Understanding HIV care delays in the US South and the role of the social-level in HIV care engagement/retention: a qualitative study.

INTRODUCTION: In a significant geographical shift in the distribution of HIV infection, the US South--comprising 17 states--now has the greatest number of adults and adolescents with HIV (PLHIV) in the nation. More than 60% of PLHIV are not in HIV care in Alabama and Mississippi, contrasted with a national figure of 25%. Poorer HIV outcomes raise concerns about HIV-related inequities for southern PLHIV, which warrant further study. This qualitative study sought to understand experiences of low-income PLHIV on the AIDS Drug Assistance Program in engagement and retention in continuous HIV care i

The pathological gambler as criminal offender. Comments on evaluation and treatment.

Over the past three decades, gambling has been the nation's fastest growing industry. Although there is now some leveling off, states are still turning to legalized gambling to address financial problems without having to raise taxes. In addition, there is new technology that produces more rapidly addicting games. States are accepting some responsibility and, as of this writing, 12 of them have funded some programs in public education, research, training, and treatment. Although there are only a half dozen inpatient programs and very few qualified counselors and therapists, we can anticipate t

Neuro-symbolic LLM Integration in Clinical Medicine: A Systematic Review and Taxonomy.

BACKGROUND: LLMs are promising for clinical workflows but hallucinations limit deployment. Neuro-symbolic systems pair LLMs with explicit rules, ontologies, or knowledge graphs to constrain outputs, yet integration patterns are described inconsistently and their deployment trade-offs remain unclear. METHODS: We conducted a PRISMA 2020 systematic review (PROSPERO CRD420261296004) of peer-reviewed studies (PubMed/MEDLINE, CENTRAL, Web of Science, Scopus; English; January 1, 2022 to January 30, 2026) and assessed risk of bias using PROBAST adapted to neuro-symbolic evaluations. RESULTS: We iden

Leptomeningeal and Ependymal Seeding of a High-Grade Glioma Following Ventricular Opening and Carmustine Wafer Implantation: A Case Report Highlighting a Diagnostic Dilemma.

Leptomeningeal and ependymal seeding (LES) is an uncommon but devastating complication of high-grade gliomas, and its incidence appears to be rising as improved multimodal treatment prolongs patient survival. Intraoperative ventricular entry and repeated surgery are well-established risk factors, and their coexistence with carmustine wafer implantation raises technical and diagnostic questions that remain underexplored in the literature. We report the case of a 37-year-old man with a right parieto-occipital WHO grade III anaplastic astrocytoma, initially treated with gross total resection and

Family-Centred Care for Children and Adolescents Living With HIV in Sub-Saharan Africa: A Scoping Review.

INTRODUCTION: Children and adolescents living with HIV in sub-Saharan Africa (SSA) continue to face significant barriers to diagnosis, treatment adherence, retention in care and psychosocial support, despite progress in prevention and treatment coverage. Family-centred care (FCC) has emerged as a promising model that positions families-not just individual children-as the central unit of care, with potential to improve clinical and psychosocial outcomes while addressing persistent gaps in health system responsiveness. METHODS: Our scoping review synthesized evidence on FCC implementation strat

Structural barriers to medically indicated abortion in Germany: A qualitative study of provider perspectives.

BACKGROUND: In Germany, to perform an abortion after 14 weeks of gestation, physicians are legally required to provide a medical indication due to a serious risk to the pregnant person’s life or health. This study explores legal, institutional, and societal factors influencing abortion care in Germany from the perspective of abortion providers and counsellors. METHODS: We conducted 42 semi-structured expert interviews with gynaecologists and abortion counsellors across Germany. Data were analysed using Mayring’s qualitative content analysis with a mixed inductive–deductive approach. RESULTS: I

Balanced Area Deprivation Index (bADI): Enhancing Social Determinants of Health Indices to Strengthen Their Association With Healthcare Clinical Outcomes, Utilization, and Costs.

BACKGROUND: With the rapid expansion of value-based care in the US health-care industry, addressing health disparities has become a central focus for stakeholders. Social determinants of health (SDoH) indices serve as critical tools for assessing neighborhood inequities and informing programs aimed at managing patient health and costs. The Area Deprivation Index (ADI), recently endorsed by the US Department of Health & Human Services, is widely used in these efforts. However, the ADI's reliance on housing-related variables, such as median home value, raises concerns about its applicability, pa

Stigmatizing attitudes toward people with migraine by people without active migraine: results of the OVERCOME (US) study.

BACKGROUND: Migraine is associated with various types of stigma. This study aims to evaluate stigmatizing attitudes towards people with migraine by people without active migraine. METHODS: OVERCOME (US) was a web survey among adults from a representative US population-based sample that collected information about stigmatizing attitudes of people without active migraine (i.e., no migraine/severe headaches in the previous 12 months) toward people with migraine and related conditions. The following associations were examined in the current analysis: (1) association between stigma toward migraine

Increase of invasive pneumococcal disease in children temporally associated with RSV outbreak in Quebec: a time-series analysis.

BACKGROUND: Respiratory viruses have been previously suspected to trigger invasive pneumococcal disease (IPD). After progressive non-pharmaceutical interventions (NPI) lifting, an unusual RSV outbreak has been observed in the Fall 2021, raising concerns about the possible consequences on IPD. We aimed to analyse the evolution of IPD incidence across age-groups since NPI lifting, and its temporal association with respiratory viral infections. METHODS: We conducted a time-series analysis using 1) population-based IPD surveillance data and 2) statistics from the laboratory surveillance network o

Incarceration, menstruation and COVID-19: a viewpoint of the exacerbated inequalities and health disparities in South African correctional facilities.

PURPOSE: The menstrual health and menstrual hygiene management (MHM) of incarcerated women remains relatively low on the agenda of public health interventions globally, widening the inequitable access of incarcerated women to safe and readily available menstrual health products (MHP). The COVID-19 pandemic has adversely impacted on the MHM gains made in various development sectors in the global North and South, through its amplification of vulnerability for already at-risk populations. This is especially significant to developing countries such as South Africa where the incarcerated female pop

The effects of radiofrequency electromagnetic fields exposure on tinnitus, migraine and non-specific symptoms in the general and working population: A protocol for a systematic review on human observational studies.

BACKGROUND: Applications emitting radiofrequency electromagnetic fields (RF-EMF; 100 kHz to 300 GHz) are widely used for communication (e.g. mobile phones), in medicine (diathermy) and in industry (RF heaters). Concern has been raised that RF-EMF exposure affects health related quality of life, because a part of the population reports to experience a variety of symptoms related to low exposure levels below regulatory limits. OBJECTIVES: To systematically review the effects of longer-term or repeated local and whole human body RF-EMF exposure on the occurrence of symptoms evaluating migraine,

Updated View on Kidney Transplant from HCV-Infected Donors and DAAs.

BACKGROUND: The discrepancy between the number of potential available kidneys and the number of patients listed for kidney transplant continues to widen all over the world. The transplant of kidneys from hepatitis C virus (HCV)-infected donors into HCV naïve recipients has grown recently because of persistent kidney shortage and the availability of direct-acting antiviral agents. This strategy has the potential to reduce both waiting times for transplant and the risk of mortality in dialysis. AIM: We made an extensive review of the scientific literature in order to review the efficacy and saf

SARS-CoV-2 antibody-positivity protects against reinfection for at least seven months with 95% efficacy.

BACKGROUND: Reinfection with the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been documented, raising public health concerns. SARS-CoV-2 reinfections were assessed in a cohort of antibody-positive persons in Qatar. METHODS: All SARS-CoV-2 antibody-positive persons from April 16 to December 31, 2020 with a PCR-positive swab ≥14 days after the first-positive antibody test were investigated for evidence of reinfection. Viral genome sequencing was conducted for paired viral specimens to confirm reinfection. Incidence of reinfection was compared to incidence of infection in th

Challenges of transferring rural adults with chronic HCV infection for further HCV RNA confirmation and free DAAs treatment: a success story of the interdisciplinary collaboration approach.

BACKGROUND: Chronic hepatitis C virus (HCV), which is a concern in many countries, is the leading cause of liver cancer around the world. Since Taiwan launched its national health insurance system in 1995, it has managed to extend health coverage to 99% of the Taiwanese population, providing free but limited antiviral treatment each year since 2017. However, many people in rural areas are unaware that they have chronic HCV; nor do they realize that new drugs with high cure rates could drastically reduce their health burden. The aim of this study is to explore the implementation facilitators of

A national public health programme on gambling policy development in New Zealand: insights from a process evaluation.

BACKGROUND: In New Zealand, a public health programme on gambling policy development is part of a national gambling harm reduction and prevention strategy mandated by the Gambling Act 2003. Funded by the Ministry of Health, the programme directs workplace/organisational gambling policies, non-gambling fundraising policies, and local council policies on electronic gaming machines (EGMs). We carried out a process evaluation of this programme to identify practical information (e.g. advocacy approaches; challenges and ameliorating strategies) that can be used by programme planners and implementers

The Good School Toolkit for reducing physical violence from school staff to primary school students: a cluster-randomised controlled trial in Uganda.

BACKGROUND: Violence against children from school staff is widespread in various settings, but few interventions address this. We tested whether the Good School Toolkit-a complex behavioural intervention designed by Ugandan not-for-profit organisation Raising Voices-could reduce physical violence from school staff to Ugandan primary school children. METHODS: We randomly selected 42 primary schools (clusters) from 151 schools in Luwero District, Uganda, with more than 40 primary 5 students and no existing governance interventions. All schools agreed to be enrolled. All students in primary 5, 6

Early versus delayed treatment of relapsed ovarian cancer (MRC OV05/EORTC 55955): a randomised trial.

BACKGROUND: Serum CA125 concentration often rises several months before clinical or symptomatic relapse in women with ovarian cancer. In the MRC OV05/EORTC 55955 collaborative trial, we aimed to establish the benefits of early treatment on the basis of increased CA125 concentrations compared with delayed treatment on the basis of clinical recurrence. METHODS: Women with ovarian cancer in complete remission after first-line platinum-based chemotherapy and a normal CA125 concentration were registered for this randomised controlled trial. Clinical examination and CA125 measurement were done ever

Antiseizure medications consumption in 73 countries and regions from 2012 to 2022: a longitudinal trend study.

BACKGROUND: International trends in antiseizure medication use across countries from different geographical regions and income levels remain underexplored. Valproic acid (valproate) use has raised concerns due to its teratogenic risks, with World Health Organisation (WHO) guidelines recommending lamotrigine or levetiracetam as first-line therapy for epilepsy in women and girls of childbearing potential, advising against valproate use in this population. This study aimed to assess multinational trends in antiseizure medication (ASM) consumption from 2012 to 2022 in the context of evolving polic

The effect of next-generation, dual-active-ingredient, long-lasting insecticidal net deployment on insecticide resistance in malaria vectors in Benin: results of a 3-year, three-arm, cluster-randomised, controlled trial.

BACKGROUND: Insecticide resistance among malaria vector species now occurs in 84 malaria-endemic countries and territories worldwide. Novel vector-control interventions, including long-lasting insecticidal nets (LLINs) that incorporate new active ingredients with distinct modes of action, are urgently needed to delay the evolution and spread of resistance and to alleviate reversals in malaria-control gains. We aimed to assess the longitudinal effect of two dual-active-ingredient LLINs on insecticide resistance during a cluster-randomised, controlled trial in Benin. METHODS: This 3-year, three
