White Matter Microstructural Alterations Mediate the Association between Polygenic Risk for Alzheimer's Disease and Cognitive Performance in Clinically Asymptomatic Adults.

PURPOSE: Alzheimer's disease (AD) is a progressive neurodegenerative disorder influenced by genetic factors, with a long preclinical phase characterized by subtle alterations in brain microstructure. Although the apolipoprotein E ε4 allele is a well-established genetic risk factor, AD is increasingly recognized as a polygenic condition. However, how polygenic risk manifests in white matter (WM) microstructure and cognition remains unclear. This study aims to investigate the associations between AD polygenic risk scores (ADPRS), WM microstructure, and cognitive performance in clinically asympto

The dietary index for gut microbiota, genetically predicted gut microbiome, and the risk of chronic kidney disease: a cohort study.

Background: the dietary index for gut microbiota (DI-GM) is a recently proposed index that reflects the diversity of the gut microbiota from the perspective of dietary intake. However, its association with chronic kidney disease (CKD) has not been widely studied. Methods: this prospective cohort study included 166 865 participants free of CKD at baseline. The DI-GM was assessed through a 24 h dietary recall questionnaire. Incident CKD was ascertained using hospital inpatient records, death registry data, and primary care data. A genetic risk score for gut microbial abundance was constructed ba…

Longitudinal Study of Frailty Phenotype in Relation to Chronic Kidney Disease Incidence.

BACKGROUND: The longitudinal relationship between frailty phenotype and CKD development, and the modifying role of genetic CKD risk in this association, remains unclear. Research on simple, noninvasive and quantifiable CKD prediction models incorporating frailty is limited. METHODS: We analysed 214 502 CKD-free participants from the UK Biobank cohort. Frailty was assessed using a modified phenotype with five components: weight loss, exhaustion, low grip strength, low physical activity and slow gait speed, to match UK Biobank data. Self-reported walking pace and weight change served as proxies

Polygenic Risk Identifies Older Adults Who May Benefit From Aspirin for the Primary Prevention of Ischemic Stroke.

BACKGROUND: Low-dose aspirin is no longer recommended for routine primary prevention in older adults due to bleeding risks outweighing vascular benefits. We hypothesized that an integrative polygenic score (iPGS) could identify a subgroup of older individuals who derive net benefit from aspirin for the primary prevention of ischemic stroke. METHODS: We performed post hoc analysis of the ASPREE randomized, placebo-controlled trial (Aspirin in Reducing Events in the Elderly) of daily 100-mg aspirin, in 12 031 genotyped participants of European ancestry aged >70 years without prior cardiovascula

Polygenic Risk Based Detection and Treatment of Subclinical Coronary Atherosclerosis in the PROACT Clinical Trials.

BACKGROUND: Coronary artery disease (CAD) polygenic risk scores (PRS) may identify individuals at elevated genetic risk "flying under the radar" in contemporary practice. The aims of the PROACT (Polygenic Risk Based Detection and Treatment of Subclinical Coronary Atherosclerosis) trials are to prospectively identify these individuals, quantify subclinical coronary plaque, and slow its progression with pharmacologic interventions. OBJECTIVES: The aim of this study is to report interim feasibility and implementation findings from PROACT, a genotype-first, biobank-enabled trial, characterizing e

The Relationship Between Polygenic Risk of Depression and Regional Brain Atrophy in Older Adults.

BACKGROUND: Depression has been identified as a risk factor for the development of dementia, but the relationship between the two conditions is complex. If the mechanism by which depression increased the risk of dementia was better understood then new treatment targets could be identified. Previous studies have found evidence of reduced hippocampal volume in depressed patients, but because depression may also be an early sign of dementia, these studies may have inadvertently included individuals in the early stages of dementia. We used genetic risk of depression as our exposure and examined wh

Polygenic risk score for drug-induced long QT syndrome: independent validation in a real-world patient cohort.

OBJECTIVE: Drug-induced long QT syndrome (diLQTS) is an adverse reaction from over 150 FDA-approved medications, posing the risk of triggering torsades de pointes and sudden death. While common genetic variants may modestly impact QT interval individually, their collective effect can significantly amplify risk of diLQTS. Consequently, this study aimed to validate a polygenic risk score (PRS) for diLQTS previously proposed by Strauss et al . METHODS: A retrospective cohort study was conducted utilizing patients from the Michigan Genomics Initiative prescribed 27 high-risk QT-prolonging drugs a

Preoperative prediction of metastatic pheochromocytoma and paraganglioma using clinical, genetic, and biochemical markers: A cohort study.

BACKGROUND: The prevalence of metastatic pheochromocytoma and paraganglioma (PPGL) is approximately 15%-20%. Although there are indicators to assess metastatic risks, none of them predict metastasis reliably. Therefore, we aimed to develop and validate a scoring system using clinical, genetic, and biochemical risk factors to preoperatively predict the metastatic risk of PPGL. METHODS: In the cross-sectional cohort (n = 180), clinical, genetic, and biochemical risk factors for metastasis were identified using multivariate logistic regression analysis, and a novel scoring system was developed.

Prediction of fetal hemoglobin in sickle cell anemia using an ensemble of genetic risk prediction models.

BACKGROUND: Fetal hemoglobin (HbF) is the major modifier of the clinical course of sickle cell anemia. Its levels are highly heritable, and its interpersonal variability is modulated in part by 3 quantitative trait loci that affect HbF gene expression. Genome-wide association studies have identified single-nucleotide polymorphisms (SNPs) in these quantitative trait loci that are highly associated with HbF but explain only 10% to 12% of the variance of HbF. Combining SNPs into a genetic risk score can help to explain a larger amount of the variability of HbF level, but the challenge of this app…

Intrinsic capacity modifies the risk of depression and anxiety through metabolomic signature.

BACKGROUND: The World Health Organization (WHO) developed an assessment for healthy aging termed intrinsic capacity (IC). However, the association between IC and depression/anxiety remains poorly understood, with limited understanding of the underlying metabolic mechanisms. METHODS: This prospective cohort study comprised 212,033 UK Biobank participants recruited between 2006 and 2010. We calculated a UK Biobank-adapted four-domain IC deficit score using seven functional status indicators across psychological, sensory, vitality, and locomotor domains. Elastic net regression was employed to de

Diagnosis of Familial Hypercholesterolemia in Children: From Clinical Features Through Gene Variants to Polygenic Score.

BACKGROUND: Early diagnosis of familial hypercholesterolemia (FH) is crucial to improve long-term outcomes. FH diagnosis relies on elevated low-density lipoprotein cholesterol (LDL-C) levels, familial clinical characteristics, and identification of pathogenic variants in FH-related genes. Secondary factors, such as overweight and obesity, are known to influence lipid profiles in the general population. More recently, polygenic risk scores based on single-nucleotide polymorphisms (SNPs) have been proposed as additional determinants of LDL-C levels. METHODS: We enrolled 214 pediatric subjects w

Influence of Cardiometabolic and Alzheimer Disease Genetics on Cognitive-Related Outcomes in a Diverse Population.

BACKGROUND AND OBJECTIVES: Late-life dementia has a strong heritable component. While the APOE ɛ4 allele is the strongest genetic risk factor of Alzheimer disease (AD), polygenic risk factors also play a significant role. Cardiometabolic diseases (CMDs) and their risk factors predict dementia in many cohorts, but how genetic predictors of CMD influence dementia-related outcomes remains unclear. In this study, we examined the associations of polygenic risk scores (PRSs) for major CMDs and the established APOE risk alleles with dementia-related outcomes in the Reasons for Geographic and Racial D

Transferability of European-derived Alzheimer's disease polygenic risk scores across multiancestry populations.

A polygenic score (PGS) for Alzheimer's disease (AD) was derived recently from data on genome-wide significant loci in European ancestry populations. We applied this PGS to populations in 17 European countries and observed a consistent association with the AD risk, age at onset and cerebrospinal fluid levels of AD biomarkers, independently of apolipoprotein E locus (APOE). This PGS was also associated with the AD risk in many other populations of diverse ancestries. A cross-ancestry polygenic risk score improved the association with the AD risk in most of the multiancestry populations tested w
