the keys to therapeutic success. Balloon catheter dilation (BCD) and implantable sinus stents/spacers represent strategies to improve sinus ventilation respecting the integrity of mucosa and reducing abnormal post-surgical scarring. The purpose of this study was to evaluate the effectiveness, safety and correct indication about the use of BCD and a non-absorbable stent (Re

Superficial surgical landmarks for the frontal sinus.

OBJECT: There is a lack of reports in the literature that contain descriptions of superficial anatomical landmarks for the identification of the internally located frontal sinus. Neurosurgeons must often enter the cranium through the frontal bone and knowledge of the frontal sinus is essential to minimize complications. METHODS: Seventy adult cadaveric frontal sinuses were evaluated. Measurements included both the lateral and superior extent of the frontal sinus in reference to a midpupillary line, and the superior extent of the frontal sinus from the nasion. Frontal sinuses were found bilate

Radiologic anatomy of the paranasal sinuses in the child.

The development of the paranasal or accessory sinuses begins very early in utero. In the newborn the ethmoidal sinus, which gives rise to all the other sinuses, as well as the maxillary sinus, can already be identified on x-rays. The frontal sinus appears as a triangle at the age of four and oversteps the supraorbital margin at the age of six. The sphenoidal sinus begins to excavate the concha sphenoidalis at the age of four and can be seen on x-rays at the age of eight, when it extends to the hypophyseal fossa. In the twelve-year-old child all sinuses almost reach their final sizes. However,

Osteoblastoma originating from frontoethmoidal sinus causing personality disorders and superior gaze palsy.

Osteoblastoma is a rare, solitary benign tumor that is usually situated in axial skeleton mainly in vertebra. It is rarely seen in ethmoid and frontal sinuses. A 40-year-old man who had osteoblastoma originated from frontal and ethmoidal sinuses that extends up to frontal lobe and gave rise to personality disorders by compressing the frontal lobe, and caused superior gaze palsy by compressing the superior rectus muscle. We present this rare case with clinical, radiological and histopathological findings.
