The authors retrospectively compared the results of postoperative speech and swallowing in patients who had undergone hemiglossectomy for carcinoma of the anterior tongue. Immediate reconstruction in 16 patients was with a free radial forearm flap and in another 16 with an anterolateral thigh flap. Clinical speech pathology evaluation included the Fletcher time-to-time maximum syllable repetition rate, multiple rhyme test, and overall quality and intelligibility of the patients' speech. Evaluation of swallowing included deglutition duration, bolus volume, and ingestion rate. The functional res…

INTRODUCTION: Facioscapulohumeral muscular dystrophy (FSHD) is an autosomal dominant disorder with a shortened fragment of a D4Z4 repeat on chromosome 4q35. The infantile form of FSHD is relatively rare.
CASE REPORT: Within this case report, we describe the orthognatic surgery in a patient with infantile FSHD to diminish the functional and esthetic disturbances in the orofacial region. We saw a 20-year-old female patient suffering from infantile FSHD with remarkable macroglossia and frontal open bite. Diagnosis was based on molecular genetic investigations. To improve the functions of the sto

For cancers of the tongue, floor of mouth, mandibular gingiva and buccal mucosa, in which the widest diameter is greater than 2 cm, we perform neck dissection in continuity with resection of the primary growth as a part of the planned therapy, regardles of the clinical state of the cervical lymph nodes. Whenever the lesion is so situated that an in-continuity neck dissection cannot be performed (cancer of the palate) it may be wise to delay neck dissection until the lymph nodes become clinically apparent.

OBJECTIVE: Conventional free flaps for tongue reconstruction often result in adhesion and limited mobility. This study presents a case series of two patients evaluating a novel reinnervated free gracilis flap with a basal curtain barrier design. METHODS: Two patients with cT3N0M0 tongue squamous cell carcinoma underwent hemiglossectomy. Case 1 was reconstructed using the novel technique (gracilis muscle with a skin island barrier), while Case 2 was reconstructed with a standard reinnervated gracilis muscle flap without a skin island. RESULTS: In Case 1, the skin island effectively closed the f…

BACKGROUND: Tongue reduction surgery can improve symptomatic macroglossia in Beckwith-Wiedemann syndrome (BWS), but there is no consensus on the optimal tongue reduction surgical technique. This study assessed trends and outcomes of several tongue reduction techniques for treating macroglossia in BWS. Using perioperative and polysomnographic data, the authors present their institutions' rationale for increasingly adopting the peripheral resection technique.
METHODS: Medical records of patients with BWS seen between December of 2004 and June of 2024 were reviewed for molecular diagnosis, surgi

BACKGROUND: We report a novel reconstruction technique that maintained effective swallowing after total glossolaryngectomy (TGL) by restoring pharyngeal constriction using a vascularized vastus lateralis muscle transfer.
METHODS: A 65-year-old male with recurrent tongue cancer underwent TGL and anterolateral thigh flap reconstruction with the vastus lateralis muscle. The bilateral cut ends of the remaining posterior pharyngeal wall constrictor muscle were sutured to the transferred vastus lateralis muscle so that the two muscles encircled the reconstructed pharynx. The femoral nerve of the va

OBJECTIVE: Anterior tongue reduction is indicated when macroglossia causes problems with oral hygiene, airway compromise, deglutition, articulation or orthognathic complications. Causes of macroglossia include hypothyroidism, mucopolysaccharide and lipid storage disease, lymphangioma, haemangioma, neurofibroma, and muscular macroglossia. This paper presents an 11-year experience of anterior tongue reduction at Great Ormond Street Hospital.
METHOD: Retrospective study of patient medical records identified from the hospital ENT database. Anterior wedge resection was the preferred technique.

OBJECTIVES: Metastases to the heart are rare. We report a case of squamous cell carcinoma of the base of tongue secondarily complicated with cardiac metastasis 6 months after surgical treatment resulted in successful local control.
METHODS: The lesion was found using computerized axial tomography in a patient with minimal cardiologic symptoms.
RESULTS: The patient died shortly due to complications of his metastatic disease. No curative treatment was possible.
CONCLUSIONS: Cardiac metastasis should be suspected when new cardiovascular symptoms are observed in patients with a history of head

The base of the tongue after tumor resection can be reconstructed in several ways. In addition to direct suturing of small defects, a variety of local or microvascularly anastomosed flaps have been described. The present paper is aimed at introducing a local tongue flap for the reconstruction of medium-sized defects. Six patients were treated at the hospital between 1999 and 2002 with the above technique to cover defects affecting one-half to three-fourths of the base of the tongue. One patient had secondary bleeding and the other one experienced no complications. Nutrition could be started af

Although several reports describe the prosthetic management of patients after hemiglossectomy, the techniques are related to the fabrication of the prostheses, no reports on maximizing the functional potential of the remaining tongue musculature and surrounding tissues were identified by the authors. This clinical report describes the use of myofunctional therapy as an aid to the maxillofacial prosthodontic rehabilitation of an edentulous patient who was diagnosed with invasive squamous cell carcinoma of the tongue and underwent hemiglossectomy. Myofunctional therapy was introduced before, dur…

Foreign bodies in a mobile tongue are rarely presented to the laryngologist, because such bodies are commonly lodged superficially and are easily removed by the patients themselves or by general practitioners. Thus, it is rare that a foreign body totally embedded in the mobile part of the tongue presents as an enlarged tongue mass. We have described a 64-year-old female with a 3-month history of an enlarged mass in the anterior right tongue. Physical examination showed a mass located in the anterior right tongue, with intact mucosa and normal color. A benign tongue neoplasm was considered firs

The objectives of this study were (1) to determine the extent of sensory recovery on hemitongues reconstructed with innervated radial forearm flaps and (2) to assess the influence of various clinical and surgical factors over the return of sensation, including the use of different recipient nerves for neurorrhaphy. Twenty-eight patients with tongue cancer who underwent hemiglossectomy and primary reconstruction with innervated radial forearm flaps over a 3-year period were studied. Mean postoperative follow-up was 18.2 months (range 6 to 32 months). Sensory recovery was assessed in a blind man…

BACKGROUND: Leech therapy is used in diverse ways in the medical field, such as for flap salvation and for treatment of osteoarthritis. While bleeding is a recognized adverse effect, severe coagulopathy is rare and underreported, especially when compounded by concurrent anticoagulant use.
CASE PRESENTATION: We present a case of a 76-year-old female undergoing leech therapy in combination with systemic heparin for free flap salvage following partial glossectomy and neck dissection. Shortly after initiating both therapies, the patient developed marked coagulopathy, evidenced by prolonged activa…
