BACKGROUND: Intravenous patient-controlled analgesia (IV-PCA) is often used in the postoperative period. However, determining an appropriate opioid dose is difficult. A previous study suggested the usefulness of variable-rate feedback infusion. In this study, we used a dual-channel elastomeric infusion pump to provide changes in PCA infusion rate by pain feedback.
METHODS: Ninety patients undergoing orthopedic surgery of American Society of Anesthesiologists grade I-III and 65 to 79 years of age participated in the study. All patients were given a dual-chamber PCA. Patients were randomly allo

INTRODUCTION: Delirium is a neurocognitive disorder common in older adults in acute care settings. Those who develop delirium are at an increased risk of dementia, cognitive decline and death. Electroencephalography (EEG) during delirium in older adults is characterised by slowing and reduced functional connectivity, but markers of vulnerability are poorly described. We aim to identify EEG spectral power and event-related potential (ERP) markers of incident delirium in older adults to understand neural mechanisms of delirium vulnerability. Characterising delirium vulnerability will provide sub

BACKGROUND: In the future there will be an increasing demand for professional care with simultaneous retention of the dentition in older people. Due to inadequate dental knowledge, it is often not possible for caregivers to adequately assess dental deficits.
OBJECTIVE: The aim of the study was to develop and validate a simple tool (Mini Dental Assessment, MDA) to assess possible dental treatment needs (DTN) of residential geriatric facilities by nursing personnel.
MATERIAL AND METHODS: In the study 169 patients (51 from the University Hospital Giessen, 118 from the Bonifatius Hospital Lingen

AIM: To compare the effects of exergames (EGs) using the Xbox Kinect™ device and home exercise (HE) on balance, functional mobility, and quality of life of individuals aged 65 years or older.
METHOD: One hundred participants who met the inclusion criteria were randomized to the EG or HE group. The EG group took part in a 6-week programme using the Xbox360Kinect™ device, and the HE group took part in a 6-week balance exercise programme at home 5 days a week. The Berg Balance Scale (BBS) was used to assess balance, the Timed Up and Go (TUG) test was used to evaluate functional walking, and the

PURPOSE: Interleukin-6 (IL-6), interleukin-10 (IL-10), and monocyte chemoattractant protein-1 (MCP-1), also known as chemokine (C-C motif) ligand 2 (CCL2), are pivotal mediators of the immune response following trauma. While IL-6 predominantly promotes inflammation, IL-10 serves a regulatory function, and MCP-1/CCL2 facilitates monocyte recruitment and differentiation. Dysregulation of these cytokines has been linked to adverse postoperative outcomes in geriatric patients. An investigation was conducted to analyze the cytokine release profile in geriatric patients with proximal femur fractures

In the context of demographic trends, two geriatric syndromes that partially overlap but are conceptually distinct are becoming increasingly prevalent: multimorbidity and frailty. These two terms are often insufficiently distinguished from one another. However, they differ fundamentally in terms of their underlying concepts, pathophysiology, and clinical implications. A deeper understanding of these two entities is essential both for the individualized medical care of geriatric patients and for the prevention of functional decline in old age.

Older adults comprise the fastest growing demographic in correctional facilities in the United States. Studies have shown older adults in prison report difficulty accessing healthcare and navigating the prison environment. Little is known about how older adults navigate aging and access healthcare in the jail setting. We sought to explore experiences of older adults incarcerated in a county jail. We conducted semi-structured qualitative interviews with 20 older adults (> 50 years old) who were incarcerated at a local county jail and were referred to the emergency department for acute care need

Polypharmacy is common among the elderly. This case report details the experience of an 85-year-old man whose notable polypharmacy and additional over-the-counter medicines led to anemia and acute kidney injury. These issues were traced to an unintended »triple-whammy« effect due to the interaction of diuretics, ACE inhibitors, and NSAIDs, and concurrent treatment with an anticoagulant. Highlighting the role of systematic medication reviews, this case illustrates how targeted interventions can prevent severe health outcomes and underscores the importance of medication monitoring in elderly pat

With the aging population in Germany, the incidence of fragility fractures is increasing significantly. To improve the care of these patients, geriatric trauma centres (ATZ-DGU) have been established by the German Society for Trauma Surgery (DGU). These centres differ from pure orthogeriatric co-management (OGCM) by more comprehensive requirements regarding process quality, outcome quality, prevention, and continuity of care. The evidence is strongest for hip fractures: studies demonstrate significantly reduced mortality, improved mobility, and increased prescription rates of osteoporosis medic

The surgical management of tibial plateau fractures in geriatric patients presents relevant challenges compared with younger patient populations. In addition to fracture-specific aspects, these particularly include patient-related factors, such as osteoporosis, multimorbidity and cognitive impairment, all of which can influence the interpretation of indications, operative strategy and postoperative management. This review article provides an overview of the key considerations for osteosynthesis of geriatric tibial plateau fractures and summarizes the relevant influencing factors that should be

Ankle fractures are among the most common fragility fractures in older adults. Osteoporosis, age-related impairment of bone regeneration, and multimorbidity increase the risk of delayed union, nonunion, and postoperative complications. Besides anatomical reduction and stable fixation, adjunctive therapies aimed at enhancing biological fracture healing have gained growing interest. This review summarizes the pathophysiological aspects of fracture healing in elderly patients and evaluates current biological, pharmacological, and physical strategies to promote healing after geriatric ankle fractu

BACKGROUND: With more than half of orthopedic surgeries performed on patients aged ≥ 65 years, optimizing perioperative care in this population is essential. Age-related physiologic changes, multimorbidity, and frailty significantly increase surgical risks, including infection, venous thromboembolism, impaired wound healing, delirium, and prolonged recovery. Effective perioperative management strategies are essential to reduce morbidity and improve functional outcomes.
OBSERVATIONS: Using tools such as the Fracture Risk Assessment Tool and the Comprehensive Geriatric Assessment, frailty scree

Geriatrics and neurosurgery have several points of contact, which require a close cooperation. In this context, diseases frequently affecting old people include chronic subdural hematomas, benign and malignant brain tumors, normal pressure hydrocephalus, spinal compression syndromes, spinal nerve root and peripheral nerve affections. In the case of absolute indications for surgery (e.g., symptomatic subdural hematoma, paraparesis or tetraparesis due to compression of the myelon), no alternative conservative treatment option is available to prevent death or irreversible damage. Such interventio…

Interprofessional collaboration is critical to optimizing care for older adults, yet comprehensive geriatric assessment (CGA) remains challenging in community and primary care settings lacking team-based infrastructure. Given their accessibility and longitudinal patient relationships, pharmacists are well positioned to support CGA beyond medication review. This pilot project used a modified Delphi process to identify practical, evidence-based tools pharmacists could use to assess CGA domains including functional ability, mobility, cognition, and social support. A multidisciplinary panel, inclu

PURPOSE: Current evidence on the use of eHealth in geriatric rehabilitation is limited. This aim of this study was to achieve international consensus on three key eHealth-related topics in geriatric rehabilitation: the use, domains, and scientific evaluation of eHealth. Additionally, we developed a model that provides insight into the use of eHealth in geriatric rehabilitation.
METHODS: An international, two-round Delphi study was conducted. Two models served as a framework for the initial statement draft, with a total of 28 statements based on our systematic review results, an international

As the aging population progresses, more and more traumatic events involve the elderly. Given the systemic "frailty" of the elderly, there is currently no universally accepted method for geriatric trauma assessment. The existing methods for assessing geriatric trauma patients - the Geriatric Trauma Outcome Score (GTOS), the Elderly Mortality After Trauma (EMAT), and Geriatric Trauma Mortality Score (GERtality)-were reviewed. GTOS, GERtality and EMAT have 3, 5 and 26 parameters, respectively. EMAT contains almost every aspect of clinical scenarios, such as demographic data, comorbidities, injur
