Error-augmentation gait training to improve gait symmetry in patients with non-traumatic lower limb amputation: A proof-of-concept study.

BACKGROUND: Asymmetrical stepping patterns are chronic gait impairment for individuals with non-traumatic lower limb amputation. Persistent gait asymmetries contribute to poor gait efficiency, decreased physical function, and development of secondary orthopedic conditions. OBJECTIVES: Evaluate the feasibility and preliminary responsiveness of a treadmill-based, error-augmentation gait training protocol to improve gait symmetry in patients with non-traumatic transtibial amputation. STUDY DESIGN: Single group, pre- and post-test. METHODS: The error-augmentation gait training protocol involved…

Effects of Intrinsic Feedback-Based Gait Training Under Visual Deprivation on Balance and Gait in Stroke Survivors: A Randomized Controlled Study.

Feedback is an essential component of motor learning and rehabilitation. Although extrinsic feedback is widely used in stroke rehabilitation, excessive dependence on external cues may limit opportunities for independent movement regulation. Training that emphasizes intrinsic feedback during visual deprivation (VD) may provide individuals with greater opportunities to perceive and regulate their movements using internally available sensory information. This study investigated the effects of gait training with intrinsic feedback on balance and gait abilities in individuals with stroke compared w…

The gait lab effect: symmetry restoration strategy after anterior cruciate ligament reconstruction is different in natural environments than the gait laboratory.

Inertial measurement units (IMUs) offer a promising pathway to extend functional-recovery assessment after anterior cruciate ligament reconstruction (ACLR) beyond laboratory or in-clinic evaluations. Yet, it remains unclear how joint kinematics change in natural environments over time. In this study, 26 participants wore five sticker-like IMUs on the pelvis, thighs, and shanks for up to seven days, three and nine months after their surgery. They also participated in a laboratory assessment at each timepoint using standard marker- and inertial-based motion capture systems. Knee extension excurs

Combined Ankle Robot Training and Robot-assisted Gait Training Improved the Gait Pattern of a Patient with Chronic Traumatic Brain Injury.

BACKGROUND: : Walking disability caused by central nervous system injury often lingers. In the chronic phase, there is great need to improve walking speed and gait, even for patients who walk independently. Robot-assisted gait training (RAGT) has been widely used, but few studies have focused on improving gait patterns, and its effectiveness for motor function has been limited. This report describes the combination of "RAGT to learn the gait pattern" and "ankle robot training to improve motor function" in a patient with chronic stage brain injury. CASE: : A 34-year-old woman suffered a trauma…

Detecting gait abnormalities after concussion or mild traumatic brain injury: A systematic review of single-task, dual-task, and complex gait.

BACKGROUND: While a growing number of studies have investigated the effects of concussion or mild traumatic brain injury (mTBI) on gait, many studies use different experimental paradigms and outcome measures. The path for translating experimental studies for objective clinical assessments of gait is unclear. RESEARCH QUESTION: This review asked 2 questions: 1) is gait abnormal after concussion/mTBI, and 2) what gait paradigms (single-task, dual-task, complex gait) detect abnormalities after concussion. METHODS: Data sources included MEDLINE/PubMed, Scopus, Web of Science, and Cumulative Inde…

The effect of cognitive task and gait speed on gait spatiotemporal parameters in athletes with anterior cruciate ligament reconstruction: Cross-sectional study.

BACKGROUND: Restoring pre-injury normal gait following Anterior Cruciate Ligament Reconstruction (ACLR) is a critical challenge. The purpose of this study was to compare spatiotemporal parameters in athletes following ACL reconstruction with healthy athletes when cognitive load and speed were manipulated. METHODS: Twenty male soccer players with an ACLR history and 20 healthy matched individuals completed walking tasks under four conditions: with and without a cognitive load (auditory Stroop task), and at preferred speed as well as high speed (20% higher than the individual's preferred speed)…

Effect of robotic-assisted gait training on gait and motor function in spinal cord injury: a protocol of a systematic review with meta-analysis.

INTRODUCTION: Robotic-assisted gait training (RAGT) has been reported to be effective in rehabilitating patients with spinal cord injury (SCI). However, studies on RAGT showed different results due to a varied number of samples. Thus, summarising studies based on robotic-related factors is critical for the accurate estimation of the effects of RAGT on SCI. This work aims to search for strong evidence showing that using RAGT is effective in treating SCI and analyse the deficiencies of current studies. METHODS AND ANALYSIS: The following publication databases were electronically searched in Dec…

Gait variability across neurodegenerative and cognitive disorders: Results from the Canadian Consortium of Neurodegeneration in Aging (CCNA) and the Gait and Brain Study.

INTRODUCTION: Gait impairment is common in neurodegenerative disorders. Specifically, gait variability-the stride-to-stride fluctuations in distance and time-has been associated with neurodegeneration and cognitive impairment. However, quantitative comparisons of gait impairments across the cognitive spectrum of dementias have not been systematically investigated. METHODS: Older adults (N = 500) with subjective cognitive impairment, Parkinson disease (PD), mild cognitive impairment (MCI), PD-MCI, Alzheimer's disease (AD), PD-dementia, Lewy body dementia, and frontotemporal dementia, as well c…

Re-evaluating the measurement and influence of conscious movement processing on gait performance in older adults: Development of the Gait-Specific Attentional Profile.

BACKGROUND: Recent decades have seen increased interest in how anxiety-and associated changes in conscious movement processing (CMP)-can influence the control of balance and gait, particularly in older adults. However, the most prevalent scale used to measure CMP during gait (the Movement-Specific Reinvestment Scale (MSRS)) is generic (i.e., non-gait-specific) and potentially lacks sensitivity in this context. METHODS: In a preliminary study, we first sought to evaluate if MSRS scores associated with the number of CMP-related thoughts self-reported by older adults while walking. The next aim …

Gait speed assessment in confined spaces: Development of a novel automated 4-m static-start test to measure dynamic-start gait speed.

OBJECTIVES: To promote standardization and feasible measurements of gait speed across the field, we developed a device that used light detection and ranging (LiDAR) technology to measure gait speed from a standing-start testing procedure conducted on a 4-m total walkway. We compared this automated (LiDAR-based) standing-start 4-m gait speed test (AS-4MG) with automated and manual (stopwatch-based) dynamic-start 4-m gait speed tests (AD-4MG and MD-4MG, respectively) on between-method agreement, measurement repeatability, and predictive validity with functional outcomes. METHODS: A sample of 48…

Discriminating vascular parkinsonism from early-stage postural instability gait difficulty subtype dominant Parkinson's disease: a dual-task gait analysis using wearable sensors.

BACKGROUND: Vascular parkinsonism (VP) and the postural instability and gait difficulty (PIGD) subtype of Parkinson's disease (PD) exhibit similar gait characteristics. However, most research emphasizes lower-limb gait parameters, often neglecting the role of cognitive function in gait regulation. Therefore, this study investigates differences in cognitive-motor interactions between VP and PIGD to identifying specific gait biomarkers and develop a diagnostic model. METHODS: We recruited 37 PIGD patients and 37 VP patients between year 2022 to 2024 and used wearable devices to record gait para…

Applying elastic resistance bands for gait training: A simulation-based study to determine how band configuration affects gait biomechanics and muscle activation.

BACKGROUND: Wearable robotic exoskeletons and leg braces are desirable for gait rehabilitation because they can apply loads directly to an affected joint. Yet, they are not widely used in clinics because they are costly and complex to set up. Conversely, tethered devices, such as elastic resistance bands, are widely available in clinics, are low-cost, and are quick to set up. However, resistance bands will affect walking differently based on how they are configured to pull on the leg (e.g., pulling forward or backward). RESEARCH QUESTION: How can a resistance band be configured to alter muscl…

Quality, kinematics, and self-reported comfort of gait during body weight support in young adults with gait impairments - A cross-sectional study.

BACKGROUND: Body weight support (BWS) technologies offer effective gait training for individuals with neurological gait impairments. However, varying dynamic BWS levels may impact specific gait kinematics, potentially introducing maladaptive movement patterns. AIM: To investigate the acute impact of different levels of dynamically modulated BWS during overground walking at a self-selected speed on gait quality, kinematic gait patterns, and self-reported gait comfort in young adults with gait impairments. METHODS: Three-dimensional gait analysis was conducted on 18 individuals with neurologic…

Efficacy of electromechanical-assisted gait training on clinical walking function and gait symmetry after brain injury of stroke: a randomized controlled trial.

Electromechanical-assisted gait training may be an effective intervention to promote motor recovery after brain injury. However, many studies still have difficulties in clarifying the difference between electromechanical-assisted gait training and conventional gait training. To evaluate the effectiveness of electromechanical-assisted gait training compared to that of conventional gait training on clinical walking function and gait symmetry of stroke patients. We randomly assigned patients with stroke (n = 144) to a control group (physical therapist-assisted gait training) and an experimental g…

Objectifying clinical gait assessment: using a single-point wearable sensor to quantify the spatiotemporal gait metrics of people with lumbar spinal stenosis.

BACKGROUND: Wearable accelerometer-containing devices have become a mainstay in clinical studies which attempt to classify the gait patterns in various diseases. A gait profile for lumbar spinal stenosis (LSS) has not been developed, and no study has validated a simple wearable system for the clinical assessment of gait in lumbar stenosis. This study identifies the changes to gait patterns that occur in LSS to create a preliminary disease-specific gait profile. In addition, this study compares a chest-based wearable sensor, the MetaMotionC© device and inertial measurement unit python script (M…
