A polygenic score for height identifies an unmeasured genetic predisposition among pediatric patients with idiopathic short stature.

BACKGROUND: A subset of children with short stature do not have an identified clinical explanation after extensive diagnostic evaluation. We … endocrinologist. Participants were classified as having one of five short stature subtypes: primary growth disorders, secondary growth disorders, idiopathic short stature (ISS), which was sub-classified into familial (ISS-F) and non-familial (ISS-NF), and constitutional delay of puberty (ISS-DP). Differences in polygenic predisposition between subtypes were analyz …

Clinical studies with GHRH in man.

GHRH was isolated from two GHRH-secreting pancreatic tumors which resulted in clinical acromegaly. Over 98% of acromegalic patients have a pituitary adenoma; however, acromegaly may occasionally result from ectopic or eutopic GHRH secretion. Administration of GHRH to normal adults stimulates growth hormone (GH) secretion; it may also stimulate GH release in some adults with GH deficiency in childhood and in a majority of GH-deficient children. Continuous infusion of GHRH to normal men stimulates GH secretion which augments naturally occurring GH pulses. GHRH is effective when administered subc

Anxiety and growth disturbance: is there a connection? A review of biological studies in social phobia.

Current knowledge of the neurobiology of social anxiety and social phobia is reviewed within the framework of chemical models of anxiety. Preliminary evidence for noradrenergic, serotonergic, and adenosinergic systems in the neurobiology of social phobia is presented and discussed within the context of medical model versus continuum theories of anxiety. The clinical and theoretical implications of a hypothesized linkage between anxiety disorders and hypothalamic-growth hormone dysfunction are presented. The author recommends that additional research strategies be developed to examine growth pa

The association of stillbirth with placental abnormalities in growth-restricted and normally grown fetuses.

BACKGROUND: Stillbirth, defined as foetal death ≥20 weeks' gestation, is associated with poor foetal growth and is often attributed to placental abnormalities, which are also associated with poor foetal growth. Evaluating inter-relationships between placental abnormalities, poor foetal growth, and stillbirth may improve our understanding of the underlying mechanisms for some causes of stillbirth. OBJECTIVE: Our primary objective was to determine whether poor foetal growth, operationalised as small for gestational age (SGA), mediates the relationship between placental abnormalities and stillbi

The effect of partial periosteal division on growth of the long bones. An experimental study in rabbits.

The mode of action of the periosteum in the growth of the long bones was investigated by comparing the activity of the growth plate after hemicircumferential and longitudinal periosteal incisions. Twenty-eight rabbits were divided into three groups. A longitudinal periosteum incision was made on the medial upper tibia in rabbits of Group A, and a hemicircumferential periosteum incision was made in Group B. An incision of the skin and superficial tissue only, similar to the skin incision of Groups A and B, was performed on rabbits in Group C. Certain differences in the development of the right

[Growth plate regulation and short stature: mechanistic insights and therapeutic advances].

The growth plate is the key structure for longitudinal growth of long bones, and its function is coordinated by a complex network of systemic endocrine signals, local paracrine signals, structural support from the extracellular matrix, and intracellular signaling pathways. Dysregulation of this network can lead to short stature (SS). At present, treatment of SS mainly relies on various hormone replacement therapies, but efficacy varies. Emerging approaches, including targeted drugs, molecularly targeted delivery, and gene therapy, have demonstrated potential for clinical application. This revi

Human Growth and Growth Hormone: From Antiquity to the Recominant Age to the Future.

Since antiquity Man has been fascinated by the variations in human (and animal) growth. Stories and art abound about giants and little people. Modern genetics have solved some of etiologies at both extremes of growth. Serious study began with the pathophysiology of acromegaly followed by early attempts at treatment culminating in modern endoscopic surgery and multiple pharmacologic agents. Virtually at the same time experiments with the removal of the pituitary from laboratory animals noted the slowing or stopping of linear growth and then over a few decades the extraction and purification of

This brief review highlights new studies in three areas of the GH field, namely diagnostics, therapeutics and biomarkers. The diagnosis of G … ave provided reassurance on the safety of recombinant human GH (r-hGH) for those with uncomplicated growth disorders, but do emphasise the need to continue observation through adulthood. The search for biomarkers that precisely reflect GH action in children and adults is an ongoing task. One of the newer bone markers that shows promise is a fragment of collagen ty …

Isolated growth hormone deficiency.

Isolated growth hormone deficiency (IGHD) represents conditions of GH deficiency that are not necessarily associated with other pituitary hormone deficiencies or with an organic lesion. Three sub-categories of IGHD have been clinically identified (IGHD types 1-3), and IGHD type 1 has been further separated into IGHD types 1a and b. However, this clinical sub-categorization of IGHD may need reconsideration due to the recent identification of molecular heterogeneity within each sub-type of IGHD. In a small number of children with IGHD, defects in the GH, GH-releasing hormone receptor (GHRH-R), a

Indications for GH replacement in adolescents and young adults.

Data obtained from studies in childhood-onset GH deficiency (CO-GHD) in adult life indicate differences in body composition in comparison with adult-onset GHD, but the response to GH replacement is qualitatively similar in terms of reduction in fat mass, serum cholesterol and LDL-cholesterol. However, because bone development is GH dependent and achievement of peak bone mass occurs some years later than completion of linear growth, continuing somatic development is a potentially important issue in the GHD adolescent. This issue has been examined in a United Kingdom multicentre study, in which

Infant feeding and weight in the first year of life in babies of women with eating disorders.

OBJECTIVE: To examine feeding patterns and growth in the first year of life in infants of women with eating disorders in a population-based cohort. STUDY DESIGN: Women and their infants (n = 12 050) from the Avon Longitudinal Study of Parents and Children were studied. Prospectively collected data on feeding difficulties at age 1 and 6 months, breast-feeding during the first year, and weight and conditional growth at age 9 months were compared for infants of women with a self-reported history of an eating disorder (anorexia nervosa or bulimia nervosa) and women with and without other severe p

Pediatric endocrinology.

Endocrine and metabolic disorders affecting puppies and kittens from birth until 6 months of age may manifest as clinical problems related to growth or to water metabolism (polydipsia and polyuria). Most commonly, endocrine and metabolic disorders affect growth of the animal, and puppies are often presented to the veterinarian for assessment of delayed or aberrant growth. Other endocrine disorders of small animals, such as juvenile-onset diabetes insipidus or diabetes mellitus, affect water metabolism, resulting in excessive thirst and urination and resultant difficulty in house-breaking.

Growth factors and cytokines in hair follicle development and cycling: recent insights from animal models and the potentials for clinical therapy.

Hair growth disorders, particularly those that lead to hair loss (alopecia), are common and frequently cause significant mental anguish in affected individuals. The mechanisms underlying the majority of these disorders are unknown. However, insights into the specific molecular mechanisms of hair follicle development and cycling have recently been made using animal models, particularly mice that over- or underexpress a specific gene for a growth factor or cytokine. Other animal models have demonstrated that certain growth factors and cytokines can prevent much of the alopecia caused by cancer c…

Schistosomiasis and childhood stunting.

Linear growth stunting increases a child's risk for mortality and lifelong morbidities. Understanding and preventing the causes of childhood stunting is a global health priority. The leading contributors to stunting are undernutrition and infection. Schistosomiasis, a neglected tropical disease, has long been associated with childhood stunting, but there are limited reviews summarizing the evidence. This narrative review examines cross-sectional associations between infection and linear growth, longitudinal impacts of schistosomiasis treatment on growth and the influence of schistosomiasis on

Craniofacial Changes Among Children and Adolescents Submitted to Growth Hormone Therapy: A Systematic Review.

The aim of this systematic review was to investigate the association between craniofacial changes and growth hormone (GH) therapy among children and adolescents with GH deficiency or idiopathic short stature (ISS). The PRISMA guideline was followed to carry out all stages of this review. An electronic search was conducted in seven databases, without year or language restrictions. The study selection was carried out in two stages by two calibrated examiners. Studies exploring craniofacial changes among children and adolescents with GH deficiency or ISS undergoing GH therapy were included. After

Progress on growth promoting therapies other than growth hormone.

Recombinant human growth hormone is a classical therapeutic drug for children with short stature. In recent years, as the mechanism of growth in children has been further explored, growth-promoting therapies other than growth hormone have made great progress. Recombinant human insulin-like growth factor (IGF)-1 is the main treatment for primary IGF-1 deficiency, and C-type natriuretic peptide (CNP) offers a therapeutic option for children with short stature due to chondrodysplasia. Growth hormone-releasing peptide analogues stimulate growth hormone release and may be used for growth-promoting
