Synovial Sarcoma of the Hand: A Case Report and Literature Review on a Significant Challenge in Treatment.

Soft tissue sarcomas (STS) of the hand are extremely uncommon, with synovial sarcomas being even rarer. Despite their small size at presentation, hand sarcomas can indeed be aggressive and may result in significant morbidity and mortality if not effectively managed. A considerable proportion of these tumors are treated by unplanned excision before referral to a specialist oncological center. Additionally, hand sarcomas can mimic benign lesions, leading to misinterpretation and delayed diagnosis. To address these challenges, it is essential to thoroughly investigate all atypical soft tissue mas

[Radiographic research on skeletal measurements and morphology of normal hand of Japanese. Part One: Adult].

The present investigation was undertaken to develop a standard method of recording the bone measurements of the normal hand for use in comparative studies of genetic abnormalities of the skeletal system affecting the hand. The data for this study are obtained from the radiographs of the hands of normal 461 males and 565 females aged from 20 to 84 years. Measurements of length and width of all the phalanges and metacarpals are recorded. The phalangeal index and metacarpal index of each bone are obtained by dividing the length by the width. Frequencies of brachymesophalangy of the little finger

[Osteitis/Osteomyelitis of the Hand and Wrist].

Osteomyelitis of the hand and wrist is uncommon compared to the infections of the long bones but not rare. There is poor evidence on many answers to questions concerning this disease. This includes careful consideration of the prevalence, pathogenesis, microbiology, diagnostic methods and the conservative, medical and surgical treatment. The emergence of the disease can subdivided in post-traumatic, postoperative, spread from contiguous infections and hematogenous. The individual patient-specific risk must be considered. Early diagnosis and correct management are essential to preserve bony str

An unusual association of ulnar hemimelia with mesoaxial synostotic syndactyly.

Ulnar hemimelia, also referred to as post-axial longitudinal deficiency of the upper limb, is a very rare skeletal anomaly characterized by the partial or complete absence of the ulna. The majority of the reported cases are sporadic and more common in males. The disorder is mostly unilateral, right-sided and incomplete. A slight shortening of the forearm, radial bowing and ulnar-sided hand drift are the anomalies which often accompany ulnar hemimelia. Ulnar hemimelia may also be seen in association with complex wrist and hand anomalies. The absence of post-axial metacarpal and digital bones ar

[A Bizarre Parosteal Osteochondromatous Proliferation - Case Report and Review of Similar Hand Lesions Recorded Since 1983].

Bizarre parosteal osteochondromatous proliferation (BPOP) is an unusual lesion mostly affecting the bones of the hand. The mass grows from the bone surface and consists of cartilaginous, osseous and fibrous tissue. The lesion is commonly under/misdiagnosed and confused with other lesions, mostly the osteochondromas. We present a patient with BPOP that initially confused the practitioner and radiologist in their diagnosis. We discuss the clinical, radiologic and histologic characteristics of BPOP of the hand since its first report in 1983 and present its main differential diagnosis. We reviewed

Foreign-Body Reaction and Osteolysis in Dorsal Lunate Dislocation Repair With Bioabsorbable Suture Anchor.

Background: In recent years the use of biodegradable suture anchors for treating tendon and ligament pathology in hand surgery became popular. These materials are biocompatible, radiolucent, and load sharing, as they incrementally transfer load to surrounding bone during the resorption process. Despite these numerous advantages, polyglycolic (PGA) and poly-L-lactic acid (PLLA) have become a problem because of the potential risk for foreign body reactions. Methods: This article presents a case of an intraosseous foreign body reaction and massive osteolysis of the proximal carpal after dorsal lu

The contributions of microvascular surgery to emergency hand surgery.

Microvascular surgery has revolutionized the field of emergency hand surgery in the areas of replantation and microvascular transplantation. Amputated parts from as small as a distal phalanx to the entire extremity can now be successfully replanted and unreplanted, or missing parts can be restored and reconstructed with immediate microvascular transplants (MVTs). MVTs can be used to cover exposed vital structures and replace missing complex parts. Part or whole digits can be reconstructed, with up to 4 toe transplants. Joints, bones, and muscles can be replaced with osteocutaneous and neuromyo

Giant cell tumor of distal phalanx in an adolescent with Goltz-Gorlin syndrome.

We report on a unique case of a young female patient with the Goltz-Gorlin syndrome who developed a giant cell tumor of bone in the distal phalanx of the thumb. This case is noteworthy because of the combination of some unusual features. Firstly, it is only the fifth case report on the association of giant cell tumor of bone and the Goltz-Gorlin syndrome. Also the localization of the lesion in the bones of the hand and the presentation at adolescent age is rarely seen.

Xeroradiography in beta-thalassaemia.

Xeroradiographic investigations of the skull, hand, and elbow were performed on 27 patients with homozygous beta-thalassaemia. The results were compared with plain radiographic examinations. Xeroradiography, because of its technical properties (i.e. edge contrast enhancement and wide latitude), was shown to demonstrate cortical thinning of long bones, swelling of the diploic space in the skull, and reticulated patterns in the elbow better than standard radiography. Moreover, the use of "positive" mode imaging was shown to have advantages in the study of the skull and extremities.

Pathology of orbital bones. The XXXII Edward Jackson Memorial Lecture.

The orbital bones may show nearly all the pathologic changes observed in the skull and in the face. The congenital anomalies in this area are numerous and involve various forms of craniostenoses. Among the benign osseous tumors the osteoma is most frequently encountered in the orbit. Fibrous dysplasia is a tumefaction of indeterminate behavior that often involves the orbit. Osteosarcoma or other malignant neoplasms are rarely seen in this area. Eosinophilic granuloma and Hand-Schüller-Christian disease are tumor-like lesions that may involve the orbit.

Friction burn injuries to the dorsum of the hand after car and industrial accidents: classification, management, and functional recovery.

A total of 65 cases of friction burn injuries to the dorsum of the hand after car and industrial accidents were reviewed. The mean age of the study population was 31 years (range 18-68 years). The injury sustained from friction was graded into four grades: grade I (n = 6), isolated second-degree skin burn, grade II (n = 6), full-thickness skin burn without extensor tendon exposure, grade III (n = 45) the friction injury affected and exposed the extensor tendons, and grade IV (n = 8) the friction injury extended to bones and joints. Management was by dressing, split skin grafting, local/pedicle

Extended-Hours Hemodialysis without Dietary Restrictions Is Associated with Lower Risk for Developing of Dialysis-Related Amyloidosis.

Dialysis-related amyloidosis (DRA) is characterized by the deposition of amyloid consisting of beta2-microglobulin in the musculoskeletal system, causing carpal tunnel syndrome, destructive spondyloarthropathy, and/or bone cysts. Increased cystic radiolucency of the bones and tendon thickening due to inflammation are common findings in DRA. We have developed a new dialysis method, extended-hours hemodialysis without dietary restrictions for the aim of improving both hypertension and malnutrition. We retrospectively evaluated the clinical effects of dialysis time on the risk for developing of D

Clinical study of the factors affecting radioulnar deviation of the wrist joint.

BACKGROUND: The radioulnar carpal joint is critical for hand and wrist function. Radioulnar deviation indicates distal radioulnar joint flexibility and reflects the structure and function of the carpal bones, ulna, radius and ligaments. The present study examined whether radioulnar deviation is affected by gender, manual labor, playing a musical instrument, playing sport, handedness, previous fracture or prior inflammation. The study used clinical findings based on anatomical landmarks METHODS: The ulnar, radial and total deviations for both left and right hands were measured in 300 subjects

Construction and Validation of a Simplified Bone Age Assessment Method Based on Three Key Bones from the TW3 system.

BACKGROUND: The Tanner-Whitehouse 3 (TW3) method is a widely used standard for bone age (BA) assessment. Although the TW3-China (TW3-C) version was developed specifically for the Chinese population, its complexity hinders routine clinical use. OBJECTIVES: To construct a simplified TW3-based BA assessment method using only three key bones in order to assist pediatricians in quickly assessing BA in Chinese children. METHODS: A total of 2,622 eligible healthy children aged 6–18 years were selected from a population-based cross-sectional study in Beijing (2022–2023). Left-hand wrist radiographs we

Ultrasonographic Diagnosis of Finger Flexor Tendon Hypoplasia in a Child with Phalangeal Agenesis.

Agenesis and hypoplasia affecting multiple flexor tendons within the same hand represent an exceedingly uncommon occurrence, with no previous studies addressing this condition. This report details a 4-year-old girl with agenesis of the right third and fourth fingers, who sought consultation due to the inability to flex her seemingly unaffected second and fifth fingers. Ultrasound examination revealed substantial thinning of the flexor tendons in the second to fifth digits, with a notable absence of attachment to the middle phalanx. In addition to flexor tendon hypoplasia, hypoplasia of the thi

[Frostbite of the upper and lower limbs in an expert mountain climber: the value of bone scan in the prediction of amputation level].

A 38 year old man was admitted to our hospital 10 days after suffering a frostbite injury in hands and feet while practicing mountain climbing, at 8,100 meters of altitude, while he was trying to reach the top of the K2 mountain. A 99mTc-MDP bone scan performed in aseptic conditions showed: in hands: absence of bone uptake in the 3rd phalanx and distal portion of 2nd phalanx of the 5th finger of the left hand, and multiple areas of increased uptake in the distal portion of both hands. In feet: uptake decreases in the 2nd phalanx of the first toe of the left foot, and absence of bone uptake in
