Impact of health literacy and general self-efficacy on surgical outcomes 2 years after bariatric surgery.

After bariatric surgery, adherence to lifestyle recommendations is crucial. Health literacy and self-efficacy may impact recovery after surgery. In this multicentre study performed in three hospitals in Sweden, we evaluated any relation between preoperative health literacy and general self-efficacy on the one side and weight loss, health-related quality of life, length of stay, and complications up to 2 years after bariatric surgery on the other. Of 686 included patients, 56% (n = 382) had limited functional health literacy, 42% (n = 278) had limited communicative and critical health literacy,

Variability of prospective general surgery trials by subspecialty: An analysis of the national clinical trials registry.

BACKGROUND: Prospective clinical trials provide robust data to drive guideline recommendations and clinical practice. Surgical trials, as opposed to other medical specialties, have been underrepresented. Our aim was to examine how surgical practice is reflected in current prospective clinical trial activity. METHODS: This is a cross-sectional study of ClinicalTrials.gov. We identified all active phase I through IV surgery clinical trials using keywords represented by 5 surgical specialties: general, hepatobiliary, metabolic and bariatric, robotic/minimally invasive, and endocrine. Trials were

Arguments against a "general and permanent" ban on pediatric intersex surgery: A response to Clune-Taylor.

The paper offers a critical response to the proposed "dis/analogy" between the restriction of Jehovah's Witness parental right to refuse life-saving blood transfusions for their minor children and a "general" and "permanent" ban on "unnecessary" pediatric intersex surgery. The main argument of the analogy is "securing the patient's future autonomy." Feinberg's theory of rights is used to demonstrate that the proposed analogy is untenable. A new category of developmental rights-in-trust is introduced to address specific needs of gender development in DSD situations. Both premises are disputed.

Association Between Intraoperative Hypothermia and Preoperative Body Composition in Patients Undergoing General Anesthesia for Gastrointestinal Surgery.

Introduction General anesthesia induces a redistribution of deep body temperature drop, and a decrease in body temperature during surgery is associated with increased blood loss and transfusion requirements, increased incidence of wound infections, and prolonged hospital stays. Furthermore, low body mass index (BMI) is associated with a pronounced decrease in body temperature. However, subcutaneous fat mass, visceral fat mass, and skeletal muscle mass have not been precisely measured. More precise quantification of body composition can be achieved using computed tomography (CT) imaging, which

Remote Continuous Vital Sign Monitoring of Scoliosis Surgery Patients on General Wards: A Cost-Effectiveness Analysis.

BACKGROUND: Scoliosis surgery patients often require continuous postoperative monitoring in intensive care (ICU) or high-dependency units (HDU). We implemented a 24-hour remote continuous monitoring service for high-risk inpatients (HIVE) to allow monitoring in general wards. This study aimed to evaluate the cost-effectiveness of the HIVE service. METHODS: We compared scoliosis surgery patients admitted pre- and postimplementation of the HIVE service and applied multivariable regression to adjust for differences in baseline characteristics. The primary outcome was incremental cost per ICU hou

Risk for cancer after bariatric surgery compared with the general population: a nationwide matched cohort study.

OBJECTIVE: Our objective was to evaluate how the incidence of cancer after metabolic and bariatric surgery (MBS) compared with the general population. METHODS: A nationwide cohort of patients who underwent MBS in Sweden from 2007 to 2020 were matched (age, sex, area of residence) to controls from the general population. All patients were followed for new incidence of cancer. RESULTS: Among the 68,424 patients who underwent MBS compared with the 640,944 controls, there was no difference in the risk for new onset of cancer (incidence rate ratio [IRR]: 1.03, 95% CI: 0.99-1.07). Compared with co

Processed electroencephalography-guided general anesthesia and norepinephrine requirements: A randomized trial in patients having vascular surgery.

STUDY OBJECTIVE: Processed electroencephalography (pEEG) may help clinicians optimize depth of general anesthesia. Avoiding excessive depth of anesthesia may reduce intraoperative hypotension and the need for vasopressors. We tested the hypothesis that pEEG-guided - compared to non-pEEG-guided - general anesthesia reduces the amount of norepinephrine needed to keep intraoperative mean arterial pressure above 65 mmHg in patients having vascular surgery. DESIGN: Randomized controlled clinical trial. SETTING: University Medical Center Hamburg-Eppendorf, Hamburg, Germany. PATIENTS: 110 patients

Prioritizing rapid COVID-19 testing in emergency general surgery patients decreases burden of inpatient hospital admission.

OBJECTIVES: The COVID-19 pandemic has changed delivery of emergency general surgery (EGS) and contributed to widespread bed shortages. At our institution, rapid testing is not routinely approved for EGS patients. We examined common EGS conditions (appendicitis and acute cholecystitis), hypothesizing that necessity of testing for COVID-19 significantly delayed operative intervention. METHODS: We performed a prepost study to examine a 2-month timeframe, or historical control, prior to COVID-19 testing (January 1, 2020-March 1, 2020) as well as a 2-month timeframe during the COVID-19 era (Januar…

Functional recovery with peripheral nerve block versus general anesthesia for upper limb surgery: a systematic review.

BACKGROUND: Peripheral nerve block is a common anesthetic technique used during orthopedic upper limb surgery. Injection of local anesthetics around the target nerve inhibits the action of voltage-dependent sodium channels, inhibiting neurotransmission of pain impulses and providing motor immobility. Compared to general anesthesia, it could improve functional recovery by inhibiting nociceptive impulses and inflammation, thus reducing postoperative pain and immobilization and improving postoperative rehabilitation. This systematic review evaluates the impact of peripheral nerve block versus gen

Risk factors for discontinuation of intravenous patient-controlled analgesia after general surgery: a retrospective cohort study.

Identifying patients at risk for developing side effects secondary to intravenous patient-controlled analgesia (IV PCA) and making the necessary adjustments in pain management are crucial. We investigated the risk factors of discontinuing IV PCA due to side effects following general surgery; adult patients who received IV PCA after general surgery (2020-2022) were included. Data on postoperative pain intensity, PCA pain relief, side effects, continuity of PCA use, and PCA pump settings were collected from the records of the acute pain management team. The primary outcome was identifying the ri…

Cesena guidelines: WSES consensus statement on laparoscopic-first approach to general surgery emergencies and abdominal trauma.

BACKGROUND: Laparoscopy is widely adopted across nearly all surgical subspecialties in the elective setting. Initially finding indication in minor abdominal emergencies, it has gradually become the standard approach in the majority of elective general surgery procedures. Despite many technological advances and increasing acceptance, the laparoscopic approach remains underutilized in emergency general surgery and in abdominal trauma. Emergency laparotomy continues to carry a high morbidity and mortality. In recent years, there has been a growing interest from emergency and trauma surgeons in ad…

Proton pump inhibitor therapy and risk of knee replacement surgery: a general population-based cohort study.

OBJECTIVE: Proton pump inhibitors (PPIs) are among the most commonly used medications for patients with osteoarthritis (OA). Various types of PPIs have different impacts on lowering serum magnesium level that may affect knee OA progression. We aimed to compare the risk of clinically relevant endpoint of knee replacement (KR) among initiators of five different PPIs with that among histamine-2 receptor antagonist (H2RA) initiators. DESIGN: Among patients with knee OA (≥50 years) in The Health Improvement Network database in the UK we conducted five sequential propensity-score matched cohort stu
