BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) for extraction of common bile duct (CBD) stones in Billroth II anatomy patients is still a technical challenge and factors affecting stone extraction have not yet been clarified. This study aimed to analyze our experience and evaluate potential factors affecting CBD stone extraction.
METHODS: A retrospective analysis of CBD stones patients with a history of Billroth II gastrectomy, who underwent therapeutic ERCP for stone extraction at our center from August 1999 to December 2017, was conducted. The outcomes of ERCP and potenti

BACKGROUND: Efficient ampullary intervention is essential for endoscopic retrograde cholangiopancreatography (ERCP) in patients with a prior Billroth II gastrectomy. We retrospectively evaluated the safety and effectiveness of ampullary intervention using fully covered self-expandable metal stents (FCSEMSs) for the management of common bile duct (CBD) stones in a subset of patients with a history of Billroth II gastrectomy.
METHODS: This retrospective analysis involved patients with a prior Billroth II gastrectomy who underwent ampullary intervention with FCSEMSs for the management of CBD sto

BACKGROUND: Despite the efficacy of pharmacotherapy for gastrointestinal ulcers, severe cases of bleeding or perforation due to gastrointestinal ulcers still occur. Giant duodenal ulcer perforation is an uncommon but difficult-to-manage pathology with a high mortality rate. We report two cases of giant duodenal ulcer perforation after neurosurgery for brainstem tumors that needed reoperation for gastric disconnection because of postoperative leakage and bleeding.
CASE PRESENTATION: Both cases had undergone neurosurgery for brainstem tumors, and the patients were in a shock state for several d

Billroth I gastric resection with end-to-end gastroduodenal anasthamosis was carried out to 73 patients with complicated duodenal ulcers using new technologies: duodenal mobilization only to the level of ulcer defect, the use of one-row (uninterrupted or nodal) everted suture with adaptation zone of submucous base of gastric stump and muscle coat of duodenal stump. Good early and long-term results were achieved.

Thirty-five patients who had undergone gastric resection between two and five years earlier were included in the study. Digestive continuity had been achieved in 18 cases using the BII method, and in 17 using a Roux en Y loop. Vagotomy was not performed in any patient. All patients were assessed using endoscopy and multiple biopsies of the gastric mucous were taken. No postoperative peptic ulcers were observed. Mucous close to the stoma appeared to be hyperemic and edematous in 16 of the patients who underwent gastric resection using the BII method and in 5 of those in whom a Roux en Y loop wa

An experience with the treatment of 106 patients with pyloroduodenal stenosis of ulcerous etiology by vagotomy with draining operations has shown high efficiency of these measures resulting in lower rate of postoperative lethality. Selective proximal and stem vagotomy does not aggravate gastric atony in patients with stenosis. Electro-stimulation was found to be an effective method of treatment of this complication in the postoperative period of patients with gastric atony.

Experience with proximal gastric vagotomy at the Mayo Clinic from 1973 to Mayo 1980 is reported. Among 298 patients who had proximal gastric vagotomy for chronic duodenal, pyloric channel, or prepyloric ulcers, a recurrent ulcer rate of 7% was present, with a mean follow-up of 49 months. Three recurrences developed in six patients who had proximal gastric vagotomy for gastric ulceration. In 40 patients, proximal gastric vagotomy was combined with gastrojejunostomy, pyloroplasty, or pyloric dilatation for obstructing ulcers. There was a 15% incidence of reoperation in the gastrojejunostomy grou

Seventy-five patients underwent a 90 percent distal gastric bypass for morbid obesity. The average weight was 121.4 kilogram, height 164 centimeters, and age 31.4 years in these 70 women and five men. There was a total of 20 surgical complications in the 75 patients, with wound infection being the most common. Sixteen chronic complications were noted and consisted of vomiting, diarrhea, reflux esophagitis, dysphagia, and vitamin deficiencies. None of the above complications were life-threatening or required dismantling of the bypass. Of 54 patients followed for 12 months or more after gastric

1. The effects of the vagus nerve and of antral gastrin on the rate of histamine formation (histamine forming capacity, HFC, i.e. histidine decarboxylase activity) in the parietal cell region of the gastric mucosa has been investigated in the following stomach preparations: gastric fistula, denervated Heidenhain pouch, antral resection with gastrojejunostomy, gastrojejunostomy with exclusion of the duodenum and in the intact stomach. The determinations of mucosal HFC were made on fasting rats and on re-fed animals when the effect of feeding was studied.2. In fasting rats with a gastrojejunosto

BACKGROUND: Robotic distal gastrectomy (RDG) with Billroth I (BI) reconstruction is predominantly performed due to its physiological congruence and simplicity. The Intracorporeal Triangular Anastomotic Technique (INTACT) aims to reduce ischemic areas compared to the conventional Delta-shaped anastomosis using the unique characteristics of robotic surgery to standardize procedures, thereby ensuring safe, simple, and reliable reconstruction. This study aims to investigate the efficacy of the INTACT in RDG with BI reconstruction, focusing on its robotic precision in minimizing ischemic zones and

PURPOSE: To report a case of vitamin A retinopathy secondary to Billroth II anastomosis triggered after the beginning of dialysis in a patient with a chronic renal failure.
CASE REPORT: A 73-year-old male complained of nyctalopia that had started 9 months ago, coinciding with the beginning of dialysis. His medical history is remarkable for hepatic cirrhosis and Billroth II anastomosis 20 years ago. Best-corrected visual acuity (BCVA) was 60 letters in both eyes. Dilated fundus examination showed faint white-yellowish dots. Optical coherence tomography (OCT) illustrated hyperreflective dots an

Crohn's disease of the small bowel frequently requires surgical intervention. While dealing with the disease complications that require intervention, treatment should be based on a long-term strategic plan that recognizes the likelihood of recurrent disease, repeat surgeries, and the possibility of a future ostomy. Resection forms the basis for surgical treatments, but strictureplasty, abscess drainage, intestinal and diversion bypasses also are used, selectively.

After Billorth-II resection of the stomach for ulcer the contamination of the stump with Helicobacter pylori (HP) was detected in 73.7% of the cases, for ulcer of the duodenum--in 81.8% of the patients. At later terms after selective proximal vagotomy (SPV) all the patients were HP-positive, after SPV with draining operations 92.3% of the patients were HP-positive. The degree of inflammatory alterations in the mucosa was proportional to the degree of its bacterial dissemination. The HP contamination of the gastric mucosa after operation had no marked influence on the clinical course of the rem

To minimize the loss of pyloric function associated with partial gastrectomy (PG), a functional surrogate pylorus, or neopylorus, was made by means of a pantaloon jejunoplastic pouch into which the gastrojejunal stoma was invaginated. This report concerns a consecutive series of 84 patients who underwent PG according to currently accepted criteria. The results showed that formation of the neopylorus apparently prevents dumping and may avoid the necessity for vagotomy in cases without serious hypersecretion. PG patients with a neopylorus formed over 13 years ago still exhibit no enterogastric r

The records of 1,079 patients with gastric carcinoma were reviewed. Of these, only 21 (about 2%) had had previous gastric surgery for benign disease, usually peptic ulcer. The average interval between the original gastric surgery and the discovery of stomach cancer was 26.9 years. The symptoms of cancer presentation were not distinguishable from other forms of the postgastrectomy syndrome. Gastric cancer tended to develop in these patients during the sixth decade of life, irrespective of when they had had their original gastric surgery, strongly suggesting an age-related factor. Although it wo
